To statistically evaluate the factors that influenced speech following maxillectomy, the speech intelligibility (SI) in 54 patients was measured with and without a prosthesis. The mean SI score without a prosthesis in all patients was 35.7 +/- 22.7% and that with a prosthesis was 84.9 +/- 12.7%. The results of the postmaxillectomy SI statistical analysis revealed that an oro-nasal communication was one of the factors that influenced SI without a prosthesis. The resection of the anterior portion of the soft palate was one of the factors that influenced SI with a prosthesis, which suggested that

Is the poor potential of growth an ineluctable consequence of mesodermal deficiency? Should we agree with the idea that all protocols are equivalent? Actually, these opinions reflect the empiricism of previous generations. We must now become rational and develop a project without compromise to achieve good functions at primary surgery. 'The normal structures are present on either side of the cleft, only modified by the fact of the cleft...' Victor Veau's hypothesis is the conclusion of rigorous anatomical and embryological research. Our current knowledge of the pathological anatomy allows for

Digital workflows have revolutionized denture fabrication, offering greater customization and rapid production. This clinical report describes 2 treatment approaches using digital techniques to address prosthetic challenges. The first patient had a large maxillary defect after mucormycosis surgery, necessitating a combination of digital and conventional methods for obturator fabrication because of computer-aided design software program limitations. For the second patient, digital workflows were combined with the conventional neutral zone technique to improve denture stability and retention, ef

The posterior maxillary region presents substantial challenges in endodontic microsurgery owing to its unique anatomical position and intricate root canal anatomy. Three factors contribute to this complexity: first, the posterior location and limited surgical field, compounded by prominent buccal and palatal contours, severely limit visual clarity and operative maneuverability. Second, the complex root canal anatomy‒particularly isthmuses and the second mesiobuccal canal‒increases the difficulties and risks associated with root-end resection and retrograde obturation. Third, the intimate proxi

This report evaluated the impact of using multiple individualized endodontic access guides in the management of obliterated root canals in maxillary molars. A 70-year-old female patient presented with pain in the upper left first molar. Intraoral examination revealed an extensive resin composite restoration, with no signs of tooth mobility or marginal periodontal disease. The clinical diagnosis was symptomatic apical periodontitis. Cone beam computed tomography (CBCT) revealed a hypodense area in the mesiobuccal and palatal (P) roots, associated with signs of apical periodontitis and obliterat

Radicular cysts are the most common odontogenic cystic lesions of inflammatory origin. Large radicular cyst is comparatively less frequently associated with primary teeth. They represent only 0.5-3.3% of the total number of cysts in primary dentition. Radicular cysts arising from deciduous teeth are reported to occur in age range of 3-19 years with a male predominance. Although large radicular cysts are treated by enucleation with extensive removal of bone and vital teeth, marsupialization can be preferred as a conservative approach to reduce the morbidity. The purpose of this article is to re

Radicular cysts are the most common odontogenic cystic lesions of inflammatory origin which can be managed by marsupialisation specially if the cyst is large and is in relation to the vital structures. This article presents a case in which a radicular cyst was present in association with grossly carious deciduous molars and has been treated by marsupialisation. Postoperatively a surgical splint was inserted to maintain the patency of the bone cavity. This obturator splint also acts as a space maintainer to prevent space loss and ensure unimpeded eruption of permanent premolars.

A visible light-cured resin system enables the dentist to accomplish chairside relining of removable prosthesis. The material is pliable and can be initially cured in the mouth with the regular hand-held visible light. A special unit is needed to completely cure the material, which thereafter is comparable in quality to heat-polymerized acrylic resin. The value of this system for the practice of maxillofacial prosthetics is demonstrated by the ease of relining of various devices and obturator prostheses.

