Based on neuroimaging data showing absence of the trochlear nerve, congenital superior oblique palsy is now classified as a congenital cranial dysinnervation disorder. A similar absence of the abducens nerve is accompanied by misinnervation to the lateral rectus muscle from a branch of oculomotor nerve in the Duane retraction syndrome. This similarity raises the question of whether some cases of Brown syndrome could arise from a similar synkinesis between the inferior and superior oblique muscles in the setting of congenital superior oblique palsy. This hypothesis has gained support from the c

We experienced a case of DiGeorge syndrome with left internal carotid artery absence probably causing one-and-a-half syndrome. MR angiogram demonstrated the apparent absence of the left internal carotid artery and consequently abnormal blood supply to the left middle cerebral artery, which was derived from the basilar artery via the left posterior communicating artery. The patient alsoshowed both an extremely narrow carotid canal on the left side and a very fine vessel extending to the terminal of the left internal carotid artery. Therefore, we regarded this abnormality as severe hypoplasia of

OBJECTIVE: To assign more specific pathophysiologic processes to the protean patterns of extraocular muscle "overaction" that we see in clinical practice.
METHODS: By extrapolating from known principles of striated muscle physiology, a cohesive theory about extraocular muscle behavior is derived.
RESULTS: The key to understanding apparent extraocular muscle overaction is to differentiate between a muscle that has decreased elasticity and one that is strengthened. Primary inferior oblique muscle overaction has the characteristics of a muscle that primarily has decreased elasticity, the superi

Familial idiopathic scoliosis with conjugate gaze palsy is a rare autosomal recessive disorder characterized by progressive scoliosis and congenital absence of conjugate horizontal eye movement. In this article, two siblings who had scoliosis and conjugate gaze palsy are presented regarding clinical and laboratory findings.

Brown's Syndrome was initially described as a superior oblique tendon sheath syndrome-a short anterior tendon resulting in a restricted elevation of the globe in the nasal field. Brown believed that a congenital paralysis of the inferior oblique muscle resulted in this secondary shortening of the anterior sheath of the superior oblique tendon. The definition of Brown's Syndrome has changed over time. It is currently defined as the inability to elevate the eye in the adducted position, both actively and passively on force duction testing, and can be acquired. The pathophysiology of acquired Bro

BACKGROUND: A complex partial seizure can cause a variety of visual system signs and symptoms, including visual hallucinations, dilated pupils, and changes in vision. Little information is known about the influence of this disorder on the visual system during nonseizure moments. This case report examines the unusual eye movements-during these nonseizure times-of a patient diagnosed with complex partial seizure disorder.
CASE REPORT: An 8-year-old boy was referred to the clinic by his pediatric neurologist for a comprehensive examination to rule out a visual cause for the abnormal eye movement

The different pathogenic and clinical aspects of the paraneoplastic ophthalmological syndromes are discussed by the author. Opsoclonus-myoclonus, retinal degeneration are largely reviewed. It is possible that new modern immunological methods to elucidate the role of antibodies in the genesis of paraneoplastic syndrome and the role of these antigens in the functioning of nervous system. Recognising a ophthalmological syndrome as paraneoplastic will prompt a search for an early detection of a cancer and a more prompt treatment.

A patient with a coup de sabre lesion of the forehead developed progressive ipsilateral limitation of ocular motility, primarily involving adduction and depression. Investigation disclosed no other explanation for the ocular motility disturbance, which we suspect represents restrictive myopathy maximally involving ocular muscles immediately subjacent to the area of linear scleroderma.

We report a case of bilateral complete failure of adduction following bilateral translid antralethmoidal orbital decompression. We believe the probable mechanism is neuropraxia (temporary dysfunction) of the third cranial nerves' supply to the medial recti, owing to these nerves' occupying an anatomically abnormal position. Partial recovery of adduction occurred over the ensuing six months.

Two adult patients with opsoclonus and tremor of the whole body associated with viral infections are reported. The first man presented with mumps (parotitis, orchitis and encephalitis). Paired serum mumps titers were both 1:80. The second patient had conjunctivitis and dizziness. Acute and convalescent sera showed significant rise of poliovirus type 3 titer. Clonazepam attenuated the symptoms in both patients.

BACKGROUND: Ocular manifestations of thyroid dysfunction constitute a wide clinical spectrum ranging from minor ocular discomfort, lid retraction, lid lag and ocular injection, to sight threatening eyeball protusion and optic nerve compression. Thyroid-related eye disorders are most commonly associated with Graves' disease, and this most frequently occurs in the setting of hyperthyroidism. However, in 10% of cases, typical eye signs have also been reported in euthyroid and hypothyroid states. The severity of thyroid eye disease has been linked to cigarette smoking. There is very little data sp

Superior oblique myokymia (SOM) is an uncommon ocular motility disorder characterized by intermittent uniocular vertical or torsional oscillations with periodic remissions and exacerbations. Although generally idiopathic in origin, isolated case reports have documented its association with intracranial pathology. We present a case of superior oblique myokymia associated with a dural arteriovenous fistula.

An increased compendium of drugs useful in ocular motor system dysfunction has expanded our capacity to treat selected ocular motility disorders. Adjunctive therapeutic modes (e.g., Fresnel prisms and orthoptic exercises) can also be beneficial. PAN and see-saw nystagmus can be treated with baclofen. Downbeat nystagmus may respond to clonazepam therapy, and prisms may help if the nystagmus can be modified with convergence. Congenital nystagmus may respond minimally to drugs (e.g., baclofen), but prisms or surgical procedures, or both, are still the primary treatment modalities. Innovar may be …

Cranial nerves innervate head muscles in a well-characterized and highly conserved pattern. Identification of genes responsible for human congenital disorders of these nerves, combined with the analysis of their role in axonal development in animal models, has advanced understanding of how neuromuscular connectivity is established. Here, we focus on the ocular motor system, as an instructive example of the success of this approach in unravelling the aetiology of human strabismus. The discovery that ocular motility disorders can arise from mutations in transcription factors, including HoxA1, Ho…
