Retrospective analysis of central venous catheters in elective intracranial surgery - Is there any benefit?

BACKGROUND: It remains unclear whether the use of central venous catheters (CVC) improves a patient's clinical outcome after elective intrac … atment. CONCLUSION: The data of our retrospective study suggests that patients undergoing elective neurosurgical procedures with CVCs do not demonstrate any additional benefits in comparison to patients without a CVC.

Neurosurgical emergencies during pregnancy - Management dilemmas.

BACKGROUND: Neurosurgical emergencies in the obstetric setting pose considerable challenges. Decision-making involves deliberations on the gestational age, critical nature of the illness, timing of surgery, maternal positioning during neurosurgery, anesthesiologic strategies, monitoring of the pregnancy during surgery, and the mode of delivery. The present study discusses the management and ethical dilemmas encountered during the management of six obstetric patients with neurosurgical emergencies. METHODS: A retrospective review of all neurosurgical operations performed between January 2016 a

Implementation of ORBEYE®-Exoscope in the Operative Treatment of Spinal Dural Arteriovenous Fistula.

Spinal dural arteriovenous fistulas (sDAVFs) are rather uncommon lesions of the spine. In sDAVFs, which represent the most frequent form of vascular malformations of the spine, operative treatment remains the most common treatment modality. In operative surgery, visualization and pathology detection have a key impact on the results of the neurosurgical treatment of an sDAVF. The exoscope is one of the most recent imaging devices developed alongside the microscope and endoscope. The exoscope is being increasingly applied in neurosurgical procedures as an alternative to operative microscopes due…

Development and validation of a nomogram for suspected post-neurosurgical bacterial ventriculitis/meningitis.

OBJECTIVE: The diagnosis of post-neurosurgical bacterial ventriculitis/meningitis (BV/M) remains challenging, particularly in the patients with leukocytic pleocytosis and hypoglycorrhachia. This study aimed to establish a nomogram identifying the high-risk and low-risk post-neurosurgical BV/M and evaluate the utility of this risk scoring system. METHODS: Adult patients with CSF leukocytes ≥ 100/mm3 and glucose ≤ 2.2 mmol/L experienced neurosurgical or invasive procedures in three hospitals were divided into training and validation cohort, patients from Medical Information Mart for Intensive C

Prophylaxis for venous thrombo-embolism in neurocritical care: a critical appraisal.

Venous thrombo-embolism (VTE) is frequently encountered in critically ill neurological and neurosurgical patients admitted to intensive care units. This patient population includes those with brain neoplasm, intracranial hemorrhage, ischemic stroke, subarachnoid hemorrhage, pre- and post-operative patients undergoing neurosurgical procedures and those with traumatic brain injury, and acute spinal cord injury (SCI). There is a wide variability in clinical practice for thromboprophylaxis in these patients, in part due to paucity of data based on randomized clinical trials. Here, we review the cu…

Single-stage deformity correction in children with neurogenic kyphoscoliosis: nuances and a proposal for risk stratification.

OBJECTIVE Neurogenic kyphoscoliosis in pediatric patients has varied causes and diverse management options. The most common management strategy is to stage the orthopedic and neurosurgical aspects of the correction at an interval of 3-6 months from each other. The aim of this study was to report the authors' experience with correction of deformity in children with associated neurological abnormalities requiring intervention as a single-stage surgical treatment. METHODS Of 591 pediatric patients with scoliosis treated at the Amrita Institute of Medical Sciences and Research Centre between Janua

Neurosurgical Management of Interspinous Device Complications: A Case Series.

BackgroundBest practice guidelines for treating lumbar stenosis include a multidisciplinary approach, ranging from conservative management with physical therapy, medication, and epidural steroid injections to surgical decompression with or without instrumentation. Marketed as an outpatient alternative to a traditional lumbar decompression, interspinous process devices (IPDs) have gained popularity as a minimally invasive stabilization procedure. IPDs have been embraced by non-surgical providers, including physiatrists and anesthesia interventional pain specialists. In the interest of patient s

Intrawound vancomycin powder for infection prevention in neurosurgical patients: a retrospective cohort study.

