Adrenal androgen-secreting carcinoma in a fertile woman.

The article reports on a case of functioning adrenal carcinoma in a woman of 40 with concomitant in situ carcinoma of the portio (CIN 3). She had had amenorrhoea for 14 months and early menopause was suspected. Hirsutism and other signs of virilization such as clitoromegaly and voice changes were detected upon hospitalization.. The characteristic of the clinical case is given by hormone production involving not only androgens of prevalently adrenal genesis (dehydroepiandrosterone, dehydroepiandrosterone sulphate and androstenedione), but also testosterone, more suggestive of a gonadal neoplasm

Radiation therapy: a primary treatment modality for seminoma.

It would appear that: (1) Seminomas are radiosensitive neoplasms. (2) Patients with stage I seminoma, including anaplastic seminoma, should receive radiation therapy to the para-aortic and ipsilateral pelvic and inguinal nodes. (3) Most patients with stage II seminoma (including anaplastic seminoma) with negative human chorionic gonadotropin results also respond well to radiation therapy that includes the mediastinum and appropriate supraclavicular area. However, surgery and chemotherapy are indicated in patients with bulky stage II disease and in those patients with persistent elevation of po

Clinical management of a unique case of PNET of the uterus during pregnancy, and review of the literature.

RATIONALE: PNETs (primitive neuroectodermal tumors) are a family of highly malignant neoplasms characterized by small round cells of neuroepithelial origin. They usually involve bone and soft tissues, and have a higher incidence in childhood. PATIENT CONCERNS: In this case report, we describe the obstetric and oncological outcome of a huge mass diagnosed as a leiomyoma in a 39-year-old pregnant woman who complained of low back pain, dysuria, and urinary frequency at 22 weeks of gestation. DIAGNOSES: During the 25th week of pregnancy, the patient was referred to our hospital at night with sev

Intravascular papillary endothelial hyperplasia within the uterine myometrium: a case report and literature review.

Intravascular papillary endothelial hyperplasia (IPEH) is a rare vascular lesion that can histologically mimic malignant vascular tumors, particularly angiosarcoma, often leading to misdiagnosis and overtreatment. IPEH involving the uterine myometrium is a novel finding. We describe an 83-year-old postmenopausal woman presenting with a 6-day history of spontaneous vaginal bleeding. Pelvic examination revealed a patent vagina with minimal dark red blood and an enlarged, anteverted uterus. Pelvic magnetic resonance imaging showed a well-demarcated, heterogeneous intramural lesion (6.0 × 5.5 × 5.

MR imaging of the female pelvis: current perspectives and review of genital tract congenital anomalies, and benign and malignant diseases.

MR imaging continues to be an integral problem-solving modality in the evaluation of congenital anomalies and acquired diseases of the female genital tract organs and provides effective clinical information to the practicing gynecologist in those patients in whom sonography is technically suboptimal or the results are equivocal. This article describes the state-of-the art MR imaging of the female pelvis and addresses its current perspectives in the following sections: (1) technical aspects of MR in imaging the female pelvis, (2) normal pelvic anatomy and variations that are seen on MRI, (3) ro

Total Pelvic Exenteration with Presacral Fascia and Pelvic Musculature Excision for Pelvic Recurrence of Rectal Cancer.

BACKGROUND: Local relapse has not been eradicated even in the era of total mesorectum excision.1,2 Although various approaches have been attempted, R0 resection remains the only potentially curative treatment.3,4 PATIENT AND METHODS: A 45-year-old woman with a history of laparoscopic abdominoperineal resection was diagnosed with pelvic recurrence 7 months ago. Then, she received chemoradiotherapy. Magnetic resonance imaging (MRI) and positron emission tomography-computed tomography (PET-CT) showed pelvic recurrences at the surface of the right piriformis and obturator internus, posterior vagin

Mesenchymal neoplasms with RAF/BRAF alterations: eight cases revealing novel fusions, V600E mutation and clonal evolution.

