[A long-term surviving case with invasive thymoma undergone 4 times operations for tumor recurrences].

We reported a long-term survivor with invasive thymoma associated with myasthenia gravis. The patient underwent operations 4 times for these 13 years. At the third and fourth operation, the tumor was resected after preoperative neo-adjuvant chemotherapy with cisplatin and adriamycin, and postoperative radiotherapy was added. Her postoperative course was uneventful, and she has been alive. Our strategy for invasive thymoma is that tumors which include both primary and recurrent lesions are removed as completely as possible, and that postoperative radiotherapy is added for stage II, III and IV i

The influence of timing of ketorolac administration on post-operative analgesic requirements following total abdominal hysterectomy.

One hundred and thirty-seven patients were studied to assess whether the timing of a dose of ketorolac affected cumulative morphine requirements during the first 12 post-operative hours. Pain, sedation and nausea scores, respiratory rate and degree of operative blood loss were also recorded. Thirty-six patients (group A) were given placebo injections pre- and intra-operatively. Thirty-one patients (group B) received placebo pre-operatively and ketorolac 30 mg intra-operatively. Thirty-six patients (group C) received ketorolac 10 mg pre-operatively and ketorolac 20 mg intra-operatively and thir

Congenital diaphragmatic hernia: determination of the optimal time for operation by echocardiographic monitoring of the pulmonary arterial pressure.

Preoperative stabilization and delayed operation rather than emergency repair of congenital diaphragmatic hernia (CDH) may improve survival, but there are no clear criteria for how long operation should be delayed. Because increased pulmonary vascular resistance (PVR) may be an important risk factor, we used Doppler echocardiography to study patients with CDH presenting with respiratory distress immediately after birth. During the study period 10 patients were admitted, but 2 were moribund on admission and died shortly thereafter. In the remaining patients the pulmonary arterial pressure (PAP)

Real-time Doppler spectrum analysis: predictive value in defining operable carotid artery disease.

A prospective study compared the sensitivity, specificity, and predictive value of indirect periorbital Doppler screening and direct carotid Doppler interrogation using real-time sound spectrum analysis to detect operable extracranial carotid occlusive disease. The results of noninvasive studies were compared with independently assessed contrast arteriograms of carotid arteries of patients studied for symptomatic cerebrovascular disease. Periorbital Doppler ultrasound was insensitive to carotid stenosis of less than 75% and did not distinguish operable stenosis from inoperable occlusion of the

Optimizing first case on-time starts in Veterans Affairs operating rooms: A quality improvement initiative.

Delays in operating room start times lead to inefficiencies, decreased patient satisfaction, and significant hospital costs. FCOTS (first case on-time start) is a widely used performance metric, yet interventions to improve it remain understudied in VA settings. The 4 Disciplines of Execution (4DX) model was implemented to target an 85% on-time start rate. Weekly FCOTS data from January to December 2023 were analyzed using statistical process control charts, and causes of delays were reviewed by a multidisciplinary panel. Among 1467 first-start cases, baseline FCOTS rates averaged 76% (weeks 1

Moving Lumbar Drain Placement to IR From the Operating Room Creates Time and Cost Savings.

OBJECTIVE: Surgeons often place lumbar drains (LD) perioperatively in patients undergoing repair for spontaneous cerebrospinal fluid (CSF) rhinorrhea. We analyzed the cost implications of a transition from operating room (OR) LD placement to a separate Interventional Radiology suite (IR) on the morning of surgery. STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary care center. METHODS: We analyzed patients undergoing endoscopic repair of spontaneous CSF rhinorrhea with LD placement from August 2015 to June 2023. Patient factors and inflation-adjusted perioperative costs were compar

Laser-guided percutaneous microwave ablation for lung nodules: a promising approach with reduced operation time.

BACKGROUND: Pulmonary nodule ablation is an effective method for treating pulmonary nodules. This study is based on the traditional CT-guided percutaneous microwave ablation (MWA) of pulmonary nodules. By comparing laser guidance technology with freehand method, this study aims to explore the safety and efficacy and patients' pain scores of these two approaches. METHODS: This study retrospectively analyzed 126 patients who underwent CT-guided percutaneous lung ablation at the First Affiliated Hospital of Soochow University from April 2020 to April 2024. Based on the guidance method, we divide

Time characteristics of aero-optical imaging degeneration of the ellipsoidal dome under variable operating conditions.

As a missile flies at high speed, the index and surface shape of the conformal dome will vary under the influence of the aero-optical effect, which will degenerate the performance of the seeker's imaging detection system. However, many previous studies on aero-optical imaging deterioration of optical domes were usually carried out under fixed operating conditions, which are not in line with the real flight scene of the missile. In addition, the aero-optical imaging degeneration of the dome is diverse as the flight time increases. Therefore, it is of great significance to study the time charact

Bringing Artificial Intelligence to the operating room: edge computing for real-time surgical phase recognition.

