Chronic distress, age-related neuropathology, and late-life dementia.

OBJECTIVE: The objective of this study was to test whether common age-related neuropathology could account for the relation of chronic distress to dementia. METHODS: In a selected cohort of more than 1000 older Catholic clergy members undergoing annual clinical evaluations, 326 persons died, of whom 306 (94%) underwent brain autopsy, the results of which were available in 219 (mean age at death = 85.4, standard deviation [SD] = 6.6; mean postmortem interval = 7.6 hours, SD = 6.9). A composite measure of chronic distress was constructed from standard measures of two traits, neuroticism and anx…

Lethal African trypanosomiasis in a traveler: MRI and neuropathology.

The authors report a case of human African trypanosomiasis with CNS involvement caused by Trypanosoma brucei rhodesiense in a 52-year-old woman, which relapsed after melarsoprol treatment. After a second regimen, she developed a severe toxic polyneuropathy, progressing to coma and eventually death. MRI revealed rapidly progressive multiple white matter lesions as well as damage of the central gray matter and cortex. The autopsy results confirmed the diagnosis of human African trypanosomiasis.

New insights on the neuropathology of West Nile virus.

West Nile virus (WNV) is a mosquito-borne disease that emerged in North America where it caused in 2002 te largest arboviral meningoencephalitis outbreak ever recorded in this area. The viral variant responsible for this outbreak has been found to share 99.7% identity over the entire genome with the viral variant that caused the epizootic in Israel in 1998 and has been referred as "Isr98/NY99". It has been shown to exhibit an increased neurovirulence in humans, as well as in experimental infections in different animal models. Mouse model has allowed to demonstrate the preferential infection of

HIV-1/AIDS neuropathology in a Canadian teaching centre.

BACKGROUND: The nervous system is a major target of HIV-1 infection and site of many complications of AIDS. Most of our knowledge pertaining to the range and frequency of neuropathology in HIV-1/AIDS is from large centres outside Canada in different social and health care settings. The goal of the present study was to describe HIV-1/AIDS-associated neuropathology before and during the era of highly active antiretroviral therapy at a Canadian teaching centre. METHODS: The records of the Department of Pathology, London Health Sciences Centre were electronically searched for cases of HIV-1/AIDS …

New findings on the neuropathology of multiple system atrophy.

Multiple system atrophy (MSA) provides a typical example of the integrative role of the central autonomic network in controlling cardiovascular, respiratory, bladder and gastrointestinal functions. There is increasing evidence that neurochemically defined neuronal groups of the brainstem are selectively affected in MSA to a much greater degree than in Parkinson's disease. These include the catecholaminergic neurons of the rostral ventrolateral medulla (C1 group) which project to the intermediolateral cell column and are involved in modulation of sympathetic vasomotor outflow, and noradrenergic

Biomechanics and neuropathology of adult and paediatric head injury.

The objective of this study was to understand the biomechanics in age-related primary traumatic brain injuries (TBI) causing initial severity and secondary progressive damage and to develop strategy reducing TBI outcome variability using biomechanical reconstruction to identify types of causal mechanisms prior to clinical trials of neuro-protective treatment. The methods included the explanation of TBI biomechanics and physiopathological mechanisms from dual perspectives of neurosurgery and biomechanical engineering. Scaling of tolerances for skull failure and brain injuries in infants, childr

Contributions of neuropathology to understanding schizophrenia in late life.

The neurobiological basis of cognitive and functional deterioration commonly observed in elderly persons with schizophrenia is unclear. Despite superficial similarities in the clinical and neuropsychological profiles of schizophrenia in late life with neurodegenerative dementias, extensive neuropathological investigations have failed to find any evidence of neurodegeneration or neural injury beyond what is typically observed in brains of individuals without neuropsychiatric illness. In contrast, growing neuropathological data indicate aberrant brain development and connectivity in schizophreni

[Neuropathology of the new variant of Creutzfeldt-Jakob disease].

Identification in the United Kingdom of cases of Creutzfeldt-Jakob disease with clinical, genetic, neuropathological and phenotype characteristics different from those previously reported, together with its relation to bovine spongiform encephalopathy led to the establishment of a new condition known as a new variant of Creutzfeldt-Jakob disease. In this article we describe the neuropathology and immunohistochemistry of the prion protein involved.

