Is the minimal clinically important difference (MCID) in acute pain a good measure of analgesic efficacy in regional anesthesia?

In the field of acute pain medicine research, we believe there is an unmet need to incorporate patient related outcome measures that move beyond reporting pain scores and opioid consumption. The term "minimal clinically important difference" (MCID) defines the clinical benefit of an intervention as perceived by the patient, as opposed to a mathematically determined statistically significant difference that may not necessarily be clinically significant. The present article reviews the concept of MCID in acute postoperative pain research, addresses potential pitfalls in MCID determination and qu

Cross-cultural adaptation and validation of the Dutch version of the core outcome measures index for low back pain.

PURPOSE: The core outcome measures index (COMI) is a validated multidimensional instrument for assessing patient-reported outcome in patients with back problems. The aim of the present study is to translate the COMI into Dutch and validate it for use in native Dutch speakers with low back pain. METHODS: The COMI was translated into Dutch following established guidelines and avoiding region-specific terminology. A total of 89 Dutch-speaking patients with low back pain were recruited from 8 centers, located in the Dutch-speaking part of Belgium. Patients completed a questionnaire booklet includ

The effect of intra-articular vanilloid receptor agonists on pain behavior measures in a murine model of acute monoarthritis.

Arthritis is the most common cause of disability in the US, and the primary manifestation of arthritis is joint pain that leads to progressive physical limitation, disability, morbidity, and increased health care utilization. Capsaicin (CAP) is a vanilloid agonist that causes substance P depletion by interacting with vanilloid receptor transient receptor potential V1 on small unmyelinated C fibers. It has been used topically for analgesia in osteoarthritis with variable success. Resiniferatoxin (RTX) is an ultra potent CAP analog. The aim of this study was to measure the analgesic effects of i

Clinician-reported lesion measurements in skin infection trials: Definitions, reliability, and association with patient-reported pain.

OBJECTIVES: Outcome assessments as clinical trial endpoints should be well-defined, reliable, and reflect meaningful treatment benefits. For acute bacterial skin and skin structure infections (ABSSSI) trials, recent recommendations suggest a primary endpoint of reduction in skin lesion area. Objectives were: evaluate ABSSSI lesion area measurement reliability, evaluate impact of various lesion area definitions on treatment effect size, and explore relationships between lesion area and pain. METHODS: Data from two randomized, double-blinded Phase 3 trials comparing tedizolid to linezolid in AB

Effect of Sacral Neuromodulation on Outcome Measures and Urine Chemokines in Interstitial Cystitis/Painful Bladder Syndrome Patients.

OBJECTIVES: Sacral neuromodulation (SNM) may improve interstitial cystitis/painful bladder syndrome (IC/BPS) symptoms of urinary frequency, urgency and perhaps even pain, but objective measures of improvement are lacking. We evaluated the potential for urinary chemokines to serve as measures of treatment response over time to SNM. METHODS: Women with IC/BPS undergoing SNM consented for this study. Three-day bladder/pain diaries were collected at baseline and validated Interstitial Cystitis Symptom Problem Index (ICSPI) scores and mid-stream urine specimens were collected at baseline and at 24

Comparison of two methods for the evaluation of treatment in medial epicondylitis: pain estimation vs grip strength measurements.

INTRODUCTION: Determination of a precise and reproducible diagnostic tool for the evaluation of patients with medial epicondylitis (ME) is important for an effective follow-up. The commonly suggested use of grip strength measurements with a hand dynamometer is not always easily available and may be affected by the patient's compliance. We propose that pain estimation based on the visual analog scale can be considered for this purpose as well because it addresses the painful degenerative lesion in the common flexor origin, which is the basic pathological process in ME. Association between measures of spinal mobility and low back pain. An analysis of new attenders in primary care.