OBJECTIVE: While kinase-altered spindle cell tumours were classically recognised in paediatric superficial soft tissues, recent literature indicates an expanding clinicopathologic spectrum. This study investigates a cohort of RAF/BRAF-altered mesenchymal tumours to further characterise their presentations in adults, deep-seated/visceral locations, variable immunophenotypes and complex molecular evolution. METHODS: Eight molecularly confirmed RAF/BRAF-altered mesenchymal tumours were retrospectively evaluated. Clinical characteristics, histomorphology, immunohistochemistry, targeted next-gener

Large Submucosal Uterine Leiomyoma Prolapse Into the Vagina After Complicating a Pregnancy: A Case Report.

Uterine leiomyomas are the most common pelvic neoplasm in females. They are non-cancerous monoclonal tumors that develop from the fibroblasts and smooth muscle cells of the myometrium. They can develop in females of reproductive age and post-menopausal as well. When symptomatic, they frequently manifest as abnormal uterine bleeding and/or pelvic pain or pressure. Reproductive effects are also possible in fibroids such as infertility and poor pregnancy outcomes. In this report, we present a case of a 39-year-old woman, G9P4A4, GA 16 weeks who came to the ER with an incomplete abortion at home a

PAX8 Positivity, Abnormal p53 Expression, and p16 Negativity in a Primary Endometrial Squamous Cell Carcinoma: A Case Report and Review of the Literature.

Primary endometrial squamous cell carcinoma (PESCC) is a rare entity. As the clinicopathologic features and the immunophenotype have not been completely defined yet, here we report our experience and review of the literature on this topic. A 73-yr-old nulliparous woman presented with pelvic pain and vaginal bleeding. Endometrial biopsy showed a carcinoma with squamous differentiation infiltrating the myometrium. Total hysterectomy with bilateral salpingo-oophorectomy and selective pelvic lymphadenectomy was performed. Definitive diagnosis was squamous carcinoma of the endometrium, with one lym

Pubertal Development as a Marker of Gonadal Neoplasm in 46XY Complete Gonadal Dysgenesis.

BACKGROUND: 46XY Complete Gonadal Dysgenesis (CGD), also known as Swyer syndrome, is characterized by nonfunctional streak gonads and an increased risk of gonadal neoplasms. While spontaneous puberty is not expected, hormonally active gonadal tumors can mimic normal puberty delaying diagnosis. CASE PRESENTATION: We present two adolescents with 46XY CGD and tumor-driven puberty: a 16-year-old with spontaneous thelarche and adrenarche found to have a left gonadoblastoma, and a 14-year-old with spontaneous menarche and pelvic pain who was subsequently diagnosed with a dysgerminoma and a sex cord

A Case Report of the Synchronous Occurrence of Ovarian Granulosa Cell Tumour and Malignant Endometrial Polyp with Immunohistochemical Expression of Hormone Receptors and Biomarkers p-53 and Ki-67.

Background and Clinical Significance: Abnormal uterine bleeding during the postmenopausal years is a pathological sign that may be due to simultaneous intrauterine and ovarian pathology. Granulosa cell tumours of the ovary are malignant neoplasms producing oestradiol, which leads to the abnormal proliferation of the endometrium, precancerous lesions, and endometrial carcinoma type I. Case Presentation: The authors present a clinical case of a 67-year-old woman with postmenopausal bleeding who underwent a total abdominal hysterectomy with bilateral adnexectomy, pelvic lymphadenectomy, and parti

Mixed Large Cell Neuroendocrine Carcinoma and Endometrioid Adenocarcinoma: A Rare Case Report with Review of Literature.

We report the case of a 60-year-old female who presented with postmenopausal bleeding after she underwent investigations followed by a total abdominal hysterectomy with bilateral salpingo-oophorectomy and pelvic lymphadenectomy. A diagnosis of low-grade endometrioid adenocarcinoma admixed with large cell neuroendocrine carcinoma (NEC) was made based on the histomorphology and immunohistochemical profile. NEC of the endometrium is a rare and highly aggressive neoplasm requiring a multidisciplinary approach for its treatment. As treatment strategies are changing over time, preoperating imaging e

Eccrine porocarcinoma presenting with unusual clinical manifestations: a case report and review of the literature.

