Buprenorphine for the Management of Intractable Pain in Skin Ulcers Associated With Calcific Uremic Arteriolopathy.

BACKGROUND: Calcific uremic arteriolopathy, or calciphylaxis, is a rare and severe condition marked by calcification of small-to-medium blood vessels, causing tissue ischemia and necrosis. The pain is excruciating, emanating from tender lesions, severely impacting movement and touch. No approved treatment exists, making effective pain management vital for improving quality of life and outcomes. CASE REPORT: A woman with end-stage renal failure and calciphylaxis-related ulcers on the inferior pannus, left medial thigh, and right gluteus experienced refractory pain despite opioid therapy. Palli…

[Re-do operation for coronary stenosis after Cabrol procedure; report of a case].

The Cabrol technique has been used to reimplant coronary arteries at the time of aortic root replacement. We describe a case of coronary stenosis after Cabrol procedure. A 42-year-old female with Marfan syndrome and chronic type B dissection who underwent aortic root replacement with Cabrol procedure complained of chest pain. Computed tomography( CT) showed extended thoraco-abdominal aortic aneurysm. Angiography revealed bilateral Cabrol limb stenosis at anastomotic sites. Re-do operation was performed under median re-sternotomy. Pannus around the coronary anastomosis was resected and patch ex…

Redo aortic valve replacement with ''porcelain'' aorta in an aortitis patient. A case report.

We report a successful reoperation of aortic valve replacement with the severely calcified, ''porcelain'' aorta due to Takayasu's disease. The patient was a 46-year-old female diagnosed with aortic prosthetic valve failure. Takayasu's aortitis had been noted at 17 years old and the patient underwent aortic valve replacement for aortic regurgitation at age 32. Chest CT scan revealed severe circumferential calcification of the whole thoracic aorta. Echocardiography revealed 100 mmHg of pressure gradient across the aortic prosthetic valve with pannus formation. Replacement of the aortic prostheti…

Congenital aniridia: a histopathologic study of the anterior segment in children.

Seven eyes from seven children with congenital aniridia were studied histologically in order to assess the relationship between congenital a … of Bowman's membrane. Three acquired abnormalities of the anterior segment were identified: corneal pannus, peripheral anterior synechiae, and lenticular degeneration. The two cases showing anomalous development of the anterior chamber angle occurred in children with a partial deletion of the short arm of chromosome 11.

Elevated levels of collagenase and prostaglandin E2 from synovium associated with erosion of cartilage and bone in a patient with chronic Lyme arthritis.

A patient with chronic Lyme arthritis and roentgenographic evidence of bony erosion underwent a synovectomy; proliferative synovium (pannus), containing aggregates of small lymphocytes, was found adherent to eroded cartilage and bone. During 8 days in tissue culture, the synovial cells produced large amounts of collagenase and prostaglandin E2, but only low levels of both neutral and acid proteinases. Sixty-seven percent of the lymphocytes from the synovium were T cells; 19% were B cells. Attempts to identify agent/antigen in the synovial cells were unsuccessful. Thus, the synovium of this pat…

Pulmonary valve-in-valve implants: how long do they prolong reintervention and what causes them to fail?

BACKGROUND: The valve-in-valve (VinV) procedure is a minimally invasive, transcatheter, off-pump, alternative to conventional valve replacem … due to the proximity of the host's tissues to the prosthetic device, resulting in a combination of pannus, calcification, and a cusp tear. CONCLUSIONS: Additional long-term follow-up of pulmonary VinV implantations is needed in order to determine the life span of VinVs and what causes them to fail.

Malfunction of a Hancock bioprosthesis in the mitral position 24 years after initial implantation.

BACKGROUND AND AIMS OF THE STUDY: Although many types of cardiac bioprosthetic valves are currently available, the major disadvantage of the … showed leaflet perforations and a commissural tear, moderate calcification on the commissures, and pannus overgrowth. CONCLUSION: The findings in this case suggest that individual patient variables, as well as the design and the construction of the valve are major determining factors in the rate of valve failure.

Transesophageal echocardiography for prosthetic valve evaluation: is it always necessary?

Transesophageal echocardiography provides excellent images of mitral and aortic prostheses, due to its higher resolution and less interferen … eal echocardiography can also have advantages in recognizing obstructive lesions such as thrombi or pannus, and help understand the complex morphology of prostheses in grafts. Transesophageal echocardiography should be considered a backup technique whenever transthoracic echocardiography, which always remains the primary approach, is inconclusive or nega …

Structural valvular deterioration of Hancock standard valve in the mitral position 33 years after initial implantation.

A 77-year-old woman who had undergone mitral valve replacement (MVR) with a 29 mm Hancock standard (H-S) bioprosthesis (Model 242) and tricu … planted bioprosthesis showed mild calcification and a tear in the leaflet, dehisced commissures and pannus overgrowth. To our knowledge there are no reports describing H-S valves that were still functioning over 30 years after implantation. Herein, we report a case of reMVR in a patient with an H-S valve that had been implanted 33 years previously.

