[Modern principles of ophthalmic surgery for combat trauma: analysis of experience from the Special Military Operation].

Combat ocular trauma presents a complex medical and tactical challenge in modern armed conflicts, it is characterized by a high frequency of combined injuries and requires a specialized system of care. The experience of the Special Military Operation (SMO) provides unique clinical material for analysis and for developing modern surgical principles. This review describes the specific features of combat ocular trauma in the context of the SMO: the predominance of multiple radiolucent orbital foreign bodies, extensive lacrimal drainage system injuries, and a pronounced contusion component. The ar

Reverse 4-Throw Pupilloplasty for Endothelial Keratoplasty.

PURPOSE: The aim of this study was to describe the feasibility and applicability of a reverse 4-throw (RFT) pupilloplasty technique for endothelial keratoplasty. METHODS: In RFT, the 9-0 polypropylene suture needle and a 26-Gauge needle pierce the iris tissue from the posterior surface to emerge on the anterior surface along the proximal and distal portion of iris tissue to be apposed, respectively. The 9-0 needle is threaded into the barrel of a 26-Gauge needle and is withdrawn from the eye. The suture loop is withdrawn and the suture end is passed through the loop 4 times. Both suture ends

Surgery without skin resection for eyelid entropion.

The previously reported surgical methods for eyelid entropion are basically manipulations of the skin or the conjunctival surface. When entropion is marked, manipulations also involve the tarsus. In the most widely used method, the eyelashes are directed outward by horizontal wedge resection of an appropriate amount of the excessive skin in the ciliary vestibule. This method is simple and effective but sometimes causes recurrences, conspicuous scars due to tension of the resection site, or lagophthalmos due to excessive resection. We speculated that partial swelling of the orbicularis muscle a

Controversies in orbital reconstruction-III. Biomaterials for orbital reconstruction: a review with clinical recommendations.

The goal of orbital reconstruction is to repair trauma defects, to correct the position of the eye anatomically, avoiding enophthalmos, and to restore ocular function. For the reconstruction of (trauma) defects, many surgeons recommend materials that can be bent into an anatomical shape and that possess the properties of radiopacity and long-term stability. However, apart from these desired properties, the ideal material for orbital reconstruction remains controversial. Autologous bone is often mentioned as the 'gold standard,' likely because of its mechanical properties, revascularization pot

Dysthyroid optic neuropathy (DON).

Dysthyroid Optic Neuropathy (DON) affects a small percentage of patients with Graves disease, but, when it occurs, it can cause significant and permanent loss of vision. DON is treatable if recognized early. Systemic steroids can be effective, but may cause side affects. Orbital injection of steroids may play a role in selected patients. Orbital radiation has a more permanent effect and has gained wide acceptance as a relatively non-invasive method of reversing DON. Surgery to decompress crowded orbits has been used for years and continues to be a viable approach for those patients with optic

CGBA-Net: context-guided bidirectional attention network for surgical instrument segmentation.

PURPOSE: Automatic surgical instrument segmentation is a crucial step for robotic-aided surgery. Encoder-decoder construction-based methods often directly fuse high-level and low-level features by skip connection to supplement some detailed information. However, irrelevant information fusion also increases misclassification or wrong segmentation, especially for complex surgical scenes. Uneven illumination always results in instruments similar to other tissues of background, which greatly increases the difficulty of automatic surgical instrument segmentation. The paper proposes a novel network

Optic canal decompression: a cadaveric study of the effects of surgery.

PURPOSE: To simulate a transphenoidal medial optic canal decompression and determine the anatomic effect on the optic nerve. METHODS: A medial optic canal decompression was performed on 5 cadaveric optic canals within 12 hours of death. Two canals were decompressed under direct visualization and 3 were decompressed by a transphenoidal endoscopic approach. The optic canal was subsequently removed en bloc, beginning at the annulus of Zinn and extending to the optic chiasm. Each specimen was processed and examined grossly. Serial coronal step sections of the entire length of the intracanalicular

The treatment of pterygium.

The treatment of pterygium is still quite controversial, with various treatments being advocated in the scientific literature. Unfortunately, there are very few well-conducted controlled clinical trials of treatments. However, years of anecdotal and noncontrolled studies have confirmed that some methods, such as bare scleral closure, are no longer acceptable in the treatment of pterygium and that other methods are likely to be more useful. In the future it will be important to develop a grading system, and surgeons will need to be conservative in the treatment of pterygium until such time as a

Neuro-ophthalmology for the pediatrician.

The pediatrician needs to develop some skills in evaluating afferent visual functions and ocular motor abnormalities. He must know some fundamental neuro-ophthalmologic facts to prevent his patients from undergoing unnecessary diagnostic and surgical procedures. In addition, he needs to understand the fundamentals of strabismus and amblyopia, which are briefly considered in this article. First, the most common type of nystagmus in children is congenital nystagmus. These children often have a head turn or tilt. Also, it should be remembered that numerous drugs may cause nystagmus. Second, any c

[Platelet aggregation inhibitors and anticoagulants during ophthalmic interventions].

In ophthalmology many patients undergo surgical treatment who need to take anticoagulant medication due to cardiovascular diseases. The proper handling of these drugs requires both correct assessment of the risk of thromboembolism as well as the rating of the risk of surgery-related hemorrhages. While there are established recommendations for estimation of the risk of thromboembolism based on a large body of prospective randomized trials, data regarding the evaluation of the related complications secondary to ophthalmic surgery are limited. In comparison to other surgical procedures, most inte