BACKGROUND: Surgical site infections (SSIs) are the most frequent healthcare-associated infections in surgical patients and are largely preventable. Neurosurgical procedures are particularly vulnerable due to inherent risk factors. While intrawound vancomycin has been extensively studied in spinal surgery, evidence in cranial procedures remains limited. METHODS: We performed a retrospective cohort study of 741 patients undergoing cranial or spinal neurosurgery between November 2014 and February 2024. The primary outcome was the 90-day SSI rate, defined in accordance with CDC surveillance crit…

Skeletal Muscle Relaxants and Their Impact on Intracranial Pressure in Neurosurgery.

Skeletal muscle relaxants have their place in everyday use in numerous anesthesiological procedures, such as preparing a patient for surgery … due to kidney or liver dysfunction, age, and sex. Skeletal muscle relaxants have also been used in neurosurgical procedures. It is believed that these drugs do not cross the blood-brain barrier. By reducing intrathoracic pressure and central venous pressure, they can lower intracranial pressure. However, in some studies, an increase in intracranial pressure has been obse …

Enhancing the Microneurosurgical Training: Development of the Folded Placenta Model for Simulation of the Deep Operative Field Challenges of Cranial Procedures.

BACKGROUND AND OBJECTIVE: Neurosurgery relies heavily on advanced manual skills, necessitating effective training models for skill development. While various models have been utilized, the human placenta has emerged as a promising candidate for microneurosurgical training due to its anatomical similarities with cerebral vasculature. However, existing placenta models have primarily focused on simulating superficial procedures, often neglecting the complexities encountered in deep operative fields during cranial surgeries. METHODS: This study obtained ethical approval and implemented a modified

Using robotics to move a neurosurgeon's hands to the tip of their endoscope.

A major advantage of surgical robots is that they can reduce the invasiveness of a procedure by enabling the clinician to manipulate tools as they would in open surgery but through small incisions in the body. Neurosurgery has yet to benefit from this advantage. Although clinical robots are available for the least invasive neurosurgical procedures, such as guiding electrode insertion, the most invasive brain surgeries, such as tumor resection, are still performed as open manual procedures. To investigate whether robotics could reduce the invasiveness of major brain surgeries while still provid…

Ceftizoxime versus vancomycin and gentamicin in neurosurgical prophylaxis: a randomized, prospective, blinded clinical study.

In a prospective, randomized, blinded study, 826 patients undergoing clean neurosurgical procedures received single intravenous doses of ceftizoxime (2 g) (n = 422) or a combination of vancomycin (1 g) and gentamicin (80 mg) (n = 404) 1 hour before an incision was made. Patients with infected or contaminated wounds and those receiving shunts or other implants were excluded. Primary wound infections occurred within 30 days in five patients in each group and were most common after spinal surgery and procedures through previous incisions. Secondary infections (pneumonias, urinary tract infections…

Swedish trial on embolization of middle meningeal artery versus surgical evacuation in chronic subdural hematoma (SWEMMA)-a national 12-month multi-center randomized controlled superiority trial with parallel group assignment, open treatment allocation and blinded clinical outcome assessment.

BACKGROUND: Chronic subdural hematoma (cSDH) is one of the most common neurosurgical disorders and the incidence is rising. The routine treatment is neurosurgical hematoma evacuation, which is associated with recurrence rates up to 10-25%. In recent years, endovascular embolization of the middle meningeal artery (eMMA) has garnered much attention due to recurrence rates as low as < 5%. Several randomized controlled trials are planned or ongoing. In most of these trials, conventional neurosurgical treatment with or without adjunctive endovascular embolization is compared. The proposed trial aim

[Pain therapy in tumor patients and in palliative medicine. 2: Invasive measures].

Anesthesiological and neurosurgical methods in the treatment of cancer pain have to be considered as parts of a holistic approach. To treat … ctive pain treatment all influencing variables have to be taken into account. Anesthesiological and neurosurgical procedures are only a part of the possible and necessary treatment options. Especially before using one of the invasive methods described here, it seems imperative to involve the patient in the process of decision making more closely than currently practiced.
