Appearance of melanocytic nevi on the backs of young Australian children: a 7-year longitudinal study.

To determine the rate of appearance of nevi in a cohort of children between ages 6 and 12 years. The number of nevi has been established as the strongest known risk factor for melanoma, but whether the rate at which nevi appear during childhood varies by age is not well understood. The study involved analysis of nevus development over time in a cohort of 640 Western Australian school children who formed the control group in the Kidskin intervention trial. Children were assessed at ages 6, 10 and 12 years, with nevi on the back counted from photographs. The changes in both number and density of

Congenital Cutaneous Neurocristic Hamartoma of the Scalp with Poor Melanin Content.

Cutaneous neurocristic hamartoma (CNH) is a rare tumor that arises from cells of neural crest origin. This tumor typically proliferates within the dermis or subcutaneous adipose tissue and usually exhibits pigmentation, which requires differentiation from conditions such as blue nevus and congenital nevus. We report CNH in a 5-month-old female infant. A subcutaneous mass in the parietal region was identified 4 days after birth. Imaging studies revealed that the subcutaneous mass was approximately 7 cm in diameter and involved the intracranial space. An open biopsy specimen obtained at 3 months

[Unusual and unknown aspect of cutaneous malignant melanoma: minimal deviation malignant melanoma. Retrospective study of 45 cases].

Minimal deviation malignant melanoma (MDMM) is a rare melanocytic tumor of the skin that shares both malignant and benign histologic features: 1) a vertical growth phase similar to that of malignant melanomas with expansile nodules invading throughout the reticular dermis (Clark's level IV), and even the subcutaneous fat (Clark's level V); 2) but a monotonous and uniform proliferation of melanocytic cells that are only moderately atypical. A very slight cellular maturation from the top to bottom of the lesions may be observed, but usually there is no maturation at all. By themselves, the cells

[Blue rubber bleb naevus syndrome involving the pleura].

Bean's syndrome ('blue rubber bleb nevus syndrome') is a rare disease characterized by venous malformations involving various organs. Most often these lesions are localized to the skin and the digestive system. Gastro-intestinal bleeding is the most frequent presentation. Though other organs can be affected, chest localizations are infrequent and pleural localization is exceptional. We report the case of an asbestos-exposed patient with Bean's syndrome with characteristic skin lesions, smoker, hospitalized for the investigation of a hemorrhagic pleural effusion. A medical thoracoscopy revealed

[Neurocutaneous syndromes with vascular alterations].

There are several syndromes in which neurological and cutaneous alterations of vascular origin, among other symptoms, occur. The key point of this fact is that these cutaneous signs permit early diagnosis, thus helping in further recognition of more complex syndromes and preventing unnecessary, harmful and costly diagnostic procedures or having to wait until the appearance of neurological signs. Therefore, these diseases should be classified attending to the most notorious vascular lesions they show, though they may show other less frequent cutaneous vascular lesions. In this way, these syndro

Problems of Differential Diagnosis in Melanoma Arising from Blue Naevus.

Melanoma arising from blue naevus is an exceedingly rare variant of melanoma. Most of the cases are located in head and neck area, with scalp the most affected site. This type of melanoma develops on blue naevus or resembles blue naevus, most frequently cellular blue naevus. We present the case of a 52 years old female diagnosed with a pigmented melanoma of the scalp, with maximum tumoral thickness of 6.8 mm and invasion of the hypodermis, highly resembling cellular blue naevus. All criteria of malignancy were present: prominent nuclear pleomorphism, infiltrative pattern, tumoral necrosis, hig

Dermoscopic Features of Small, Medium, and Large-Sized Congenital Melanocytic Nevi.

BACKGROUND: Congenital melanocytic nevi (CMN) are present at birth. It is well known that the presence of large-sized congenital nevus in early life could predict a major risk of developing melanoma. OBJECTIVE: To investigate the clinical and dermoscopic features of the CMN, to search for and highlight any differences between small-sized, medium-sized, large-sized CMN. METHODS: A nonrandomized observational study was performed. A total of 108 melanocytic nevi were analysed by clinical and dermoscopic examination. RESULTS: Of the subjects, 57.4% were aged less than 16 years, 42.6% were aged

A graphite foreign body granuloma that caused palatal perforation. Case report and literature review.

BACKGROUND: We present an unusual case of a graphite foreign body granuloma causing palatal perforation. CASE DESCRIPTION: A 62-year-old female presented with a macule on the hard palate clinically consistent with a blue nevus. On biopsy a black nodular mass was excised, establishing oroantral communication that was verified by a computed tomography scan. A diagnosis of malignant melanoma was strongly suspected, but microscopic examination showed a graphite foreign body granuloma. It was suggested that the graphite was implanted in a thin area of the palatal bone causing perforation. CONCLUS

The spectrum of grossly visible pigmented lesions in the uterine cervix: a prospective study.

Pigmented lesions of the uterine cervix (UC) have not been systematically studied in the literature. Over an 18-mo period, we prospectively investigated the histologic spectrum of all macroscopically visible pigmented lesions of the UC. The incidence of pigmented UC was 1.6% (33/2118). Histologic examination revealed 32 cases (97%) with a histologic correlate, of which 26 lesions were of melanocytic nature including 25 blue nevi (BNs) (81%) and 1 melanotic macule (3%). The nonmelanocytic lesions included 1 case of focal granulomatous vasculitis (3%), 2 biopsy site-associated reactive changes w

Large plaque-type blue naevus with subcutaneous cellular nodules.