With previous prosthetic restorations of bilateral maxillary resections, tissue erosion and bleeding on the cephalic parts of the cavity have been caused by movement and pressure from the prosthesis. Retention of a prosthesis is a major determinant in its successful use. Physical and psychologic stresses are common when one is unable to use a prosthesis comfortably. The use of a musculocutaneous flap and an obturator prosthesis had provided near-immediate reconstruction of a massive midfacial defect. This method of reconstruction resulted in soft tissue replacement for appearance and functiona

BACKGROUND: Cleft lip and palate (CLP) are among the most common congenital anomaly that affects up to 33,000 newborns in India every year. Nasoalveolar moulding (NAM) is a non-surgical treatment performed between 0 and 6 months of age to reduce the cleft and improve nasal aesthetics prior to lip surgery. The NAM treatment has been a controversial treatment option with 51% of the cleft teams in Europe, 37% of teams in the USA and 25 of cleft teams in India adopting this methodology. This treatment adds to the already existing high burden of care for these patients. Furthermore, the supporting

OBJECTIVE: This study evaluated the effectiveness of computer-aided design-nasoalveolar molding (CAD-NAM) on maxillary alveolar morphology in infants with unilateral cleft lip and palate (UCLP).
METHODS: 15 infants with UCLP treated by CAD-NAM therapy composed the treatment group, and the control group consisted of 15 infants with non-presurgically treated UCLP. The maxillary morphology was analyzed by Rapidform XOR3 software. Differences in all variables pre- and post-CAD-NAM were discussed.
RESULTS: Significant difference was found in arch length, cleft gap, labial frenum deviation, A-X, A

UNLABELLED: SUBJECT MATTER: In cases of bilateral cleft lip, alveolus and palate, preoperative positioning of the premaxilla using Latham's appliance has been described. This method is controversial, since it may cause growth defects.
PATIENTS AND METHODS: From 1992 to 2000, Latham's appliance was inserted preoperatively into 20 patients with bilateral lip, alveolus and palate clefts with extreme premaxillary protrusion. Pin fixation ensued at the age of 3 months. After adjustment of the premaxilla, the soft palate was closed and bilateral lip adhesion as well as bilateral gingivoperiosteopla

This retrospective study describes the frequency of one team's acceptance of speech pathologists' recommendations for specific secondary treatment procedures for the correction of VPI for 100 consecutive patients. In addition, assessment was made of the level of success in eliminating VPI relative to treatments utilized that were recommended by speech pathologists versus level of success when treatment other than that recommended by speech pathologists were used. For the 78 patients who received the treatment procedure recommended by speech pathologists, only 10 percent continued to demonstrat

OBJECTIVE: Oral-cavity feeding movements were analyzed during sucking and used to modify a Hotz-type plate to facilitate sucking in infants with cleft lip and palate.
DESIGN: Sucking movements were analyzed using lateral view cine radiography as three adults fed from a bottle. A Hotz-type plate was then modified to better isolate the oral cavity and to occlude the oral airway. The plate was then utilized in direct breast feeding by 10 babies.
OUTCOME MEASURES: Sucking rate, amount of milk taken, and duration of use of the plate were assessed.
RESULTS: Wearing this plate, four babies with cl

Mucormycosis is an infection caused by mucoral fungi, particularly the genera Rhizopus, Rhizomucor, and Mucor. It leads to invasive necrotic lesions primarily in the nose and palate, manifesting symptoms such as pain, fever, orbital cellulitis, proptosis, and purulent rhinorrhea, among others. The predominant manifestations of this infection include rhino-orbital-brain and pulmonary forms, which occur predominantly in individuals with immunosuppression or diabetes. Mucormycosis is an uncommon pathology associated with a high mortality rate. Standard treatment involves the use of antimycotics a

Maxillary ameloblastoma is one of the rarest odontogenic epithelial tumors encountered, as 80% of ameloblastomas are seen within the mandible. Ameloblastoma is usually incidentally detected in the third to fourth decades of life, as most patients remain asymptomatic; yet some patients may complain of a slowly growing, painless swelling. We present a case of maxillary ameloblastoma with pulmonary metastasis along with a brief literature review. A 17-year-old male initially presented with painless right facial swelling, which, on examination, was non-tender, immobile, irregular, pink in color, w
