Transureteroureterostomy and terminal loop cutaneous ureterostomy in advanced pelvic malignancies.

Transureteroureterostomy was combined with terminal loop cutaneous ureterostomy, without complications, in 8 patients with advanced pelvic malignancy and a poor prognosis. Urinary diversion was palliative in all patients and followed pelvic exenteration in 4, debulking of pelvic tumor in 2 and radical cystectomy in 1, while 1 had inoperable bladder cancer. All patients had at least unilateral hydroureteronephrosis preoperatively. In each case a postoperative excretory urogram revealed significant improvement of the hydroureteronephrosis and the serum creatinine improved or stabilized. No patie

Absorbable synthetic mesh (polyglactin 910) for the formation of a pelvic "lid" after radical pelvic resection.

Isolation of the abdominal contents from the denuded, potentially infected pelvis after radical pelvic surgery has been advocated. Of the many materials evaluated, omentum is usually chosen. In six patients in whom omentum was unsuitable, we have substituted polyglactin 910 (Vicryl, Ethicon, Sommerville, New Jersey) mesh to form a pelvic "lid." In these patients, omentum was either densely adherent in the upper abdomen because of prior surgery or was removed as part of surgical treatment for recurrent cancer. Vicryl mesh formed a "lid" across the true pelvis. No gastrointestinal complications

Vaginoplasty.

The historical development of vaginal reconstruction in vaginal atresia is reviewed. Two more recent indications for vaginoplasty are (i) total pelvic exenteration for carcinoma; and (ii) sex conversion in trans-sexuals. The operations available for these conditions are discussed, with particular emphasis on sex-conversion techniques. A method of vaginoplasty in inadequately treated transsexuals, using random medial thigh flaps, is presented with illustrative case reports.

Double-barreled wet colostomy: urinary and fecal diversion.

PURPOSE: Double-barreled wet colostomy represents simultaneous urinary and fecal surgical diversion performed most commonly after pelvic exenteration as a palliative procedure or after actinic damage. We report the structural and functional results of double-barreled wet colostomy with special attention to surgical technique, morbidity and functional results compared to those described in the available literature. MATERIALS AND METHODS: We retrospectively followed 38 patients who underwent double-barreled wet colostomy at our institution from April 2003 to November 2007. The parameters were p

Small bowel obstruction associated with post-hysterectomy vaginal vault prolapse.

BACKGROUND: Patients may present with post-hysterectomy vaginal vault prolapse in conjunction with small bowel obstruction. Prior pelvic surgery, malignancy, and radiation therapy may be associated with this presentation. CASE: An 83-year-old multiparous woman with a history of poorly differentiated endometrial adenocarcinoma was treated with radiation therapy, total abdominal hysterectomy, and salpingo-oophorectomy. Anterior exenteration was performed for a recurrence. Seventeen years after her last pelvic operation, she had small bowel obstruction that coincided with a worsening post-hyster

The "Pelvic Bowl" Framework: An Objective-centered Approach to Perineal Reconstruction After Extirpative Pelvic Surgery.

Perineal and pelvic reconstruction after abdominoperineal resection or pelvic exenteration is challenging due to high rates of wound complications, perineal hernia, and empty pelvis syndrome. Morbidity arises from residual dead space, poor vascularity, and inadequate pelvic floor support. We performed a narrative review synthesizing evidence on perineal and pelvic reconstruction following extirpative pelvic surgery, critically appraising existing decision-making algorithms through the lens of 3 core reconstructive objectives: pelvic floor support, dead space obliteration, and perineal resurfac

Radical surgery for extensive rectal cancer: is it worthwhile?

In a small proportion of patients with extensive primary or locally recurrent rectal cancer, disease remains confined to the pelvis for a prolonged period. Symptoms are highly prejudicial to quality of life and often refractory to treatment short of extirpative surgery. Cure requires en bloc excision of all involved pelvic viscera with tumor-free margins. The pelvic exenterations (PE) are the most radical operations for rectal cancer. PE carries a high risk of perioperative morbidity and mortality, and has profound functional, psychological, and psychosexual implications for patients. Careful

Splenic metastasis from cervical cancer: case report and review of the literature.

Cervical cancer isolated splenic recurrence is a rare event. We report a case of splenic cervical cancer metastasis treated with salvage surgery 30 months after supralevator anterior pelvic exenteration. In this patient the plasmatic level of squamous cell carcinoma antigen increased in the absence of any evidence of disease at the instrumental examination 24 months after surgery. Surgery had a high impact on quality of life.

The Martius flap neovagina revisited.

The Martius bulbocavernosus flap has been used frequently in the repair of various vaginal fistulas. It can also be a useful source for the construction of a neovagina if used in the appropriate patient. We present a case of a patient who underwent a total pelvic exenteration for recurrent cervical cancer. For her neovagina, a novel variation of the Martius flap was constructed. In carefully selected patients, this can be a useful option for neovagina reconstruction.

Ureteroarterial fistula: a case report.

We report on a patient with a fistula between the right common iliac artery and the distal right ureter who had undergone pelvic exenteration for carcinoma of the uterine cervix. The patient also had received prior radiation therapy and was being treated with an indwelling ureteral stent at the time the fistula developed. Diagnosis was made by an occlusive ureterogram and the lesion was treated successfully with embolization of the common iliac artery.