Eccrine porocarcinoma is a rare neoplasm that originates from the intraepidermal ductal portion of the eccrine sweat gland, and it usually occurs on the pre-existing lesion of benign eccine poroma. Its occurrence is more common in females and elderly persons. We present a case of a 44-year-old man who suffered from eccrine porocarcinoma, which developed on the right scrotum and pelvic area with metastases to the lung, adrenal gland, esophagus, ureter, and distant lymph nodes. Here we report on a unique case of eccrine porocarcinoma that developed primarily on the scrotum, which is an uncommon

Aggressive angiomyxoma of the vulva.

BACKGROUND: Aggressive angiomyxoma is a rare soft tissue tumour that carries a high risk of local relapse. It is a slowly growing and locally infiltrating tumour. CASE: We describe the case of an aggressive pelvic-perineal angiomyxoma arising in a 36-year-old woman. The patient had a mass that grew before, during and after her pregnancy. Transperineal surgery was performed. The resection margins were free of disease. CONCLUSION: Our case confirms what has previously been published concerning the possible hormone-dependence of this neoplasm. Given the positive estrogen and progesterone recept

Mesenchymal chondrosarcoma of the foot, an unusual location: case report and review of the literature.

We present a case of primary mesenchymal chondrosarcoma of the proximal phalanx of the first toe. The bones of the foot represent an infrequent primary site for this neoplasm. The tumour consisted of layers of undifferentiated round cells with scanty cytoplasm and hyperchromatic nuclei. The presence of brain, lung, and left auricle metastasis was demonstrated, and the patient died due to brain edema 18 days after admission. Mesenchymal chondrosarcoma is a rare tumor that more frequently involves the pelvic bones, the femur, and the humerus. To our knowledge, only nine cases of primary mesenchy

[Bone, joint, spinal and pelvic-spinal localizations of sarcoidosis (apropos of a case)].

Authors report on one case of particularly flourishing and evolutive sarcoidosis that conditioned numerous manifestations among which stand out lesions interesting one hip and one sacro-iliac articulation and also extensive and destructive bone lesions. This case is for them an opportunity for recalling the major facts related with bone lesions and articular manifestations of sarcoidosis and for calling attention on the spinal and pelvic-spinal involvements. The latter, when signs of bone lysis exist, are evocative of a neoplasm. More often, spondylodiscitis, coexisting sometimes with spindle-

[Expert consensus on surgical management of pelvic metastatic tumors (2026 edition)].

The pelvis is one of the common sites of metastatic tumors. Due to the wide variety of primary tumor origins and differences in tumor malignancy and the degree of bone destruction, clinical decision-making for pelvic metastatic tumors is complex, and there is a lack of unified diagnostic and treatment guidelines both domestically and internationally. To standardize the surgical treatment of pelvic metastatic tumors in China, the Bone Tumor Group of the Orthopedics Branch of the Chinese Medical Association and the Professional Committee on Bone Tumors and Bone Metastasis of the China Anti-Cance

Microscopic Sertoliform Sex Cord Proliferation Involving Pelvic Peritoneum: Report of a Case Associated With Endosalpingiosis.

Microscopic sex cord proliferations are an uncommon finding, most often associated with ovarian or uterine stromal, epithelial, or mixed epithelial and stromal neoplasms. Rarely they occur in conjunction with a non-neoplastic process such as endometriosis or adenomyosis, and occasionally in the absence of concurrent pathology in locations such as the fallopian tube. Most reports of this phenomenon document adult granulosa cell tumor-like morphology but more uncommonly the proliferations exhibit Sertoliform features. We report a case of a multifocal sex cord proliferation (inhibin and calretini

Rhabdomyosarcoma of the Vagina in an Adolescent Girl.

BACKGROUND: Gynecologic neoplasms are rare in children and represent only less than 5% of all childhood tumors. Rhabdomyosarcoma (RMS) of the female genital tract of children accounts for only 3.5% of the cases. CASE: A 16-year-old adolescent presented with a proliferating growth and foul smelling discharge from her vagina, which, on biopsy was diagnosed as RMS. She received chemotherapy and radiation to the primary site. She is alive in remission at 8 years, and with normal menstrual function. SUMMARY AND CONCLUSION: RMS of the vagina is a rare, but highly curable tumor in adolescent girls.
