Late salvage of failed open reduction and internal fixation of posterior wall fractures of the acetabulum.

OBJECTIVES: The purpose of this study was to review the outcome of late revision surgery in a series of patients with posterior wall fractures having recurrent hip instability after failed initial open reduction and internal fixation. DESIGN: Retrospective analysis of consecutive patients. SETTING: Level 1 trauma center. PATIENTS: A retrospective analysis of consecutive acetabulum patients treated by a single surgeon between 1989 and 2005 identified 4 patients referred for recurrent hip instability subsequently treated by revision open reduction and internal fixation performed 3 weeks or mo

Open reduction and internal fixation with 90-90 plating of bicolumn distal humeral fractures.

A bicolumn intra-articular fracture is the most challenging distal humeral fracture to treat. Open reduction and internal fixation of these fractures is best achieved using either 90-90 medial-lateral or parallel opposing plates. Either technique can be used to treat most patients, although in some instances, one technique may be preferred over another. The goal of treatment is accurate anatomic restoration and stable fixation that will permit early range of motion.

[Surgical resection of heterotopic ossification after open reduction internal fixation of acetabular fractures].

OBJECTIVE: To evaluate the clinical effect of surgical resection of the severe heterotopic ossification (HO) after the open reduction internal fixation (ORIF) of acetabular fractures. METHODS: Five cases of severe HO after the ORIF of acetabular fractures were treated by surgical resection from October 2005 to April 2007. All patients were male, the average age was 34 years (22 to 45 years). The average time of HO after ORIF of acetabular fractures was 14.2 months (3 to 30 months). The original surgical approaches were: Kocher-Langenbeck approach as 4, ilioinguinal combined K-L approach as 1.

Treatment of select open calcaneus fractures with reduction and internal fixation: an intermediate-term review.

BACKGROUND: Open calcaneal fractures are devastating, high-energy injuries frequently occurring in multiply-injured patients. Unsatisfactory outcomes with significant complications rates are frequently reported. Our purpose was to assess outcomes of delayed ORIF for open calcaneal fractures with medial hindfoot wounds and limited posterior facet comminution. MATERIALS AND METHODS: Following acute management of the open fractures, seven patients were determined to have stable medial hindfoot wounds and posterior facet comminution of three or fewer fracture fragments. The average age was 32.5 y

Soft tissue sleeve approach to open reduction and internal fixation of proximal phalangeal fractures.

Proximal phalangeal fractures often lead to motion loss at the proximal interphalangeal joint primarily because of adhesions between the zone 4 extensor tendon and the fracture site. This most commonly manifests as an extensor lag at the proximal interphalangeal joint, but can also include incomplete recovery of flexion range. The occurrence of the fracture alone likely generates some degree of insurmountable motion loss. Additional motion loss may be iatrogenic based on the specific surgical technique and rehabilitation strategy. It has been promoted that percutaneous pinning methods, as oppo

Open reduction and internal fixation compared with excision for unstable displaced fractures of the radial head.

PURPOSE: To determine if excision of the radial head for treatment of an unstable, displaced fracture is associated with a higher rate of early complications or late arthrosis as compared with open reduction and internal fixation (ORIF). METHODS: Unstable, displaced fractures of the radial head treated with either excision (15 patients) or ORIF (13 patients) were compared. Implants were removed routinely during the study period. Three patients had instability after radial head excision: 2 were treated in a cast and 1 had temporary pinning of the ulnohumeral joint. Two patients in the ORIF coh

Open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus.

BACKGROUND: Controversy persists concerning the preferred treatment of displaced fractures of the proximal part of the humerus. The present study was undertaken to evaluate the results of open reduction and internal fixation of three and four-part fractures of the proximal part of the humerus and the functional limitations of patients in whom avascular necrosis of the humeral head develops as a complication of this fracture. METHODS: We assessed the intermediate and long-term results for sixty patients with a three or four-part fracture of the proximal part of the humerus who had undergone op

Wound-healing risk factors after open reduction and internal fixation of calcaneal fractures.

This retrospective study investigated outcomes of wound healing in a series of 63 consecutive patients with 64 fractures of the calcaneus who underwent open reduction and internal fixation done by two surgeons experienced in this fracture during a 3-year period. Thirty-nine patients were managed preoperatively as outpatient referrals before surgery. Twenty-four patients were admitted directly to the trauma service and were managed as inpatients preoperatively. Minimum patient follow-up was 6 months, with an average follow-up of 18 months. A trend correlating the time between injury and operati

Displaced scapular fractures: indication and long-term results of open reduction and internal fixation.

