[Cerebral Erdheim-Chester disease].

We report the case of a 26-old-year man hospitalized for first partial complex epileptic seizure. Brain MRI showed an asymptomatic pseudo-tumor lesion in the brainstem. Diabetes insipidus, hypophyseal gonadotropic deficiency and osteosclerosis of long bones strongly suggested Erdheim-Chester disease, a rare histiocytosis, confirmed after tibial biopsy. Six months later, the patient remained stable. A persistent, and even increased, enhancement with Gd-DTPA on brain MR images was noted as previously described. The review of the literature collected 64 cases, and only 7 cases of cerebral "tumor"…

The spectrum of radiographic bone changes in children with fluorosis.

Painful, crippling deformities in Tanzanian children from an area of endemic fluorosis are reported. Excessive fluoride ingestion in pregnant women may possibly poison and alter enzyme and hormonal systems in the fetus causing disturbances to osteoid formation and mineralization. Knock-knees, bowlegs, and saber shins develop when walking begins. Combinations of osteomalacia, osteoporosis, and osteosclerosis result in a spectrum of bone changes from an early age. Male hormones, and dietary and genetic deficiencies may aggravate individual response of actively growing bones. Some radiographic ch…

Sclerosing bone dysplasias: genetic, clinical and radiology update of hereditary and non-hereditary disorders.

There is a wide variety of hereditary and non-hereditary bone dysplasias, many with unique radiographic findings. Hereditary bony dysplasias include osteopoikilosis, osteopathia striata, osteopetrosis, progressive diaphyseal dysplasia, hereditary multiple diaphyseal sclerosis and pyknodysostosis. Non-hereditary dysplasias include melorheostosis, intramedullary osteosclerosis and overlap syndromes. Although many of these dysplasias are uncommon, radiologists should be familiar with their genetic, clinical and imaging findings to allow for differentiation from acquired causes of bony sclerosis. …

Disseminated histoplasmosis in a non-immunocompromised host.

Histoplasma infections in Europe are rare, and acute disseminated histoplasmosis has only been observed in immunocompromised persons. We describe a case of acute disseminated histoplasmosis in a young, nonimmunocompromised European woman. The probable source of infection was Sri Lanka or the Maldives. At presentation she was severely ill with fever, lymphadenopathy, anemia, thrombocytopenia, hepatosplenomegaly, and polyserositis. Histologically, myelofibrosis and osteosclerosis were observed with extramedullary hematopoiesis. Histoplasma capsulatum yeasts were detected in bone marrow trephine …

Painful bone end sclerosis. Variant of renal osteodystrophy simulating osteonecrosis.

Painless sclerotic change in bone ends occurring as a manifestation of renal osteodystrophy has been reported in two previous cases. The sclerotic change can, however, be accompanied by nonspecific periarticular pain. Radiologic changes in "bone ends" may mimic osteonecrosis. Nuclear bone scanning may show increased radionuclide uptake early in both conditions but late uptake may be less in "bone end sclerosis." The "rugger jersey" spine occurring in the usual osteosclerosis of renal osteodystrophy has been consistently absent in this variant. Osteonecrosis should be excluded in making the dia…

Osteopetrosis, renal tubular acidosis and basal ganglia calcification in three sisters.

Three adult sisters with osteopetrosis in infancy had spontaneous resolution of bone modeling defects and osteosclerosis. During adolescence, basal ganglia calcification developed in two. Renal tubular acidosis (type I) was diagnosed in each during early adulthood. The disorder was transmitted apparently as a recessive trait--the same mode of inheritance as for the "malignant" form of osteopetrosis which is usually fatal during childhood. Electron microscopy of bone suggested that osteoclasts failed to form "ruffled membranes" characteristic of active bone resorbing cells. Chronic systemic aci…

Denosumab Can Prevent Collapse in Patients with Early-Stage Steroid-Induced Osteonecrosis of the Femoral Head by Inhibiting Osteoclasts and Autophagy.

OBJECTIVE: The osteoclastic bone resorption inhibitors might have positive effect in preventing femoral head collapse in patients with osteo … ter the initial diagnosis of ONFH. Secondary outcomes included the Harris hip score, progression of osteosclerosis, increase in necrotic area, bone marrow oedema relief, and bone mineral density increase in the femoral head. The Mann-Whitney U test and chi-square tests were performed to identify the differences between the continuous and categorical variables, r …

Atypical Bone Involvement in Erdheim-Chester Disease Showing No or Subtle Structure Changes and Increased Activity on FDG PET/CT.