Pelvic and Perineal Reconstruction.

LEARNING OBJECTIVES: After studying this article, the participant should be able to: 1. Classify types of bony pelvic resections. 2. Outline reconstructive options for pelvic and perineal defects. 3. Identify advantages and drawbacks of various reconstructive techniques. 4. Recognize the functional benefits of bony and soft-tissue reconstruction. SUMMARY: Defects of the pelvis and perineum arise from tumors, trauma, infection, congenital differences, and gender incongruence. Pelvic resections can result in bony instability and soft-tissue deficiency. The goals of reconstruction are maintenanc

Functional Outcomes of Orthotopic Neobladder in Women.

PURPOSE OF REVIEW: This review paper summarizes the available literature on the evolution of surgical approach to radical cystectomy in female bladder cancer patients and its impact on functional outcomes in orthotopic neobladder. RECENT FINDINGS: Traditionally, radical cystectomy in female bladder cancer patients has been maximally extirpative with pelvic exenteration. Recently, new techniques which include pelvic organ-sparing, nerve-sparing and vaginal-sparing have demonstrated improved rates of urinary incontinence and retention. Additional techniques include prophylactic apical suspensio

Pure large-cell neuroendocrine carcinoma of the bladder with pelvic organ invasion: A case report.

Large-cell neuroendocrine carcinoma (LCNEC) of the bladder is a rare, high-grade malignancy with aggressive behavior. We report a 69-year-old woman presenting with recurrent macroscopic hematuria. Imaging revealed a large tumor of the bladder floor involving the right ureterovesical junction, with local extension to adjacent pelvic organs but no nodal or distant metastases. Histology and immunohistochemistry confirmed pure LCNEC. She received four cycles of carboplatin-etoposide, followed by anterior pelvic exenteration with ileal conduit diversion. Postoperative recovery was uneventful, and e

Advanced female urethral adenocarcinoma: A case report.

Urethral cancer in females is a rare occurrence. Although initially believed to be derived from skene glands, there is no consensus on the cell of origin. Squamous cell carcinoma is the most common, whereas adenocarcinoma is the least common. We report the case of a 58-years-old lady with a history of voiding lower urinary tract symptoms, later requiring a suprapubic cystostomy for retention. Cystoscopy and biopsy showed prostate-specific antigen (PSA) negative adenocarcinoma and postiron emission computed tomography (PETCT) showing pelvic and inguinal nodal metastasis. The patient underwent n

Adenocarcinoma of the paraurethral glands: a case report.

Adenocarcinoma of the paraurethral glands represents a very rare neoplasm of the urinary tract. Due to the rarity of this disease, there is no standard therapeutic approach. We report a case of adenocarcinoma of the paraurethral glands in a 56-year-old woman, presenting with abnormal serous vaginal discharges. The radiologic examination revealed a 5-cm mass around the urethra, which underwent surgical resection. After surgical resection, the histology revealed a moderately differentiated adenocarcinoma, probably arising from the paraurethral glands. One month later, a pelvic recurrent mass was

Bladder-sparing surgery in locally advanced nonurological pelvic malignancy.

INTRODUCTION: The urinary bladder is commonly involved in pelvic malignancy. The incidence of apparent extension into adjacent organs in locally advanced colorectal malignancy is 5-12%. It is not known with other pelvic malignancy. No guidelines are available for its management. Often a dilemma exists between cystectomy and a bladder-sparing procedure. We studied the validity of bladder-sparing surgery (BSS) in locally advanced nonurological pelvic malignancy. METHODS: Hospital records of patients who underwent BSS along with other surgeries (abdomino-perineal resection, anterior resection, a

Invasive giant condylomata acuminata: a case report.

We report on a patient with invasive giant condylomata acuminata (Buschke-Loewenstein tumor). This tumor is an exophytic human papilloma viral lesion with downward growth without histological malignancy. The aggressive progression of this lesion requires early surgical excision with clear margins. Delay in recognition of this tumor can otherwise lead to a fatal clinical course.

[Legg-Perthes-Calvé disease. Plea for an original ambulatory orthosis].

Legg-Perthes-Calvé disease is a local disease which should be treated by non-iatrogenic means. The authors have designed a new ambulatory orthosis allowing containment in abduction, flexion and medial rotation, with relief of weight-bearing on the femoral head. It was tested in thirty six hips. In only eight of them a surgical procedure had to be performed during the evolution of the disease (femoral or pelvic osteotomy) because of secondary exenteration of the hip.

Assessment of Salvage Surgery in Persistent Cervical Cancer after Definitive Radiochemotherapy: A Systematic Review.

Background and Objectives: The standard treatment approach in locally advanced cervical cancer (LACC) is exclusive concurrent chemoradiation therapy (RTCT). The risk of local residual disease after six months from RTCT is about 20-30%. It is directly related to relapse risk and poor survival, such as in patients with recurrent cervical cancer. This systematic review aims to describe studies investigating salvage surgery's role in persistent/recurrent disease in LACC patients who underwent definitive RTCT. Materials and Methods: Studies were eligible for inclusion when patients had LACC with ra