Keratoconjunctivitis associated with sialodacryoadenitis in rats.

A high incidence of keratoconjunctivitis was observed in a closed colony of inbred Lewis/Wistar rats. Clinical signs including blinking, ocular discharge, circumcorneal flush, corneal opacity, ulceration, pannus, hypopyon, and hyphema were observed at about three weeks of age. Acute disease subsided by six weeks of age, but some lesions progressed to low-grade chronic keratitis. Six per cent of affected rats developed megaloglobus, which usually appeared by three weeks of age. Lesions included focal or diffuse interstitial keratitis, corneal ulceration, anterior synechia, and inflammatory exud…

Mapping the synovial immune ecosystem in rheumatoid arthritis: cellular cartography and pathotype-guided immune restoration.

Rheumatoid arthritis (RA) is not a uniform inflammatory pannus. It is a spatially organized, stage-dependent synovial immune ecosystem. Cellular cartography resolves synovitis into lymphoid, myeloid, stromal, and neuroimmune niches that track erosive disease, fibroproliferation, and pain-dominant states. Ecosystem state can differ between joints and shift over time, explaining a key paradox: inflammation may fall while relapse risk or pain persists. Longitudinally, RA evolves from preclinical immune dysregulation with mucosal priming to joint niche consolidation, then to refractory states rein…

Surgical Treatment of Juvenile Idiopathic Arthritis in the Era of Novel Drug Therapies.

Juvenile idiopathic arthritis is the most common chronic rheumatic disease encountered in children under the age of sixteen and causes significant impairments in daily life. Over the last two decades, the introduction of new drug treatments (including disease-modifying antirheumatic drugs and biologics) has changed the course of this disease, thus reducing the indication for surgery. However, some patients fail to respond to drug therapy and thus require personalized surgical management, e.g., the local reduction of joint effusion or a synovial pannus (via intra-articular corticosteroid inject…

Characteristics of Vernal Keratoconjunctivitis in Lhasa: A Single-Center, Observational Study.

The incidence of vernal keratoconjunctivitis (VKC) varies across different ethnicities and regions. To investigate the clinical features of VKC in Tibet, a single-center case series of VKC was retrospectively reviewed from a general hospital in Lhasa over one year. General data and typical manifestations were recorded. Horner-Trantas dots, limbal neovascularization, pseudogerontoxon and upper palpebral conjunctival papilla were seen in most of these patients. Patients with signs such as limbal pannus (t = 2.18, P < 0.05), pseudogerontoxon (t = 2.61, P < 0.05), and Horner-Trantas dots (t = 2.33…

Massive Localized Abdominal Lymphedema Treatment Challenges.

Massive localized lymphedema of the abdomen is a rare condition resulting from a neglected lower abdominal pannus associated with significant disability and morbidity. Compared to other surgical procedures, postbariatric surgery is usually considered a financial drain. In the United Kingdom, this requires National Health Service approval and delays may lead to sequelae that adversely impact on patients' quality of life with increased morbidity. We present a wheelchair-bound patient whose body mass index increased from 53 to 82, while awaiting funding approval increasing her anesthetic and surg…

Corneal changes in familial iris coloboma.

We recently studied members from a family with autosomal dominant iris coloboma. All affected members had either unilateral or bilateral iris coloboma and bilateral peripheral corneal changes indistinguishable from those seen in aniridia. The corneal changes consisted of well-demarcated pannus-like growths extending from the limbus onto the peripheral cornea for 360 degrees without any stromal vascularization. In one adolescent member, bilateral lenticular opacities limited to the meridian of the colobomata were seen. No members had primary glaucoma, macular or optic disc hypoplasia, nystagmus…

[Peripheral ulcerative keratitis associated with dupilumab: a case report].

A 68-year-old male developed foreign body sensation in both eyes, photophobia and tearing in the right eye approximately 6 weeks after initiating dupilumab for severe atopic dermatitis. The right eye presented a crescent-shaped superficial-stromal ulcer in the peripheral cornea with an undermined edge, which was separated from the limbus of the cornea by a clear zone. The left eye had a pannus at the limbus of the cornea. Corneal scraping, bacterial culture, and fungal culture excluded infections. The patient was administered 0.1% fluorometholone eye drops and recombinant bovine basic fibrobla…

Histological Analyses of Arthritic Joints in Collagen-Induced Arthritis Model Mice.

Histological analysis is a morphological technique and an effective method for understanding the pathology of rheumatoid arthritis (RA). RA is an inflammatory disease characterized by increased synovial tissue and osteoclasts, angiogenesis, infiltration of inflammatory cells, and pannus formation. These pathologies can be observed in a collagen-induced arthritis model mouse using formaldehyde-fixated paraffin-embedded (FFPE) samples. For the preparation of FFPE samples, the conditions of the fixation and decalcification process significantly affect tissue staining results. Since the lesion sit…