Bones are the most common sites involved in Erdheim-Chester disease. On imaging, the bone involvement in Erdheim-Chester disease typically presents as osteosclerotic lesions, and osteolytic lesions are unusual. Bone lesions showing no or subtle structure changes are rarely reported. We describe FDG PET/CT findings in a case of Erdheim-Chester disease with atypical bone involvement. The bone lesions showed no or subtle structure changes without obvious osteosclerosis or osteolysis on radiographs and CT, and increased FDG activity mimicking lymphoma. Familiarity with these atypical imaging findi…

A Rare Case of Cementoblastoma of the Second Right Maxillary Premolar in a 30-Year-Old Man.

Cementoblastoma is a benign odontogenic mesenchymal tumor characterized by cementum production. Cementoblastoma is considered a relatively rare neoplasm with a predilection to the posterior region of the mandible. The main clinical differential diagnoses include hypercementosis, cemento-osseous dysplasia, condensing osteitis, idiopathic osteosclerosis, osteoblastoma, odontoma, and osteosarcoma. Imaging findings may be pathognomonic when demonstrative. Although rather identical histologically, cementoblastoma exhibits unequivocal fusion to the root of the tooth, which distinguishes it from oste…

[Erdheim-Chester disease : a differential diagnosis of retroperitoneal fibrosis].

Erdheim-Chester disease is a rare multisystemic non-Langerhans histiocytosis with about 500 reported cases. Typical features include retroperitoneal and perirenal fibrosis (hairy kidney), periaortitis with a coated aorta, osteosclerosis of the lower limbs, and sometimes exophthalmia or diabetes insipidus. Histology is the cornerstone for diagnosis showing an infiltrate with foamy histiocytes and occasional multinucleated giant cells (Touton cells). There is no standard treatment regimen, current options include corticosteroids, interferon alpha, systemic chemotherapy, and radiation therapy ; h…

FDG PET/CT in Early and Late Stages of SAPHO Syndrome: Two Case Reports With MRI and Bone Scintigraphy Correlation.

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a rare disease, which may be underdiagnosed or misdiagnosed because of nonspecific clinical and imaging findings. We present 2 cases of SAPHO syndrome with MRI, bone scintigraphy, and FDG PET/CT findings. In case 1 with early-stage disease, the active osteitis showed osteolytic bone destruction with increased FDG uptake. In case 2 with late-stage disease, the inactive bone lesions presented as osteosclerosis with normal FDG uptake. Familiarity with the FDG PET/CT findings of SAPHO syndrome in different stages may be he…

Imaging spectrum in sclerotic myelomas: an experience of three cases.

The classic radiographic presentation of multiple myeloma is lytic skeletal lesions. Primary sclerotic manifestations are rare and occur only in 3% of cases. The imaging spectrum in three cases of multiple myeloma with primary osteosclerosis is described. The first patient had spiculated sclerosis of the orbit, which is an uncommon site for myeloma. The second patient with POEMS syndrome had multiple, scattered, skeletal lesions with sclerotic margins. The third patient presented with a chest wall mass and had an expansile thick spiculated sclerosis in the rib. The wide imaging spectrum possib…

Complex odontoma as a periapical and interradicular radiopacity in a primary molar.

Only rarely is a mature complex odontoma seen in a periapical location and in association with a primary tooth. Complex odontomas usually present as a radiopacity with a radiolucent rim. The differential diagnosis must include mature fibro-osseous lesions (e.g. cementoblastoma). Complex odontomas can be differentiated from fibro-osseous lesions because they usually present a nonhomogeneous radiopacity and almost always will present as solitary lesions. For complex odontomas located in the periapical region, the differential diagnosis must include idiopathic periapical osteosclerosis. In this c…

Chronic myelomonocytic leukemia associated with primary amyloidosis.

A case of chronic myelomonocytic leukemia (CMML) associated with primary amyloidosis (AL) is presented. Hepatosplenomegaly, macroglossia, and xanthelasma were the major physical findings. Laboratory tests showed macrocytic anemia, thrombocytopenia, monocytosis and a bi-clonal gammopathy. Early monocytes and monoblasts were noted in the bone marrow aspiration biopsy. Cytogenetic evaluation showed a clonal deletion of chromosome 21 long arm (21q-). Amyloid was present in the liver, tongue and xanthelasma. In addition, the patient was noted to have osteosclerosis of the lower extremities. Treatme…
