Postural Orthostatic Tachycardia Syndrome (POTS): A Review.

IMPORTANCE: Postural orthostatic tachycardia syndrome (POTS) is a chronic autonomic disorder characterized by excessive orthostatic tachycardia and multisystem symptoms that affects an estimated 0.1% to 1% of the US population. POTS can substantially impair daily functioning and quality of life. OBSERVATIONS: Consensus criteria define POTS by chronic symptoms of orthostatic intolerance accompanied by a sustained heart rate increase of at least 30 beats/min (≥40 beats/min in adolescents aged 12-19 years) within 10 minutes of standing or head-up tilt, in the absence of orthostatic hypotension, …

Cardiovascular autonomic neuropathy in patients with diabetes mellitus.

Cardiovascular autonomic neuropathy associated with diabetes mellitus is caused by an impairment of the autonomic system. The prevalence of this condition ranges from 20% to 65%, depending on the duration of the diabetes mellitus. Clinically, the autonomic function disorder is associated with resting tachycardia, exercise intolerance, orthostatic hypotension, intraoperative cardiovascular instability, silent myocardial ischemia and increased mortality. For the diagnosis, the integrity of the parasympathetic and sympathetic nervous system is assessed. Parasympathetic activity is examined by mea

Lorazepam-Induced Orthostatic Hypotension and Secondary Headache in a Low Blood Pressure Phenotype.

A 30-year-old man with a lifelong low but stable blood pressure (BP) (typically 90-95/50-55 mmHg) developed dose-related orthostatic symptoms and evening headaches after the initiation of nightly lorazepam. Classic orthostatic hypotension (OH) thresholds were not consistently met; however, standing repeatedly reproduced symptoms with concurrent heart rate (HR) increases, a pattern more consistent with orthostatic intolerance (OI) time-locked to benzodiazepine (BZD) exposure. Symptoms were reliably worse on 2 mg nights and attenuated on 1 mg or off-nights (dechallenge/re-challenge). Co-exposure…

Systemic exertion intolerance disease/chronic fatigue syndrome is common in sleep centre patients with hypersomnolence: A retrospective pilot study.

Symptoms of the central disorders of hypersomnolence extend beyond excessive daytime sleepiness to include non-restorative sleep, fatigue and cognitive dysfunction. They share much in common with myalgic encephalomyelitis/chronic fatigue syndrome, recently renamed systemic exertion intolerance disease, whose additional features include post-exertional malaise and orthostatic intolerance. We sought to determine the frequency and correlates of systemic exertion intolerance disease in a hypersomnolent population. One-hundred and eighty-seven hypersomnolent patients completed questionnaires regard…

Impaired orthostatic blood pressure stabilization and reduced hemoglobin in chronic kidney disease.

Impaired orthostatic blood pressure (BP) stabilization is prevalent in patients with chronic kidney disease (CKD) and is associated with adverse outcomes. We aimed to test the hypothesis that reduced hemoglobin is an important contributor to orthostatic intolerance in CKD in the present study. This study included 262 patients with non-dialysis-dependent CKD. Seated and standing BP was measured, and orthostatic BP reduction was calculated for both systolic BP (∆ SBP) and diastolic BP (∆ DBP). The association between orthostatic BP reduction and hemoglobin was determined by multiple linear regre…

Low-Grade Oligodendroglioma of the Pineal Region: Case Report.

Purpose Postural orthostatic tachycardia syndrome (POTS) is a form of orthostatic intolerance characterized by an increased heart rate upon transition from supine to standing, and head-up tilt without orthostatic hypotension. Its etiology is multifactorial, and no clear cause has been identified. Common symptoms include light-headedness, blurred vision, weakness, cognitive difficulties, and fatigue and are often accompanied by palpitations, shortness of breath, syncope, or gastrointestinal symptoms. Management includes volume expansion, physical counter maneuvers, and pharmacological agents su…

[Myalgic Encephalitis/Chronic Fatigue Syndrome: Diagnostic and Therapeutic Approach and Biological Research].

Myalgic encephalitis/chronic fatigue syndrome, an intractable disease characterized by profound fatigue, sleep disturbance, cognitive impairment, and orthostatic intolerance, among other features, often occurs after infectious episodes. Patients experience various types of chronic pain; however, post-exertional malaise is the most significant feature, which requires pacing. In this article, I summarize the current diagnostic and therapeutic approaches and describe recent biological research in this domain.

Symptoms are an optimal target for orthostatic hypotension screening and management: an argument in favor.

Orthostatic hypotension (OH), defined as a sustained reduction of ≥ 20 mmHg in systolic or ≥ 10 mmHg in diastolic blood pressure within 3 minutes of standing, represents one of the most clinically significant manifestations of autonomic failure. Beyond its hemodynamic definition, OH is associated with disabling symptoms, falls, syncope, reduced quality of life, increased healthcare utilization, and excess mortality. In clinical practice, management often requires balancing objective blood pressure measurements with the patient's lived experience of orthostatic intolerance. This viewpoint argue…

Non-invasive Vagus Nerve Simulation in Postural Orthostatic Tachycardia Syndrome.

