Comprehensive clinical evaluation as an outcome assessment for a graduate orthodontics program.

To supplement the American Board of Orthodontics (ABO) objective grading system (OGS) for posttreatment dental casts and panoramic radiographs, a comprehensive clinical assessment (CCA) method was developed to assess facial form, dental esthetics, vertical dimension, arch form, periodontium preservation, root resorption, and treatment efficiency. The sum of the CCA and the ABO OGS scores was defined as the clinical outcome. To determine a 3-year baseline for treatment outcomes in a graduate orthodontic program, the posttreatment records of 521 consecutive patients were evaluated. The mean ABO …

Let's Get Moving: Surgically Facilitated Orthodontic Therapy to Enhance Orthodontic Outcomes.

Surgically facilitated orthodontic therapy (SFOT) has emerged as an interdisciplinary approach to address the limitations of conventional orthodontics in patients with moderate to severe malocclusion. This article reviews the biologic rationale, indications, diagnostic considerations, and clinical protocols underlying SFOT, with emphasis on regional acceleratory phenomenon-mediated bone remodeling and its impact on treatment efficiency. Integration of cone-beam computed tomography, intraoral scanning, and digital treatment planning enables precise evaluation of dentoalveolar boundaries and per…

Orthodontic pain is related to oral health-related quality of life in orthodontic patients.

INTRODUCTION: Orthodontic pain (OP) is a subjective experience induced by prolonged force on the teeth in fixed orthodontic treatment. OP acts as a predictor of aspects related to oral health-related quality of life (OHRQOL). It is important to understand the extent of the impact of OP on a patient's daily life, especially on stomatognathic function and social impact. OBJECTIVE: To assess the correlation between OP and OHRQOL in fixed orthodontic treatment patients. MATERIAL AND METHODS: The comparative pain rating scale is used to assess pain intensity, while the oral health impact profile-

Dynamics of the oral microbiome during orthodontic treatment and antimicrobial advances for orthodontic appliances.

The oral microbiome plays an important role in human health, and an imbalance of the oral microbiome could lead to oral and systemic diseases. Orthodontic treatment is an effective method to correct malocclusion. However, it is associated with many adverse effects, including white spot lesions, caries, gingivitis, periodontitis, halitosis, and even some systematic diseases. Undoubtedly, increased difficulty in oral hygiene maintenance and oral microbial disturbances are the main factors in developing these adverse effects. The present article briefly illustrates the characteristics of differen

Mechanisms of Osteoclastogenesis in Orthodontic Tooth Movement and Orthodontically Induced Tooth Root Resorption.

Orthodontic tooth movement (OTM) is achieved by the simultaneous activation of bone resorption by osteoclasts and bone formation by osteoblasts. When orthodontic forces are applied, osteoclast-mediated bone resorption occurs in the alveolar bone on the compression side, creating space for tooth movement. Therefore, controlling osteoclastogenesis is the fundamental tenet of orthodontic treatment. Orthodontic forces are sensed by osteoblast lineage cells such as periodontal ligament (PDL) cells and osteocytes. Of several cytokines produced by these cells, the most important cytokine promoting os

[Orthodontic treatment need: the IOTN, or Index of Orthodontic Treatment Need (part 2)].

IOTN is an index of orthodontic treatment need that has been devised on the basis of the available evidences of the deleterious effects of malocclusion on the aesthetic and health of the dentition. It has two components i.e. an aesthetic component and a dental health component. This index is widely used in epidemiological research and by third party payment for determination of those patients who should benefit from public funding.

Orthodontic evolution: an update for the general dental practitioner. Part 2: psychosocial aspects of orthodontic treatment, stability of treatment, and the TMJ-orthodontic relationship.

As a result of recent innovations and improvements, orthodontic treatment has become easier and more efficient to carry out, allowing greater numbers of patients to receive treatment. The main result of orthodontic treatment is improved dental alignment and aesthetics. Treatment has no effect on caries or periodontal disease, and the dental health gain is modest, apart from a very small percentage of destructive malocclusions. Psychological improvements using different psychological parameters show differing results and it is not clear that any psychological gains are long lasting. Social gain

Cost-effectiveness of interceptive orthodontics: a long-term evaluation of early treatment strategies.

OBJECTIVES: Malocclusions occur at high frequencies in children and adolescents. While early orthodontic (interceptive) interventions may reduce the need for later comprehensive treatment, their cost-effectiveness in publicly funded health systems is unclear. This study compares the cost-effectiveness of interceptive orthodontic care and fixed-appliance therapy, focusing on total costs, treatment outcomes, and resource use. METHODS: A decision tree model was developed using observational data on treatment success rates, duration, and appointments to estimate resource use and costs for interce

Camera-Based Monocular Depth Estimation in Orthodontics: Vision Transformer vs. CNN Model Performance.

BACKGROUND: Monocular Depth Estimation (MDE) is a computer vision approach that predicts spatial depth information from a single two-dimensional image. In orthodontics, where facial soft-tissue evaluation is integral to diagnosis and treatment planning, such methods offer new possibilities for obtaining sagittal profile information from standard frontal photographs. This study aimed to determine whether MDE can extract clinically meaningful information for facial profile assessment. METHODS: Standardized frontal photographs and lateral cephalometric radiographs from 82 adult patients (48 Clas…

Growth Prediction in Orthodontics: ASystematic Review of Past Methods up to Artificial Intelligence.

BACKGROUND/OBJECTIVES: Growth prediction may be used by the clinical orthodontist in growing individuals for diagnostic purposes and for treatment planning. This process appraises chronological age and determines the degree of skeletal maturity to calculate residual growth. In developmental deviations, overlooking such diagnostic details might culminate in erroneous conclusions, unstable outcomes, recurrence, and treatment failure. The present review aims to systematically present and explain the available means for predicting growth in humans. Traditional, long-known, popular methods are disc

A surgical perspective on removable orthodontics and unidentified supernumerary teeth: are we aligned?

Patients undergoing orthodontics may be referred for specialist oral surgery services as part of an orthodontic-led treatment plan. Surgical interventions include extraction of teeth not amenable to alignment, such as supernumerary teeth. Aligner treatment is an accessible form of orthodontic treatment which has demonstrated a sustained demand. Aligners present several attractive features for patients, which require balance against risks of treatment through clinical examination and discussion with a qualified dentist. Dentists providing aligner treatment should consider the interface with ora…