We report a new case of 'large plaque-type blue naevus (PTBN) with subcutaneous cellular nodules' involving the breast. A 25-year-old-woman presented with a large plaque, 140 x 100 mm in size, on the right chest wall involving the right breast, associated with recent subcutaneous nodules. Histopathological examination revealed features of cellular and common blue naevus. Dermatoscopy was performed, and the diagnosis of PTBN was made. PTBN is a rare condition with an imprecise prognosis. In cases of PBTN lesions on the head, clinicians must keep in mind the risk of malignant transformation of P

A new neurocutaneous syndrome possibly related to Ota's nevus.

The case of a 42-year-old Sudanese man is reported who had developed hyperpigmentation of the left face and rapidly progressive right hemiparesis followed by myoclonus at the age of 30. Blue-grey hyperpigmentation was noted in the territory of the left ophthalmic, maxillary and mandibular trigeminal nerve branches, with alopecia of the right face and axilla. CT of the brain revealed left-sided paraventricular calcifications and MRI showed atrophy of the left cerebral hemisphere and mesencephalon, a calcified haemangioma in the left basal ganglia, an old infarct in the right central pons, and c

Bean syndrome in childhood: Use of sirolimus in a severe case.

Bean syndrome, also known as blue rubber bleb nevus syndrome, is a rare congenital disorder characterized by multiple venous malformations affecting the skin, soft tissues, and gastrointestinal tract. Its main complication is chronic gastrointestinal bleeding, which can lead to iron-deficiency anemia and a lower quality of life. We present the case of a pediatric patient diagnosed with Bean syndrome, in whom typical skin lesions and gastrointestinal involvement were identified. We describe the clinical course, the diagnostic tests performed, and the treatment regimen initiated with sirolimus.

An unusual cause of gastrointestinal bleeding in a hemodialysis patient.

Iron deficiency caused by gastrointestinal (GI) bleeding is a common cause of anemia in hemodialysis patient. Herein, we report the case of an 89-year-old hemodialysis patient who presented with acute anemia and melena. Endoscopy found gastritis and diverticulosis without active bleeding. A capsule endoscopy (CE) was then performed and revealed multiple vascular lesions that lead to the diagnosis of Blue Rubber Bleb Nevus syndrome (BRBNS). This rare disease is associated with multiple venous malformations in the skin and the GI tract, usually observed in children. The patient developed 4 month

Surgical decision criteria: Bednar tumour of the foot in a child.

An 8-year-old boy was admitted for excision of a putative 'blue nevus' on the left foot. Histological examination and immunohistochemistry revealed a Bednar tumour, the pigmented variant of dermatofibrosarcoma protuberans. Surgical options considered by a multidisciplinary team included wide local excision, Mohs micrographic surgery or a staged excision with examination of several histological sections. The third alternative procedure was chosen after consideration of tumour and patient factors to achieve the best possible clinical, cosmetic and functional outcome. After the final surgical pro

Epibulbar lesions simulating extraocular extension of uveal melanomas.

Twelve patients who were referred to the Oncology Service of Wills Eye Hospital, Philadelphia, with a pigmented epibulbar lesion that clinically resembled malignant melanoma of the choroid with extraocular extension were found to have other ophthalmic entities. The most common conditions that simulated extraocular extension of uveal melanoma were conjunctival malignant melanomas, which accounted for four of the 12 pseudomelanomas. Three of the 12 patients had staphylomas or scleral ectasis. Two of the 12 had subconjunctival hematomas. Senile scleral plaques, a presumed cellular blue nevus, and

Three Types of Nodal Melanocytic Nevi in Sentinel Lymph Nodes of Patients With Melanoma: Pitfalls, Immunohistochemistry, and a Review of the Literature.

The presence or absence of metastasis in sentinel lymph nodes often drives melanoma staging, prognosis, and treatment. However, distinguishing between metastatic melanoma cells and clusters of benign melanocytic nevus cells is not always straightforward. When morphologic hematoxylin and eosin interpretation alone is not sufficient, additional hematoxylin and eosin sections and immunohistochemical (IHC) studies may be beneficial. This review and small cases series of 3 diagnostically challenging melanocytic sentinel lymph node cases highlights the IHC approach to evaluate intraparenchymal nodal

Small-cell epidermoid carcinoma with melanocyte colonization arising in a mature ovarian cystic teratoma.

A case of small-cell epidermoid carcinoma, a rare tumor of the aerodigestive tract, is presented arising in a mature cystic teratoma of the ovary in a 69-year-old woman. Microscopically the small-cell epidermoid carcinoma had a uniform cell population with focal differentiation into keratinous cells. Both the small and keratinized cells were positive for epithelial membrane antigen and CAM 5.2 but negative for vimentin and neuroendocrine markers. This tumor is different from other recently described primary and metastatic small-cell tumors of the ovary. Furthermore, this small-cell epidermoid

Naevus varicosus osteohypertrophicus. An early diagnostic approach.

Naevus varicosus osteohypertrophicus (synonym Klippel-Trenaunay Syndrome KTS) is relatively rare circumscribed, usually quadrant-related gigantism with vascular hyperplasia or malformations based on the embryonic development. The authors observed an 18- and a 30-year-old female with a triad of symptoms: cutaneous nevus flammeus, varicose and dilated veins, and bony and soft tissue hypertrophy of the low limb. The second patient also had two venous ulcers as a dominant clinical feature--a rare manifestation of Klippel-Trenaunay syndrome. A diagnosis of Klippel-Trenaunay syndrome was made by cli