Postural orthostatic tachycardia syndrome (POTS) is a chronic debilitating condition of orthostatic intolerance, predominantly affecting young females. Other than postural tachycardia, symptoms of POTS include a spectrum of non-cardiac, systemic and neuropsychiatric features. Despite the availability of widespread pharmacological and non-pharmacological therapeutic options, the management of POTS remains challenging. Exaggerated parasympathetic withdrawal and sympathetic overdrive during postural stress are principal mechanisms of postural tachycardia in POTS. Non-invasive, transcutaneous, vag…

[Orthostatic hypotension after space flight].

Cardiovascular changes occur during spaceflight. Because of the loss of hydrostatic pressure there is, during weightlessness, a fluid shift … with possible tendency towards spontaneous syncope and decreased effort capacity. The mechanisms of orthostatic intolerance are not well known. Several factors are probably involved, including hypovolaemia, modifications of the venous system, baroreflex changes, etc. Various countermeasures have been tested in microgravity simulations on the ground, then in flight.

Genetic variants in acetylcholine processing and significant improvement with pyridostigmine in a patient with postural orthostatic tachycardia syndrome: a case report.

BACKGROUND: Postural orthostatic tachycardia syndrome (POTS) is a common autonomic disorder of heterogeneous pathophysiology involving relative sympathetic overactivity and/or parasympathetic hypofunction. Postural orthostatic tachycardia syndrome is characterized by exaggerated postural tachycardia in the absence of orthostatic hypotension, in conjunction with orthostatic intolerance and other cardiovascular and neurologic features. CASE SUMMARY: We report a 30-year-old woman with POTS who experienced significant improvement with pyridostigmine. The whole genome sequencing demonstrated four …

The Effect of Orthostatic Hypotension Detected Pre-Operatively on Post-Operative Outcome.

BACKGROUND: Exacerbation of or new onset orthostatic hypotension in perioperative patients can occur. There is complex underlying pathophysiology with further derailment likely caused by acute cardiovascular changes associated with surgery. The implications for post-operative recovery are unclear, particularly in frail and older patients. We retrospectively explored patient notes for evidence of post-operative orthostatic intolerance in relation to pre-operative orthostatic hypotension. METHODS: Supine and 1-minute and 3-minute standing blood pressure measures obtained from adult patients bef…

Presumed Postural Tachycardia Syndrome: 12-Lead Electrocardiograms During Tilt Table Testing Unmask an Atrial Tachycardia.

BACKGROUND: Tilt table testing is commonly used to evaluate orthostatic intolerance and syncope and is typically performed with three-lead electrocardiography (ECG) monitoring, which limits P-wave interpretation. CASE SUMMARY: A 21-year-old woman with congenitally corrected transposition of the great arteries presented with recurrent tachy-palpitations and near syncope and was presumed to have postural tachycardia syndrome (POTS) after extensive noninvasive testing. During head-up tilt table testing incorporating intermittent 12-lead ECGs, she developed a sustained narrow-complex tachycardia …

Transient orthostatic hypotension is common in adolescents.

OBJECTIVES: We hypothesized that transient hypotension associated with the symptom of lightheadedness with standing is a common phenomenon in healthy adolescents but may be mistaken for orthostatic intolerance. STUDY DESIGN: We studied 23 healthy adolescents, combining upright tilt to 70 degrees and to 35 degrees (in 12 patients) with continuous heart rate and blood pressure (BP) measurements. We measured peripheral blood flow and venous pressure in the arms and legs by using venous plethysmography. RESULTS: Healthy subjects with normal vasoconstrictive responses to sustained upright tilt un…

Evaluating the efficacy of an active compression brace on orthostatic cardiovascular responses.

Orthostatic intolerance, one of the principle causes of syncope, can occur secondary to concomitant venous pooling and enhanced capillary filtration. We aimed to evaluate a prototype portable calf active compression brace (ACB) designed to improve orthostatic haemodynamic control. Fourteen healthy volunteers participated in a randomized, placebo controlled, cross-over, double-blind study. Testing consisted of head-upright tilting and walking on a treadmill conducted on two consecutive days with a pair of ACBs wrapped around both calves. The ACB was actuated on one test day, but not on the othe…

HIV-associated fatigue in the era of highly active antiretroviral therapy: novel biological mechanisms?

OBJECTIVE: The aim of the study was to determine the prevalence and risk factors for HIV-associated fatigue in the era of highly active antiretroviral therapy (HAART). METHODS: A cross-sectional survey of 100 stable HIV-infected out-patients was carried out. Severity of fatigue was measured using the Fatigue Impact Scale (FIS). Symptoms of orthostatic intolerance (dysautonomia) were evaluated using the Orthostatic Grading Scale (OGS). Data for HIV-infected patients were compared with those for 166 uninfected controls and 74 patients with chronic fatigue syndrome (CFS)/myalgic encephalomyeliti…

Certain cardiovascular indices predict syncope in the postural tachycardia syndrome.

Patients with postural tachycardia syndrome (POTS) represent a patient population with orthostatic intolerance; some are prone to syncope, others are not. The underlying neurocardiovascular mechanisms are not completely understood. The current study was undertaken to assess if certain cardiovascular indices are predictive of syncope in POTS. We compared the response to tilt-up and the Valsalva maneuver in four groups: POTS patients who fainted (POTS-f; n = 11;31 +/- 11 years): POTS patients who did not faint (POTS-nf; n = 9; 29 +/- 9 years); normal controls (NLS; n = 13; 39 +/- 11 years); pati…