STUDY DESIGN: Comparison of spinal movements in new attenders to primary care with an episode of low back pain with a sample with no recalled history of ever having had low back pain. OBJECTIVES: To examine the association between restriction of spinal movement and the presence of low back pain in primary care. SUMMARY OF BACKGROUND DATA: The presence of restriction in spinal mobility has frequently been investigated by specialists in the clinical setting. What is not known, however, is whether an association exists between pain and restriction in those newly presenting to primary care and w

Reliability of Pressure Pain Threshold Measurements of Upper and Lower Limb Muscles of Young Swimmers: An Observational Study.

BACKGROUND AND PURPOSE: Objective measures should be combined with subjective assessments to monitor pain and discomfort, particularly in sports with a high prevalence of atraumatic injuries. Therefore, this study aimed to determine the reliability of pressure pain threshold (PPT) measurements in competitive swimmers and to explore the influence of intrinsic and extrinsic factors on PPT. METHODS: Forty-seven swimmers were included in the study. PPT was measured bilaterally in 10 muscles of the upper and lower limbs and trunk using a hand-held algometer. Two trained physiotherapists performed

Associations between quantitative sensory tests and measures of pain and function in persons over 45 with meniscal tear.

OBJECTIVE: Symptomatic meniscal tear and knee osteoarthritis (OA) are commonly accompanied by pain, which may arise from nociceptive mechanisms and from processes, referred to as sensitization, that alter and amplify pain perception. Sensitization may be assessed objectively with quantitative sensory testing (QST). There is a paucity of data on the magnitude of associations between QST measures and subjective and objective outcome measures. DESIGN: We assessed cross-sectional associations between QST measures and pain, muscle strength, and performance measures in participants in the TeMPO tri

Archetypal analysis of pain measures in a diverse population-based cohort: Exploring cross-sectional relationships with outcomes.

OBJECTIVE: Pain experiences are complex, multifactorial, and subjective. Pain phenotyping can provide a holistic view of pain experiences by identifying distinct pain profiles. This study aimed to characterize chronic pain phenotypes using multiple validated osteoarthritis (OA) pain measures and assess their associations with functional and psychological outcomes. DESIGN: Participants were from the Johnston County Health Study (JoCoHS), a population-based cohort of non-Hispanic White, non-Hispanic Black, and Hispanic adults aged 35-70 years. Fourteen validated pain measures were administered.

Self-Reported Measures of Nociplastic Pain are Associated With Reduced Response to Endoscopic Retrograde Cholangiopancreatography in Chronic Pancreatitis.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) provides inadequate pain relief in nearly half of patients with chronic pancreatitis (CP). With no reliable predictors for ERCP response, patient-reported outcome measures (PROMs) of nociplastic pain, which have demonstrated predictive value in other chronic pain conditions, are appealing but remain unexamined in CP. This study assessed whether PROMs of nociplastic pain are associated with analgesic response to ERCP in CP. METHODS: Between October and November 2025, we conducted an electronic survey of patients with definite CP

Translation and Linguistic Validation into Spanish of the Owner-Reported Outcome Measure "Helsinki Chronic Pain Index" (HCPI).

Due to the challenges in evaluating chronic pain in dogs, owner-reported outcome measures have been developed, such as the Helsinki Chronic Pain Index (HCPI), originally written in Finnish and published in English, limiting its use among Spanish-speaking veterinarians and owners/caretakers. The goal of this study was to obtain an equivalent Spanish version of the HCPI. The translation process followed established guidelines. First, two native Spanish speakers independently translated the HCPI from English to Spanish. A veterinary professional and a third translator then compared the translatio

Turning Patients' Open-Ended Narratives of Chronic Pain Into Quantitative Measures: Natural Language Processing Study.

BACKGROUND: Subjective report of pain remains the gold standard for assessing symptoms in patients with chronic pain and their response to analgesics. This subjectivity underscores the importance of understanding patients' personal narratives, as they offer an accurate representation of the illness experience. OBJECTIVE: In this pilot study involving 20 patients with chronic low back pain (CLBP), we applied emerging tools from natural language processing (NLP) to derive quantitative measures that captured patients' pain narratives. METHODS: Patients' narratives were collected during recorded