BACKGROUND: Automation of surgical phase recognition is a key effort toward the development of Computer Vision (CV) algorithms, for workflow optimization and video-based assessment. CV is a form of Artificial Intelligence (AI) that allows interpretation of images through a deep learning (DL)-based algorithm. The improvements in Graphic Processing Unit (GPU) computing devices allow researchers to apply these algorithms for recognition of content in videos in real-time. Edge computing, where data is collected, analyzed, and acted upon in close proximity to the collection source, is essential mee

Revisiting destructive operations to prevent second stage caesarean section in COVID times: A case series.

Destructive operations diminish the bulk of the foetus for facilitating vaginal delivery. Procedures such as craniotomy or decapitation can be carried out to deliver a dead baby in appropriately selected cases. Ours is a retrospective case series at a single tertiary facility during the first wave of the COVID pandemic. From July 2020 to January 2021, six destructive operations (five craniotomies and one decapitation) were performed in women who had arrest of descent in the second stage of labour but had intrapartum foetal demise. The average operative time was 30 minutes with a mean hospital

Timely completion of spinal fusion: A multidisciplinary quality improvement initiative to improve operating room efficiency.

BACKGROUND: Failure to complete surgery within the scheduled timeframe impairs operating room efficiency leading to patient dissatisfaction and unplanned labor costs. We sought to improve timely completion (within 30 min of scheduled time) of first-case spine fusion surgery (for idiopathic scoliosis) from a baseline of 25%-80% over 12 months. We also targeted timely completion of perioperative stages within predetermined target completion times. METHODS: The project was conducted in three overlapping phases over 16 months. A simplified process map outlining five sequential perioperative stage

Use of an automated knot fastener shortens operative times in minimally invasive mitral valve repair.

INTRODUCTION: Longer durations of cardiopulmonary bypass and aortic cross clamp are associated with increased morbidity and mortality. Little is known about the effect of automated knot fasteners (Cor-Knot®) in minimally invasive mitral valve repair on operative times and outcomes. The aim of this study was to evaluate whether these devices shortened cardiopulmonary bypass and aortic cross clamp times and whether this impacted on postoperative outcomes. MATERIALS AND METHODS: All patients undergoing isolated minimally invasive mitral valve repair by a single surgeon between March 2011 and Mar

Progression of Aortic Regurgitation After Subarterial Ventricular Septal Defect Repair: Optimal Timing of the Operation.

In patients with subarterial ventricular septal defect (VSD), the progression of aortic regurgitation (AR) still remains unclear. This review is to identify the incidence of AR progression after VSD repair and to determine the optimal operation timing for subarterial VSD repair with or without aortic valve prolapse or AR. From January 2002 to December 2015, 103 patients who underwent subarterial VSD repair alone at our hospital were reviewed. All patients routinely underwent echocardiography (echo) performed by our pediatric cardiologists. The operative approach was through the pulmonary arter

Bariatric Surgery in Women of Childbearing Age, Timing Between an Operation and Birth, and Associated Perinatal Complications.

IMPORTANCE: Metabolic changes after maternal bariatric surgery may affect subsequent fetal development. Many relevant perinatal outcomes have not been studied in this postoperative population, and the risks associated with short operation-to-birth (OTB) intervals have not been well examined. OBJECTIVE: To examine the risk for perinatal complications in women with a history of bariatric surgery (postoperative mothers [POMs]) by comparing them with mothers without operations (nonoperative mothers [NOMs]) and examining the association of the OTB interval with perinatal outcomes. DESIGN, SETTING

A risk-stratified analysis of delayed congenital diaphragmatic hernia repair: does timing of operation matter?

BACKGROUND: Congenital diaphragmatic hernia (CDH) remains a significant cause of death in newborns and, despite improved outcomes with multimodality therapies, optimal timing of repair remains undefined. We sought to evaluate the influence of surgical timing on patient outcomes and hypothesized that delayed repair does not improve survival in CDH. METHODS: Prospectively collected data from 1,385 CDH Registry infants without preoperative extracorporeal membrane oxygen therapy (ECMO) were evaluated. Patients were stratified by timing of repair: Day of life (DOL) 0-3 (group 1), 4-7 (group 2), or

Laparoscopic cholecystectomy techniques with special care treatment in acute cholecystitis patients regardless of operation timing.

BACKGROUND/AIMS: The optimal timing for laparoscopic cholecystectomy (LC) in acute cholecystitis (AC) remains controversial. In this study, we aggressively performed three types of LC techniques in AC patients at different time with our care procedure. METHODOLOGY: Forty-three patients with AC were retrospectively divided into two groups: Group one (n=16, within 72 hrs at the onset of symptoms) and Group two (n=27, beyond 72 hrs after the onset of symptoms). Standard, subtotal or prograde cholecystectomy were performed for all cases, depending on the fragility of gallbladder wall, difficulty
