Down syndrome and the autonomic nervous system, an educational review for the anesthesiologist.

Approximately one in every 700 babies in the United States is born with Down syndrome, or 0.14%. Children with Down syndrome have cognitive impairment and congenital malformations necessitating frequent occurrences of general anesthesia and surgery. The thoughtful perioperative care of children with Down syndrome is relevant and acutely complex for the pediatric anesthesiologist. Behavior, sedation, hypotonia, upper airway obstruction, venous access, and bradycardia are omnipresent concerns apart from the surgical pathology. Down syndrome is also associated with autonomic nervous system dysfun

Device-based neuromodulation for cardiovascular diseases and patient' s age.

The autonomic nervous system plays an important role in the pathogenesis of cardiovascular diseases. With aging, autonomic activity changes, and this impacts the physiological reactions to internal and external signals. Both sympathetic and parasympathetic responses seem to decline, reflecting functional and structural changes in nervous regulation. Although some investigators suggested that both the sympathetic and parasympathetic activities were suppressed, others found that only the parasympathetic activity was suppressed while the sympathetic activity increased. In addition, cardiac innerv

The Bezold-Jarisch Reflex and The Inflammatory Response Modulation in Unanesthetized Endotoxemic Rats.

Evidence indicates that the activation of the parasympathetic branch of the autonomic nervous system may be effective in treating inflammatory diseases. Previously, we have described that baroreflex activation displays anti-inflammatory properties. Analogous to the baroreflex, the Bezold-Jarisch reflex also promotes parasympathetic activation with simultaneous inhibition of the sympathetic system. Thus, the present study aimed to evaluate whether the activation of the Bezold-Jarisch reflex would also have the ability to reduce inflammation in unanesthetized rats. We used lipopolysaccharide (LP

Evidence of deviant parasympathetic response to social exclusion in women with borderline personality disorder.

Stressful social situations like social exclusion are particularly challenging for patients with borderline personality disorder (BPD) and o … nomic response was reversed and blunted in BPD. Our findings suggest that during social stress, the parasympathetic nervous system fails to display an adaptive regulation in patients with BPD, but not HC. Understanding the autonomous signature of the stress response in BPD allows the formulation of clinically relevant and biologically plausible interventions to counteract paras …

Differential cardiovascular effects of pharmacological agents in chickens selected for high and low body weight.

This study was conducted to investigate whether there are differences in the autonomic nervous system function of chickens from lines select … ephrine was given following atropine. This pattern of response suggested that HWS birds had greater parasympathetic nervous system activity in order to maintain cardiovascular function. These results demonstrate that selection for HWS or LWS has resulted in greater parasympathetic and sympathetic nervous system tone in birds from the HWS and LWS birds, respectively, and suggest …

Review of different methods used for clinical recognition and assessment of pain in dogs and cats.

In light of the need to perform surgical techniques and the importance of animal welfare because of acute pain, the objectives of the veterinary anaesthetists are to manage muscle relaxation and adequate analgesia in order to conserve a balance in the autonomic nervous system, enhance the action of the parasympathetic system in the face of the emerging action of the sympathetic portion provoked by the surgeon, and maintain a balance among them. The aim of the present review is to describe different evaluation criteria for acute pain using unidimensional and multidimensional scales, correlating

[The effect of the vibration and noise factor on the physical work capacity and autonomic nervous system function of workers in vibration-hazardous jobs].

An unexampled study was made of the peripheral vegetative incompetence syndrome developing in vibration disease. It is shown that chronic occupational exposure to vibration and noise results in damage to the segmentary apparatus of the vegetative nervous system. Vegetative inadequacies are manifested by impairement of cardiovascular functions. With exposure to vibration and noise, it is the sympathetic portion of the vegetative nervous system that is first affected. The service duration-related progression of the pathological process results in increase of the share of parasympathetic patholog

Cysto-urethrographic appearance of the bladder and posterior urethra in neuromuscular disorders of the lower urinary tract.

Cysto-urethrography in the straight lateral projection with simultaneous intravesical pressure recording was performed in 57 patients with localized neurological lesions at different levels. All patients were previously extensively evaluated urodynamically. Bladder trabeculation was not related to level of neurological lesion or reflex pattern of the detrusor. Serration of the bladder wall was a reliable sign of a contracting detrusor in bladders without severe trabeculation. Open bladder neck at rest was related to lesion of the peripheral parasympathetic nervous system, while supposed insuff…

The role of the autonomic nervous system in polycystic ovary syndrome.

This article reviewed the relationship between the autonomic nervous system and the development of polycystic ovary syndrome (PCOS). PCOS is the most common reproductive endocrine disorder among women of reproductive age. Its primary characteristics include persistent anovulation, hyperandrogenism, and polycystic ovarian morphology, often accompanied by disturbances in glucose and lipid metabolism. The body's functions are regulated by the autonomic nervous system, which consists mainly of the sympathetic and parasympathetic nervous systems. The autonomic nervous system helps maintain homeosta…

Time-Dependent Autonomic Dysregulation and Co-Activation Induced by Periodic Limb Movements in Sleep.

Background: Periodic limb movements in sleep (PLMS) are characterised by repetitive, involuntary limb movements that occur during sleep and are often associated with autonomic nervous system dysregulation. While it is known that PLMS influence cardiovascular parameters, the exact role of heart rate variability (HRV) and the balance between sympathetic and parasympathetic activity remains unclear. Previous studies have suggested that longer PLMS events may trigger more pronounced autonomic responses, but the relationship between the duration of PLMS and autonomic dynamics has yet to be fully ex

[Erectile dysfunction--value of neurophysiologic diagnostic procedures].