Displaced scapular fractures are often found in polytraumatized patients. In emergency treatment they assume a minor role. Advances in dealing with severely injured patients in most instances allow us to perform an operation on the fractured scapula within the first 2 weeks after injury. A differentiated approach is necessary as exclusively conservative treatment does not always bring about good results. From 1981-1991 we performed open reduction and internal fixation (ORIF) in 25 patients with displaced fractures of the scapula. The long-term results could be assessed in 20 patients after an

[Distal intra-articular radius fractures. Indications for and technique of open reduction and plate osteosynthesis].

In the past few years we have seen vast changes in both the assessment and the management of fractures of the distal radius. This is attributable to several factors. On the one hand, conservative treatment has shown poor results in a considerable number of patients, and on the other hand, more and more young patients are presenting with this common lesion. Furthermore, the spread of new methods of osteosynthesis and of modern materials, such as implants, etc., have greatly improved the results of operative procedures, even in complicated fractures of the radius. Therefore, in certain types of

Comparison of open versus closed reduction of intraarticular calcaneal fractures: a matched cohort in workmen.

Seventeen displaced intraarticular calcaneal fractures treated operatively were matched with 17 nonoperatively treated displaced intraarticular calcaneal fractures among Workers' Compensation Board patients. Matching criteria included type of fracture, age, year of injury, and occupation. After follow-up averaging 6.3 years and 5.4 years, respectively, no significant differences existed between the two groups in heel pain, subtalar motion, and return to work. However, in those fractures treated operatively, the overall clinical result was better when an anatomic reduction of the subtalar joint

Supracondylar fractures of the humerus in childhood: range of movement following the posterior approach to open reduction.

The posterior approach for open reduction of supracondylar fractures of the humerus has been condemned for causing decreased elbow movement. This study investigates this by comparing the range of movement in children treated by posterior open reduction and Kirschner wiring with those treated by closed reduction and immobilization. A total of 65 children with severely displaced supracondylar humeral fractures have been reviewed. There was some loss of movement in 66 per cent of the open reduction group and 42 per cent of the closed reduction group. The proportion losing more than 10 degrees of

Open reduction and internal fixation of acetabular fractures: heterotopic ossification and other complications of treatment.

Open reduction and internal fixation was performed on 50 displaced acetabular fractures in 49 patients by nine different attending surgeons over a 10-year period. At an average follow-up of 38 months, poor results were noted clinically in 38% and radiographically in 40%. The incidence of short- and long-term complications was greater than in other studies. Particularly distressing was the 58% incidence of heterotopic ossification (HO). Twenty-four percent had grade III or IV; five hips were autofused and the remainder had 40-60% loss of motion. There was no correlation of HO with age, sex, fra

Limited open reduction of the lunate facet in comminuted intra-articular fractures of the distal radius.

Intra-articular incongruity of the distal radius at the radiocarpal joint is a bad prognostic feature; reduction by closed or open operative techniques is important. However, both techniques have limitations. We describe a new technique of reduction of the lunate facet under radiographic control, with very limited operative exposure and tissue trauma. In two cases anatomic restoration of the radiocarpal joint was obtained by this technique and maintained with external and limited internal fixation. Follow-up results are very encouraging. Currently we suggest use of this simple technique when f

Open reduction and internal fixation of Gustilo type-I and type-II open pilon fractures using a lateral approach.

BACKGROUND: Traditional anteromedial incision for pilon fractures would further increase the damage to the subcutaneous tissues anterior and medial to tibia. In this study, we retrospectively evaluated the method and the outcomes of lateral approach for surgical treatment of Gustilo type-I and type-II open pilon fractures with medial soft tissue injuries. METHODS: From May 2014 to September 2017, 35 patients with Gustilo type-I and type-II open pilon fractures were treated with standard protocol using a lateral approach. The initial wound debridement and application of a spanning external fix

External iliac artery thrombosis following open reduction of acetabular fracture by para-rectus approach: A case report.

BackgroundExternal iliac artery thrombosis is a rare vascular complication that can result in a series of serious adverse outcomes. Existing studies on external iliac artery thrombosis after open reduction of acetabular fracture have primarily focused on the classical ilioinguinal approach. However, despite the increasing use of the para-rectus approach for the management of complex acetabular fractures in recent years, no case of external iliac artery thrombosis following this approach have been reported.Case presentation: A 50-year-old female sustained a left acetabular both-column fracture
