OBJECTIVES: Although various stapedotomy and stapedectomy techniques exist, anchoring the piston can be challenging. We present a novel surgical approach for treating congenital stapes malformations with an atypical facial nerve trajectory.
METHODS: This is a case of a 7-year-old boy presenting with bilateral conductive hearing loss. Prior attempts at tympanoplasty had proven unsuccessful in improving his hearing. Presurgical imaging studies revealed an unusual anatomical configuration, with the facial nerve positioned inferior to the oval window. This anatomical variation precluded the use o

To achieve a lasting result in reconstruction of the ossicular chain, strict material and surgical criteria must be combined. The best biomaterial, apart from bone, is hydroxyapatite, the mineral matrix of living bone tissue. For the reconstruction, the assembly technique is used instead of a columella. Direct contact with the drum membrane is not reliable if a columella is used, and extrusion or lateralization may influence postoperative results. In the case of a missing incus, an assembly prosthesis of dense hydroxyapatite is placed on top of the stapes head and the handle of the malleus, in

OBJECTIVE: To report a novel management strategy for mixed hearing loss in advanced otosclerosis.
METHODS: A 50-year-old male was referred to St Thomas' Hearing Implant Centre with otosclerosis; he was no longer able to wear conventional hearing aids because of recurrent otitis externa. The patient underwent short process incus vibroplasty (using the Med-El Vibrant Soundbridge device), followed at a suitable interval (six weeks) by stapes surgery. The main outcome measures were: pure tone audiometry, functional gain and monosyllabic word recognition scores.
RESULTS: Post-operative pure tone

This study aimed to evaluate the benefit but also the extrusions, dislocations, and failures of a titanium incus prosthesis along with the long-term audiological outcomes. We prospectively collected data from 139 patients undergoing ossiculoplasty using the Fisch titanium incus prosthesis between 2001 and 2016. Overall, 126 patients with at least 6 months of follow-up (mean 4.5 years, range 6-155 months) were analyzed. Patients were grouped as "extrusion" (n = 9, 7%) if the prosthesis extruded, "failure" (n = 22, 18%) if a reoperation was needed concerning the prosthesis, and "stable" (n = 95,

With the aid of a mechanical middle ear model (MMM) the sound transmission properties of different middle ear implants were investigated. Input of the MMM involved a broad-band signal from 100 to 5000 Hz that was supplied by a miniaturized loudspeaker. Displacement of an artificial stapes footplate was measured by a fiberoptic probe. The transfer functions of four different total ossicular replacement prostheses (TORPs) of different materials and shapes were compared. Three of the devices revealed similar transfer functions which corresponded to the typical curve of the normal middle ear. One

Since the original carved Teflon stapes over vein graft, stapedectomy prostheses have undergone evolution. Prostheses shapes, materials, and surgical techniques for placement have reflected advances in biomaterials and surgical tools. The variability in prostheses has reflected alternative techniques of stapedectomy and stapedotomy and differing strategies for attachment to the incus. Although many iterations of stapes prostheses have been proposed, excellent results can be achieved with various prostheses designed to rest on tissue grafts in stapedectomy techniques or pass through the footpla

Totally implantable cochlear implants may be able to address many of the problems cochlear implant users have around cosmetic appearances, discomfort, and restriction of activities. The major technological challenges that need to be solved to develop a totally implantable device relate to implanted microphone performance. Previous attempts at implanting microphones for cochlear implants have not performed as well as conventional cochlear implant microphones, and in addition have struggled with extraneous body or surface contact noise. Microphones can be implanted under the skin or act as senso

BACKGROUND: The erosive effect of chronic otitis media impairs the middle ear's ossicles. Alloplastic materials or autologous incus can be used for ossicular repair. Perichondrial roll is simple to use and saves considerable operative time and cost, especially in developing countries where ossicular prosthesis and bone substitutes are not affordable for the majority of patients.
OBJECTIVE: To evaluate the use of the perichondrial roll in the reconstruction of the long process of the incus.
SUBJECTS AND METHODS: This study involved 62 patients diagnosed with chronic suppurative otitis media c…

INTRODUCTION: Contrary to osteomas and exostosis of the external ear canal, middle ear osteomas are rare benign tumors that are usually revealed by progressive hearing loss.
CASE REPORT: We here report the case of 56-year-old woman who had a left ear conductive hearing loss. A CT-scan showed a bone-density tumor in the left posterior epitympanum, hanging up to the tegmen tympani. A surgical middle ear exploration showed an epitympanic osteoma, confirmed by a histological exploration. The tumor was left in place and an ossicular prosthesis was inserted, which permitted a significant gain in th…

Stapedectomy was performed in sixty-seven ears with tympanosclerotic footplate fixation. This was confined to the footplate in 90% of ears, and there was an association with cholesteatoma and chronic infection. Large fenestra stapedectomy was performed in 91% of cases and the surgical procedure performed was tympanoplasty in 91% of cases. Three cases (4.5%) suffered a dead ear by three years following the procedure, but only one immediately. A variety of surgical procedures were used to reconstruct the tympanic membrane to oval window connection, and the air-bone gap did not depend on the choi

PURPOSE: This study aimed to propose a radiological classification of the incudo-stapedial angle by preoperative high-resolution computed tomography (HRCT) images and to highlight its importance for predicting the use of reversal-steps stapedotomy (RSS) rather than the traditional non-reversal technique.
METHODS: We included 83 candidates for stapedotomy operation. Two physicians measured the radiological incudo-stapedial joint angle in the preoperative HRCT. According to this measurement, the radiological incudo-stapedial joint was classified into three types: obtuse, right, and acute. In ad

The human tympanic membrane (TM) and ossicles are generally considered to act as a linear system as they conduct low and moderate level environmental sounds to the cochlea. At intense stimulus levels (> 120 dB SPL) there is evidence that the TM and ossicles no longer act linearly. The anatomical structures that contribute to the nonlinear responses and their level and frequency dependences are not well defined. We used cadaveric human ears to characterize middle-ear responses to continuous tones between 200 and 20,000 Hz with levels between 60 and 150 dB SPL. The responses of the TM and ossicl
