Ankylosing spondylitis (AS) is a chronic inflammatory rheumatological disease affecting the axial skeleton with various extra-articular complications. Dysphagia due to a giant anterior osteophyte of the cervical spine in AS is extremely rare. We present a 48-year-old male with AS suffering from progressive dysphagia to soft foods and liquids. Esophagography showed an anterior osteophyte at C5-C6 resulting in esophageal compression. The patient refused surgical resection of the osteophyte and received conservative therapy. However, after 6 months there was no improvement in dysphagia. This case…

OBJECTIVE: To study the associations between structural abnormalities on ultrasound (US) or conventional x-rays (CR) and pain in hand osteoarthritis (HOA).
METHODS: In 55 consecutive patients with HOA (mean age 61 years, 86% women) fulfilling the American College of Rheumatology criteria, pain in 30 separate hand joints was assessed upon palpation; osteophytes were assessed by US and CR and joint space narrowing (JSN) by CR. Associations between structural abnormalities and pain per joint were analysed using generalised estimated equations to account for patient effects and adjusted for age, …

OBJECTIVE: Tibial spiking (i.e., spurring of tibial spines), eburnation, and osteophytes are considered features of osteoarthritis. This investigation employed direct inspection of the medial and lateral tibial plateaus in paleopathological specimens to analyze the frequency and morphological features of osteoarthritis and to define any relationship between the size of osteophytes and that of the intercondylar tibial spines.
MATERIALS AND METHODS: A total of 35 tibial bone specimens were evaluated for the degree of osteoarthritis and presence of eburnation. Each plateau was also divided into …

We suggest a method to validate the Samilson-Prieto classification system of dislocation arthropathy of the shoulder. Based on a mean standard sagital measure of the humeral head, we calculate an area index for the humeral osteophyte. The application of this method is done on a previous published series of 56 shoulders with a follow-up time of 15 years minimum after surgery with the Bankart or Bristow repairs. This technique of evaluation showed that the Bristow shoulders had a higher mean area index value on all radiological projections compared to the Bankart shoulders. The highest index was…

This report describes a case in which a large anterior osteophyte on the C2 and C3 vertebrae, due to ankylosing spondylitis, resulted in distortion of the anatomy of the upper airway and difficult intubation. Ankylosing spondylitis (AS) is a progressive inflammatory disease, characterized by stiffening of the joints and ligaments. Stiffness of the cervical spine, atlanto-occipital, temporomandibular and cricoarytenoid joints may cause difficult intubation (1). This report describes a case in which a large anterior osteophyte on the C2 and C3 vertebrae, associated with AS, resulted in distortio…

A series of 685 x-ray films of the cervical spine obtained in coal miners was analyzed to explore the occurrence of narrowed disc spaces and osteophytes in this occupational group, and to examine the association of x-ray changes with age, duration of employment, and duration of occupational exposure to hand-arm vibration. All data were extracted from individual medical files of coal miners examined for suspected hand-arm vibration-related disorders in 1989-1999 at the Occupational Medicine Center in Katowice. The narrowed intervertebral disc spaces were found in 188 coal miners (26.9%) and ost…

BACKGROUND: Central acetabular osteophytes (CAOs) have been implicated in abnormal hip biomechanics. However, their relationship with microinstability in borderline developmental dysplasia of the hip (BDDH) is unclear.
PURPOSE/HYPOTHESIS: The purpose of this study is to compare radiographic parameters between BDDH hips with and without CAOs and to determine whether CAOs are associated with microinstability. It was hypothesized that CAOs would correlate with increased version abnormalities and microinstability.
STUDY DESIGN: Case-control study; Level of evidence, 3.
METHODS: We retrospective…