Neurogenic, particularly autonomic disorders, frequently contribute to the etiology and pathophysiology of erectile dysfunction. Parasympathetic and sympathetic outflow mediates erection. Noncholinergic, nonadrenergic neurotransmitters induce activation of cyclic monophosphates, leading to relaxation of smooth muscles of the corpora cavernosa and by this to tumescence and rigidity, i.e. erection. The diagnosis of neurologic causes of erectile dysfunction requires a detailed history and neurologic examination. Conventional neurophysiological procedures evaluate the function of rapidly conductin

[Normal and abnormal response of the arterial pressure to standing (author's transl)].

Redistribution of blood volume during orthostatism is followed by a reduction in venous return and therefore cardiac output. A drop in the blood pressure following postural changes is normally prevented by the action of the baroreceptor reflex, which leads to an increase in total peripheral resistance and heart rate. Orthostatic hypotension may arise from a variety of causes. In some cases, the disorder of postural regulation is due to an autonomic nervous system disease, and various tests are available that enable the diagnosis to be confirmed and the mechanism of action to be elucidated. Sympathetic control of accommodation: evidence for inter-subject variation.

Autonomic innervation of ciliary smooth muscle is mediated principally by the parasympathetic nervous system and is supplemented by the sympathetic nervous system. Previous drug and nerve stimulation experiments on humans and animals have demonstrated that sympathetic innervation is inhibitory (via beta-2 adrenoceptors), relatively small, slow and augmented by concurrent levels of background parasympathetic activity. These characteristics are pertinent to the sympathetic system having a specific role in our ability to adapt successfully to sustained near vision tasks and, given the clear assoc…

Early parasympathetic dysfunction in Parkinson's disease: insights from information-theoretic analysis.

INTRODUCTION: Parasympathetic nervous system (PNS) dysfunction in Parkinson's disease (PD) has been frequently evaluated using heart rate variability (HRV) analysis in the time and frequency domains. Findings across studies have been inconsistent, limiting a unified understanding of early autonomic impairment. METHODS: In this study, we applied both conventional and advanced analytical methods to evaluate cardiovascular PNS function in the early-stage PD patients. Sixteen individuals with PD (<6 months after motor signs occurrence) and sixteen age- and sex-matched healthy controls were assess…

The role of youths' cardiac autonomic balance and parental responses to youth emotion in vulnerability to borderline personality disorder development.

Developmental models of borderline personality disorder (BPD) emphasize the effects of youths' biological vulnerabilities and their experiences of parental responses to emotion, as well as the interaction between these two elements. The current study evaluated the independent and interactive effects of two indices of autonomic nervous system response and parental responses to youth negative emotions on severity and exacerbation of youths' BPD features during the transition to adolescence. The sample consisted of 162 psychiatric youth (10-14 years; 47.2% female) and their parents. At baseline,

Neuroimmunology of cancer and associated symptomology.

Evolutionarily, the nervous system and immune cells have evolved to communicate with each other to control inflammation and host responses against injury. Recent findings in neuroimmune communication demonstrate that these mechanisms extend to cancer initiation and progression. Lymphoid structures and tumors, which are often associated with inflammatory infiltrate, are highly innervated by multiple nerve types (e.g. sympathetic, parasympathetic, sensory). Recent preclinical and clinical studies demonstrate that targeting the nervous system could be a therapeutic strategy to promote antitumor i

Role of the autonomic nervous system in modulating cardiac arrhythmias.

The autonomic nervous system plays an important role in the modulation of cardiac electrophysiology and arrhythmogenesis. Decades of research has contributed to a better understanding of the anatomy and physiology of cardiac autonomic nervous system and provided evidence supporting the relationship of autonomic tone to clinically significant arrhythmias. The mechanisms by which autonomic activation is arrhythmogenic or antiarrhythmic are complex and different for specific arrhythmias. In atrial fibrillation, simultaneous sympathetic and parasympathetic activations are the most common trigger.

The impact of cervical manipulation on heart rate variability.

Heart Rate variability (HRV) is the inter-beat variability in heart rate and is moderated by the balance of sympathetic and parasympathetic divisions of the autonomic nervous system. Electrocardiography (ECG) can be utilized to obtain Low frequency (LF) to high frequency (HF) ratios that represent sympathetic to parasympathetic response, respectively and these ratios may be increased in people with chronic pain. Spinal manipulation is often used to manage musculoskeletal disorders such as neck pain. This study assesses the influence of cervical manipulation on HRV using LF/HF ratio. Ten subjec

Autonomic nervous system activity in constipation-predominant irritable bowel syndrome patients.

BACKGROUND: The main mechanism underlying irritable bowel syndrome is currently believed to be a dysfunction of the brain-gut axis. Autonomic nervous system dysfunction can contribute to development of irritable bowel syndrome symptoms by disturbing visceral sensations. MATERIAL/METHODS: Thirty patients with a diagnosis of constipation-predominant irritable bowel syndrome and 30 healthy volunteers were included in the study. Resting and functional autonomic nervous system tests and percutaneous electrogastrography were performed. Plasma adrenalin, noradrenalin, insulin, ghrelin and cholecysto

Effect of itch, scratching and mental stress on autonomic nervous system function in atopic dermatitis.

Atopic dermatitis is a stress-responsive disorder that involves the autonomic nervous system. The current study used heart rate variability to examine the effect of itch, scratching and mental stress in atopic patients with moderate to severe disease. Twenty-one patients with active disease and 24 healthy volunteers participated in the study. Heart rate variability measurements were taken at 5 min intervals at rest and after each of 3 acute stress tests, which included histamine-induced itch at the forearm, scratching around the itch site, and the Trier Social Stress Test. Atopic patients disp
