Principles of Pediatric Endoscopic Ear Surgery.

The article describes the unique benefits and challenges of transcanal (and transmastoid) endoscopic ear surgery (EES) for management of middle ear disease in children. It provides a rationale for EES in children and describes differences in anatomy between the pediatric and adult ear. The basic principles of EES, from operating room layout, choice of surgical instruments, and tips and pearls to avoid complications specific to the endoscope, are reviewed. Finally, techniques and outcomes in pediatric EES for tympanic membrane perforation, congenital cholesteatoma, and acquired cholesteatoma ar

Medpor alternative for microtia repair.

Autologous costal cartilage has been the material of choice for framework in auricular reconstruction. However, the advent of porous high-density polyethylene (Medpor) has provided an effective alternative in microtia repair. A ready-to-use allopastic material, Medpor has distinguished itself from other implants via its durability, tensile strength, biocompatibility, and malleability. Results from clinical experience and bench research support this claim. When using this material, the authors noted impressive results with low associated morbidity.

Long-term Results of Endoscopically Assisted Pediatric Cholesteatoma Surgery.

OBJECTIVE: Routine endoscopic examination during primary surgery decreased the rate of residual cholesteatoma to 18% in our early experience. Based on this, we stopped performing routine second-look surgery in children who were endoscopically free of cholesteatoma at the end of primary surgery. We sought to investigate if second-look procedures after endoscopic-assisted surgery could safely be performed only in children at a high risk of residual disease (extensive inflammation, spontaneously ruptured or fragmented cholesteatoma, residual disease intentionally left). STUDY DESIGN: Case series

Surgical management of major congenital malformations of the ear.

Surgical management of major congenital malformations of the ear is a difficult and complex procedure. The risk of damaging middle and inner ear structures because of their frequently aberrant location, the failure to keep a patent ear canal over the long term, and the difficulty in achieving a good hearing result all challenge the otologic surgeon. The purpose of this study was to analyze the results obtained with a surgical technique developed to prevent postoperative stenosis or total closure of the newly formed auditory meatus and to achieve at least serviceable hearing in the majority of

[Online training for hearing implant surgery : A new approach to otological training. German version].

OBJECTIVE: Education in microsurgery of the ear includes staged training to allow for mastering of the complex microsurgical procedures, particularly in the context of middle ear reconstruction and cochlear implantation. Traditional surgical training includes temporal bone preparations by cadaver dissection and supervised operating room practice. As these on-site trainings are limited, there is a need to broaden education facilities in an on-line format. Therefore, a first basic on-line training for otosurgery was developed. MATERIALS AND METHODS: The system consists of an artificial temporal

Use of costal cartilage in middle ear surgery.

Use of costal cartilage in middle ear surgery. Tympanic membrane retraction often occurs in otologic pathology. In an effort to reduce this pathology, the surgical procedure considered incorporates the use of a new material which diminishes retraction and allows freer movement of the eardrum. Since 1991, we have been using costal cartilage allograft obtained from donors and treated to prevent disease transmission. The allografts were used for tympanoplasty, retraction pockets and attic reconstruction. More than 150 patients were treated and the results evaluated in order to determine the indic

Impact of surgical approach on taste dysfunction and quality of life in middle ear surgery: a systematic review and meta-analysis.

PURPOSE: This systematic review and meta-analysis evaluated how middle ear surgical approach and chorda tympani nerve (CTN) handling influence postoperative taste disturbance and related quality of life. METHODS: Following PRISMA 2020 guidance, databases and grey literature were searched (2000-2025) for randomized and observational studies reporting taste outcomes after tympanoplasty, stapes surgery, cholesteatoma/mastoidectomy, or cochlear implantation. Risk of bias was assessed with RoB 2.0 and Newcastle-Ottawa scales, and certainty with GRADE. CTN manipulation was standardised using a four

The Swansea Technique: a novel method of endoscopic underwater excision of external auditory canal exostoses using a piezoelectric saw.

BACKGROUND: External auditory exostosis (surfer's ear) is a progressive condition of benign hyperostotic overgrowths of the external auditory canal, secondary to repetitive cold-water exposure, in which symptoms correlate to degree of exostosis. Surgical intervention is the only available treatment, most commonly using the osteotome or microdrill, but these carry a risk of sensorineural hearing loss, post-operative stenosis, temporomandibular dysfunction and tympanic membrane perforation. METHODS: We describe in detail the Swansea technique, an innovative method of canaloplasty with endoscopi

The posterior-inferior recess of the sinus tympani - radioanatomical investigation for purposes of endoscopic otosurgery in children under five.

BACKGROUND: Sinus tympani (ST) is located in the medial retrotympanum, above the subtympanic sinus, and below the posterior tympanic sinus. The surgical access to the ST may be transmeatal-endoscopic or transmastoid-retrofacial. It depends on the depth and type of ST assessed on axial planes. Recently, a new morphology of ST was found and described using sagittal planes- posterior-inferior recess (PIR). We aimed to explore its presence and morphology in children under five. METHOD: STs were assessed in tilted-axial planes, according to Marchioni classification, in 150 sets of CT images. Then,

Core Outcome Set for Reporting Results of Cholesteatoma Surgery From International Otology Outcome Group Consensus Study.

HYPOTHESIS: International consensus exists on optimal measures for reporting outcome from cholesteatoma surgery. This consensus can be used to create a core outcome set (COS) for publication standards. BACKGROUND: Systematic reviews show that the quality of published evidence available to inform surgical decision-making in the management of cholesteatoma is limited with inadequate distinction between residual and recurrent cholesteatoma, use of survival analysis or audiometric reporting standards. METHODS: The International Otology Outcome Group (IOOG) followed COS-STAD and COS-STAR guidelin

Complications of Surgery: Recognition and Management.

The middle ear and mastoid spaces contain delicate and functionally important structures. Safe cholesteatoma surgery requires mastery of otologic anatomy as well as thorough preoperative investigation. Cholesteatoma can damage otologic structures on its own; however, there is also an opportunity for an iatrogenic complication given the distortion of anatomy by the disease process. This article explores complications that can occur during cholesteatoma surgery, guidance on how to avoid intraoperative complications, and treatment of such complications.

Updates in paediatric cholesteatoma.

PURPOSE OF REVIEW: The management of paediatric patients with cholesteatoma is complex, as the disease process is more aggressive in children than adults. New technologies and practice adaptations currently help optimize the surgical management and monitoring of these challenging patients. RECENT FINDINGS: Several options of surgical management are available and are associated with equal if not improved outcomes. Technologic advancements in endoscopic ear surgery, the discovery of chemical-assisted dissection of cholesteatoma and advanced imaging protocols have enabled surgeons to better care

[Surgical solution of selective epitympanic dysventilation syndrome].

The selective epitympanic dysventilation syndrome is a new theory of the cholesteatoma formation, the description of which was allowed by a minimally invasive surgical technique – the transcanal endoscopic ear surgery – some years ago. The transcanal endoscopic ear surgery provided the possibility of a certain amount of detailed anatomical knowledge to identify the new factor behind this process, namely obstruction of the ventilation pathways of the middle ear. Our goal was to draw the ear surgeons’ attention to this clinical picture as cholesteatoma development and subsequent worsening of the