Vertebral osteophytes are the result of intervertebral disc degeneration and can occasionally damage the aorta located in their proximity. This paper describes the case of a 69-year-old woman with a pseudoaneurysm of the descending thoracic aorta caused by a progressively growing dorsal osteophyte. The patient was asymptomatic as the pseudoaneurysm was an incidental finding diagnosed with an 18F-FDG PET/CT and computed tomography angiography (CTA) performed during follow-up for breast cancer. The comparison between the latest 18F-FDG PET/CTA performed and previous exams was crucial for the dia…

An acutely locked knee is an incapacitating condition that requires urgent orthopedic intervention. Common causes of the locked knee include a tear of the meniscus, stump of a ruptured anterior cruciate ligament (ACL), loose body, and osteochondral injury. This report describes a rare case of an acutely locked knee due to the concurrent presence of a loose body and a degenerative osteophyte, highlighting the diagnostic challenges and treatment strategies employed for successful management. Notably, to our knowledge, there is no preceding report documenting the same etiology.

STUDY DESIGN: Retrospective case series.
OBJECTIVE: To better understand the functional swallow outcomes, cervical balance, and surgical complications, we examined patients with anterior osteophytes and dysphagia who were treated operatively.
SUMMARY OF BACKGROUND DATA: Anterior osteophytes from diffuse idiopathic skeletal hyperostosis (DISH) or degenerative etiology of the cervical spine can cause dysphagia from mechanical compression of the esophagus. Osteophytectomy is generally accepted as a safe surgical treatment, but the risk of instability is unclear. The potential for associated com…

The mechanical consequences of osteophytes are not completely clear. We aimed to understand whether and how the presence of an osteophyte perturbs strain distribution in the neighboring bone. The scope of this study was to evaluate the mechanical behavior induced by the osteophytes using full-field surface strain analysis in different loading configurations. Eight thoracolumbar segments, containing a vertebra with an osteophyte and an adjacent vertebra without an osteophyte (control), were harvested from six human spines. The position and size of the osteophytes were evaluated using clinical c…

The use of prednisolone is one major risk factor for osteonecrosis in patients with systemic lupus erythematosus. Bone scintigraphy can be a diagnostic tool for early diagnosis. We present a case who had collar osteophytes at the bilateral femoral heads, which mimicked osteonecrosis in the planar bone scintigram. An SPECT/CT scan avoided this pitfall and increased the diagnostic accuracy for osteonecrosis.

PURPOSE: Erosions and osteophytes are radiographic characteristics that are found in different stages of temporomandibular joint (TMJ) osteoarthritis. This study assessed the effectiveness of digital subtraction radiography (DSR) in diagnosing simulated osteophytes and erosions in the TMJ.
MATERIALS AND METHODS: Five intact, dry human skulls were used to assess the effectiveness of DSR in detecting osteophytes. Four cortical bone chips of varying thicknesses (0.5 mm, 1.0 mm, 1.5 mm, and 2.0 mm) were placed at the medial, central, and lateral aspects of the condyle anterior surface. Two defect…

BACKGROUND: Vertebral artery compression by cervical osteophyte is a rare cause of vertebrobasilar ischemic stroke. This mechanism of stroke has been reported as the Bow Hunter syndrome defined by vertebrobasilar insufficiency because of mechanical stenosis of the vertebral artery at the cervical level triggered by head movement. The most common treatment is surgical decompression. However, in most cases, a dominant vertebral artery is involved, and its dynamic extrinsic compression is demonstrated on angiography.
CASE REPORT: We report a patient with recurrent posterior circulation infarctio…

OBJECTIVE: Previous studies of skeletal remains have suggested that both enthesophytes and osteophytes are manifestations of an underlying bone-forming tendency. A greater prevalence of osteophytes has been observed among individuals with high bone mass (HBM) compared with controls. This study was undertaken to examine the possible interrelationships between bone mass, enthesophytes, and osteophytes in a population of individuals with extreme HBM.
METHODS: Cases of HBM (defined according to bone mineral density [BMD] Z scores on dual x-ray absorptiometry) from the UK-based HBM study were comp…