The neuropathology of intestinal failure and small bowel transplantation.

The aim of this study was to elucidate the neuropathological substrate of intestinal failure before and after small bowel transplantation (SBT). Retrospective analysis of complete autopsy or brain biopsy specimens of 17 patients with intestinal failure (12 children and 5 adults) were studied. Patients were divided into two groups. Group I (transplanted group; n = 13) included those patients who underwent intestinal transplantation under tacrolimus and steroids immunosuppressive therapy. Group II (control group) included 4 children with intestinal failure who were candidates for SBT and died wh

Enteric neuropathology: recent advances and implications for clinical practice.

Because of their high prevalence in clinical practice, the field of gastrointestinal motility has tended to focus its clinical and research efforts on such functional disorders as nonulcer dyspepsia, the irritable bowel syndrome, and functional constipation. Because these disorders are difficult to define and their diagnosis remains exclusively symptomatic, progress has been difficult in these areas, and advances in clinical gastrointestinal motility generally have been hampered. This review attempts to emphasize the prevalence and importance of "organic" motility disorders, ie, those disorder

[The neurobiology of alcoholism. Neuropathology and CT/NMR findings].

The authors deal with the heuristic value of the "neurobiological model of alcohol dependence". It allows the study of the influence of a defined noxe on different brain structures. Additionally, it enables the quantification of regeneration and restitution processes in abstinence. Because of this, the alcoholism model goes beyond dementia, the model which has dominated brain research so far. Neuropathological studies in humans and animals found a reduction in the volume of white matter and a partial degeneration, or even loss of specific neurons. According to animal data, this could to a cert

Prion protein transgenes and the neuropathology in prion diseases.

The concept that prions are novel pathogens which are different from both viroids and viruses has received increasing support from many avenues of investigation over the past decade. Enriching fractions from Syrian hamster (SHa) brain for scrapie prion infectivity led to the discovery of the prion protein (PrP). Prion diseases of animals include scrapie and "mad cow" disease; those of humans present as inherited, sporadic and infectious neurodegenerative disorders, two of which are called Creutzfeldt-Jakob disease (CJD) and Gerstmann-Sträussler-Scheinker disease (GSS). The inherited human prio

[Neuropathology of peroxisomal disorders; Zellweger syndrome and neonatal adrenoleukodystrophy].

Neuropathology of peroxisomal disorders showed polymicrogyria in the cerebral and cerebellar cortices, neuronal heterotopia in the cerebral white matter, dysplasia of the inferior olivary nucleus and subependymal cyst in 6 cases of Zellweger syndrome (ZS), and diffuse loss of myelin sheath and mild polymicrogyria in a case of neonatal adrenoleukodystrophy. Developmental immunohistochemistry of catalase, acyl-CoA oxidase and ketoacyl-CoA thiolase revealed that positive reaction appears with neuronal and glial maturation. Diffuse dysmyelination may be related to maldevelopment of oligodendroglia…

Neuropathology of the locus ceruleus: a semi-quantitative study.

The locus ceruleus was studied in 86 brains of the elderly, with or without degenerative disease. Parkinson's disease, multiple system atrophy, progressive supranuclear palsy and senile dementia were among the diseases studied. Nerve cell loss, the appearance of neurofibrillary changes and Lewy bodies were examined semi-quantitatively. The number of nerve cells diminished in old age, especially over 90 years. The decrease of nerve cells was greater in cases with Lewy bodies. A marked loss of nerve cell was observed in multiple system atrophies, including Shy-Drager syndrome, olivopontocerebell

The neuropathology of progressive rubella panencephalitis of late onset.

The clinicopathological changes in a case of progressive rubella panencephalitis of late onset have been presented. The prominent pathological findings in the brain included diffuse destruction of white matter with perivascular inflammatory cells and gliosis, moderate neuronal loss, numerous amorphous vascular deposits in the white matter and severe generalized cerebellar atrophy. These changes are compared and contrasted with those found in congenital rubella and other latent viral illnesses.
