We present a case of vertebral osteomyelitis following multiple vertebral augmentations in a patient with an insidious presentation. Vertebral augmentation (kyphoplasty and/or vertebroplasty) is a minimally invasive procedure that has become a fairly common and highly effective method in treating compression fractures. A large majority of patients that undergo this procedure suffer from osteoporosis. Numerous studies have shown that patients who undergo the procedure obtain substantial pain relief and improve functional status, often times to a greater extent than other surgical and nonsurgica…

Osteomyelitis in childhood is often hematogenous and neonates are generally prone to bacteremia because of immature immunity. Neonatal osteomyelitis occurs even in the absence of apparent immunological or perinatal abnormalities.

OBJECTIVE: To analyze the clinical and laboratory characteristics of vertebral osteomyelitis in community-acquired bacterial meningitis patients.
METHODS: All episodes of vertebral osteomyelitis in a cohort study of adult patients with community-acquired bacterial meningitis in the Netherlands were analyzed. Subsequently, a systematic review of published cases was performed.
RESULTS: Between March 2006 and August 2018, 10 of 1974 (0.5%) meningitis patients were diagnosed with vertebral osteomyelitis. The median age was 70 years (interquartile range (IQR) 54-74 years); six (60%) were male and…

PURPOSE OF REVIEW: Skull base osteomyelitis (SBO) is a life-threatening condition. Due to an aging and increasingly multimorbid population, clinicians are more often challenged with this disease. Yet, there is no consensus on the optimal diagnostic and follow-up management. This review should aid clinicians in decision-making for their patients.
RECENT FINDINGS: Treatment-resistant otalgia or headache is suspicious of SBO. Pseudomonas aeruginosa remains the most common pathogen but clinicians are challenged with increasing rates of sterile or fungal cultures due to previously applied antibiot…

We describe a case of persistent tenosynovitis in the right fourth finger of a 50-year-old man that after almost a year of workup was found to be related to osteomyelitis caused by Mycobacterium arupense. The few cases found in the literature were associated with traumatic injuries, environmental contamination, and months of misdiagnosis. Treatment is challenging as there are limited data available on antimicrobial susceptibility and potential side effects of current therapy options.

A 14-year-old previously healthy girl presented with atraumatic onset of low back and flank pain and mildly elevated inflammatory markers. Magnetic resonance imaging disclosed an L5 superior endplate fracture, and cultures grew methicillin-susceptible Staphylococcus aureus. We discuss the variable presentations and appropriate treatments of adolescent vertebral osteomyelitis.

BACKGROUND: Clinical evidence for improving diagnostic accuracy in pediatric medicine is still scarce. Septic osteomyelitis is sometimes challenging for physicians to diagnose. The aim of this study was to improve patient care through identifying the incidence and reasons for errors in the diagnosis of bacterial osteomyelitis in pediatric patients.
METHODS: We retrospectively identified patients younger than 16 years with acute or chronic osteomyelitis at Tokyo Metropolitan Children's Medical Center between April 2010 and September 2017. We extracted data on patient demographics, clinical cou…

Chronic osteomyelitis treatment is always a challenge to orthopaedic surgeons which requires great dedication and perseverance. We report a successful limb salvage case of a 46-year-old man who suffered from a left tibia chronic osteomyelitis with soft tissue defect. The treatment approach was a thorough wound debridement of devitalized tissues and necrotized bone, commencement of culture-directed antibiotics, reconstruction with vascularized osteomyocutaneous fibula flap, and skeletal stabilization with internal fixation. As compared to below knee amputation, the result we obtained in this ca…

SURGICAL MANAGEMENT OF DIABETIC FOOT OSTEOMYELITIS. painless due to diabetic neuropathy. Osteomyelitis is most often managed surgically after both clinico-radiological examination of the wound and treatment of an arteriopathy if present. Multiple conservative surgical techniques currently exist, depending on the infection's location, and aim to treat and prevent ulcer recurrence. These techniques are briefly described presently. Combined conservative surgery, appropriate antibiotherapy, and off-loading allow full healing in the majority of cases. Amputation should only be considered after fail…

Gibbus deformities are characterized by anterior collapse of one or more vertebral bodies resulting in kyphosis. These deformities usually a … and T8-L3 fusion with improvement of symptoms. Microbiology returned positive for non-tuberculosis osteomyelitis, and the postoperative course was uneventful. This report further reviews the presentation, pathology, development, and neurosurgical treatment of Gibbus deformities. Methicillin-resistant Staphylococcus aureus osteomyelitis is a severe form of infection characterized by multifocal or multiple segmental osseous involvement and subperiosteal abscess formation with increased frequency of extraosseous complications including pyomyositis, septic thrombus, and septic arthritis. Bone scan showed long segment and/or multifocal involvement in 4 of 5 patients with areas of abnormal increased and decreased uptake. The clinical presentations included limp and/or pain. Joint involvement was seen in 4 cases. Bone scan abnormalities correlated well with MRI findings of s…

Cryptococcus is an encapsulated, yeast-like fungus that rarely causes infection in immunocompetent patients. We present the case report of a 66-year-old female patient with a history of rectal cancer with an isolated lumbar vertebral cryptococcosis proven by biopsy performed during operation. The patient was not an immunocompromised host and did not have any other risk factors except the history of cured rectal cancer. The presumptive diagnosis based on imaging studies was metastatic spine cancer, so operation was performed. However, cryptococcal osteomyelitis was diagnosed in the pathologic e…

Multifocal osteomyelitis is a rare complication of the Bacille Calmette-Guerin (BCG) vaccine. It particularly affects immunocompromised children and poses a difficult diagnostic problem. A 6-month-old boy had BCG vaccination postnatally and developed extensive disseminated skeletal osteomyelitis. He was found to have severe combined immunodeficiency. Despite antibiotic therapy and bone marrow grafting, the disease was fatal.

Skull base osteomyelitis is a rare but life-threatening condition that is characterized by osteitis of the temporal bone and skull base. Although skull base osteomyelitis is recognized as a rare complication of malignant external otitis or middle ear infection, it may also occur following a mastoidectomy. We present a case of an 81-year-old woman who suffered severe otalgia with normal-looking EAC and tympanic membrane that developed six weeks following a canal wall up mastoidectomy. Otalgia was the only abnormal feature to suspect occurrence of skull base osteomyelitis.

Cryptococcosis is a common opportunistic infection in AIDS patients. Almost all cases present with meningitis with or without fungaemia. Cryptococcal osteomyelitis occurs as part of a disseminated infection. Isolated cryptococcal osteomyelitis is rare and more so in immunocompetent patients. A case of isolated osteomyelitis caused by Cryptococcus neoformans in an immunocompetent patient is reported here. A female patient of 60 years old presented with pain and swelling of left clavicle. The histopathological examination of the biopsied material (bony fragment) showed histiocytes, lymphocytes w…
