From bathtub ring to osteoporosis: a clinical review of the bisphosphonates.

BACKGROUND: Etidronate and pamidronate are bisphosphonates, a class of chemical compounds originally used to soften hard water and prevent soap scum. Etidronate was serendipitously found to abate calcification in a child with myositis ossificans progressiva. OBJECTIVE: Review the basic pharmacology of these compounds, as well as clinical uses of the approved and nonapproved forms. DISCUSSION: Etidronate is approved for the treatment of hypercalcemia, Paget's disease of bone, and ectopic calcification, and has been used to treat hyperparathyroidism and nephrolithiasis with limited success. Giant cell tumours in mandible and spine: a rare complication of Paget's disease of bone.

The case of a man who was diagnosed as having polyostotic Paget's disease at the age of 52 years is described. He developed a rare neoplastic complication of Paget's disease--a giant cell tumour in his mandible, which was excised. Nine years after the diagnosis of this tumour he developed a new giant cell tumour arising from the L3 vertebral body. He was born in Avellino in Italy, from where five other cases of giant cell tumours arising from Pagetic bone disease have been reported. No family relationship between our patient and the other cases was established. His Paget's disease was particul

[Paget's disease and metastatic epithelioma on the same bone. 11 new cases and review of the literature].

The authors collected observations concerning 11 newly histologically proved cases of a combination of Paget's disease and carcinoma metastatic to bone on the same bone from members of the French Rheumatology Society. The most frequent locations can be specified from their analysis and an analysis of 26 detailed cases from the literature, i.e. pelvis, femur, rachis, tibia, humerus, and the cancers most frequently involved--prostate, bronchi, kidney, breast and intestine. The most usual histological appearance is osteolytic, except in the case of prostatic carcinoma. The metastasis reveals the

[Paget's disease: errors to be avoided].

Paget's disease of bone occurs in elderly people and resembles no other disease. The most frequent diagnostic errors are made when it is discovered in young adults, especially after an injury. Forms with osteolysis of the lower limbs are the most misleading, and it is better to avoid biopsies in such cases, as they may be followed by fractures. In geriatric pathology the failure to recognize that cerebral, spinal cord and cardiac manifestations are due to a specific vascular disorder of pagetic origin is a serious error as it deprives the patient of calcitonin which is the only effective thera

MR imaging of lytic changes in Paget disease of the calvarium.

The authors present two patients with Paget disease, both showing lytic changes in the calvarium. On T1 contrast sequences, the first patient demonstrated an extraaxial soft tissue mass in the temporooccipital region. This abnormality was hypointense to the adjacent normal diploic tissue. The second patient demonstrated a hypointense frontal abnormality on the T1-weighted sequence. Intracranial extension of this lesion was associated with lysis in otherwise mixed signal pagetoid bone. The first patient had osteoclastic Paget disease or osteoporosis circumscripta. The second patient had sarcoma

Osteomalacia in Paget's disease treated with short term, high dose sodium etidronate.

Eleven patients with Paget's disease treated with sodium etidronate 20 mg/kg/day for two and four weeks showed significant reductions in plasma alkaline phosphatase activity and urinary hydroxyproline excretion, both of which are biochemical markers of bone turnover. After four weeks of treatment, however, histological examination of iliac crest biopsy samples showed that despite a rapid reduction in bone resorption there was an appreciable mineralisation defect; even after only two weeks' treatment the abnormalities in bone formation persisted for up to 10 weeks. The adverse effects of sodium

Paraplegia in Paget's disease of the vertebral column. A case report.

A case of Paget's disease of the lumbar and thoracic vertebrae together with gradual-onset paraplegia is reported. The need for early recognition of the disease and early institution of medical treatment is highlighted.

[Paget's disease involving the face. Apropos of 4 cases].

This study describes four cases of Paget's deforming dystrophy involving the jaw. In three of them the course of the disease affected only the bone involved whilst in the fourth the bones of the vault of the skull and some cranial nerves were affected in time. The prevalence of the female sex and of involvement of the maxilla was evident. The mandible was involved in only one case. Biochemical abnormalities were the same in all cases as well as being identical to those in cases of Paget's disease with multiple bone involvement. The authors suggest medical treatment (calcitonin, Na etidronate,

Dementia and hydrocephalus in Paget's disease: a case report.

Dementia and hydrocephalus in an elderly female were related to basilar impression caused by Paget's disease of the skull. Ventricular and lumbar thecal pressures were normal, but isotope cisternography suggested obstructive hydrocephalus. The patient improved after ventricular shunting. The importance of prompt investigation of dementia occurring in patients with Paget's disease is emphasised.

Calcitonin as a drug.

Calcitonin is a peptide hormone produced by the C-cells of the thyroid gland. Its main biological effect is to inhibit the resorption of bone. This effect, in combination with a calciuric action of calcitonin, can produce a lowering of blood calcium. These biological effects of calcitonin have made it useful in the treatment of hypercalcemic and hyperresorptive disorders.

Osteoid: an ultrastructural study.

Iliac crest bone biopsy specimens from 34 patients were examined with the electron microscope. Although the biopsy examinations were undertaken because of suspected metabolic or hematopoietic disease, neither clinically nor by light microscope was any evidence of disease identified. The layer of bone matrix between the osteoblast and the mineralized bone, usually referred to as "osteoid," is found not to be a homogeneous mass but to show a constant sequence of maturation and organization of collagen fibrils and mucopolysaccharides. This pattern was found to be modified in a situation of rapid

Tomography of petrous bone in deafness associated with Paget's disease.

Two cases of Paget's disease presenting with deafness are described. Tomography of the petrous bones showed, in both cases, an upward tilt of the long axes of the bones including their auditory canals, generalized sclerosis of the petrous pyramids and enlargement of the ossicles. In one case, there was narrowing of the internal auditory canal. Review of the literature revealed three radiological phases in Paget's disease of the temporal bone:--(i) early changes consisting mainly of demineralisation; (ii) an intermediate phase consisting of diminishing visibility of the semicircular canals and

[Sarcomas associated with Paget's disease].

The authors report 12 cases of Paget's disease with sarcomatous degeneration that were observed among 311 patients with Paget's disease hospitalized in the Montpellier Rheumatology Clinic. They compare their experience with information they were able to extract from the literature in French and English. The frequency of such degeneration in cases of Paget's disease is difficult to determine but did not appear to exceed 1%. Degeneration rarely occurred before the age of 50 years, affected men twice as frequently as women, and occurred particularly in cases of diffuse Paget's disease, mainly in
