Perinatal mortality and prevalence of major congenital malformations of twins in Taipei city.

In order to examine perinatal mortality and prevalence of major congenital malformations in twins, deliveries in four teaching hospitals in Taipei City were studied. Among a total of 73,264 deliveries from October 1985 to June 1989, there were 844 pairs of twins. The zygosity of the twin pairs was determined by sex, placentation and 12 red blood cell antigens. There were 482 MZ and 252 DZ twin pairs identified, but the zygosity of a further 110 twin pairs was indeterminable due to lack of information on plancentation and/or blood types. A total of 4,573 singletons delivered in one study hospit…

[Rational tactics in the management of pregnancy and labor to prevent perinatal morbidity and mortality].

Results of a cooperative multicenter research have shown that extended studies into scientifically based and regionalized diagnostic screening programmes aimed at sequential follow-up of the gestational phases and early detection of complications is a means to reduce perinatal morbidity and mortality. These programmes should be based on the population characteristics of the fetal and neonatal growth as well as on the incidence and pattern of perinatal disease with consideration of the region-specific birth rates. An adequate range of diagnostic tests provides comprehensive information on intra

[Breech presentation of the second twin. Effect on early morbidity and perinatal mortality].

Twin delivery is often complicated by breech presentation of the second twin. To investigate the influence of breech presentation of the second twin, we reviewed all 259 twin deliveries from the 18,404 deliveries at our institution over 11 years between 1978 to 1989. The neonatal morbidity of the second twin in breech presentation (pH less than 7,15; 1-minute-Apgar less than 7, transfer to newborn hospital) was significantly higher in comparison to vertex presentation of the second twin. A dependence of neonatal morbidity on the time interval was not found. Where the second twin was in vertex

The extent and cause of perinatal lamb mortality in 3 flocks of Merino sheep.

Autopsies were performed on lamb carcases from 2 groups of Merino ewes lambing in paddocks in spring 1978 and from 1 group lambing in pens in 1980 in southwest Queensland. These showed that the main causes of death were starvation due to failure of the lamb to obtain milk, problems at parturition causing death during or shortly after birth and, in some cases, predation. The majority of lambs died within 1 week of birth with the highest mortalities occurring in the lighter lambs. In the pen study, mean birth weight of lambs that died was 3.0 (s.d. = +/- 0.5) kg. The mean daily weight loss of la

Prolapse of the cord: reduction of perinatal mortality by bladder instillation and cesarean section.

The fetal outcome in 135 cases of cord prolapse seen during the period 1970-79 was studied. The fetus was alive at diagnosis in 127 cases. In 88 (69.3%) of the cases, the cord prolapsed during the first stage of labor and they were managed by Vago's method of rapid instillation of saline into the bladder to relieve cord pressure during preparation for cesarean section (CS). In the remaining 39 cases, the cord prolapsed during the second stage of labor and the patients were delivered vaginally. The overall perinatal mortality rate was 8.1%. The perinatal mortality rate among cases with a live f…

(De)coding perinatal deaths: applying the International Classification of Diseases to perinatal mortality to a perinatal e-registry from 16 hospitals in Benin, Malawi, Tanzania, and Uganda.

BACKGROUND: To reduce perinatal deaths, identification of their causes is paramount. 'The WHO application of ICD-10 to deaths during the perinatal period' (ICD-PM) was developed to improve the quality of perinatal death data. OBJECTIVES: To determine stillbirth and very early neonatal death rates across 16 hospitals in Benin, Malawi, Tanzania, and Uganda, examine causes of death and associated maternal conditions applying the ICD-PM, and assess how a clinical perinatal e-registry performed when applying the ICD-PM. METHODS: We used cross-sectional data collected between 1st July 2021 and 29t

Prediction of perinatal mortality in early-onset fetal growth restriction: A post hoc analysis of the Dutch STRIDER trial to predict perinatal mortality in early-onset fetal growth restriction.

OBJECTIVE: Early-onset fetal growth restriction affects about 0.3% of pregnancies, posing high perinatal risks due to placental insufficiency. Early-onset fetal growth restriction often coincides with early-onset pre-eclampsia, associated with significant mortality and morbidity. Clinical management varies among clinicians, with emphasis on intensive monitoring and timely delivery. Our objective was to improve clinical prediction of perinatal mortality in early-onset fetal growth restriction for parental counseling. STUDY DESIGN: This was a secondary analysis of prospective cohort data from t…

Perinatal mortality in preterm births: an analysis of causes, presence of substandard care and avoiding mortality in three Dutch regions.

OBJECTIVE: To analyze the causes and underlying events in cases of perinatal mortality (PNM) in preterm children. SETTING: Three regions within the Netherlands. STUDY DESIGN: For this study, we combined data of a PNM audit over a 1-year (2003-2004) with the corresponding data of its source population (n=22,189). In the perinatal audit, all cases of perinatal death have been assessed by multi disciplinary teams of professionals in perinatal care in a consensus model for cause of death and the presence of substandard care factors (SSF). In this article, we restricted our analysis to children b…

Predictors of perinatal mortality in emerging regions of Ethiopia: Evidence from EDHS 2016.

BACKGROUND: Perinatal mortality rate is one of indictors used to measure the quality of obstetric and pediatric services globally. Compared to developed settings, perinatal mortality rate is higher in low-income countries, indicating societal inequities in health care and a scarcity of prenatal services. It is responsible for roughly 42% of all stillbirth in Sub-Saharan Africa, and 41% of newborn death globally. Despite Ethiopia's efforts to reduce perinatal mortality by improving the quality of care for maternal and child health, perinatal mortality rate is still very high, and as to our sear…

Causal effects of competing obstetrical interventions: mediators of placental abruption and perinatal mortality.

Placental abruption, the premature placental separation, confers increased perinatal mortality risk with preterm delivery as an important pathway through which the risk appears mediated. Although pregnancies complicated by abruption are often delivered through an obstetrical intervention, many deliver spontaneously. We examined the contributions of clinician-initiated (PTDIND) and spontaneous (PTDSPT) preterm delivery at < 37 weeks as competing causal mediators of the abruption-perinatal mortality association. Using the Consortium for Safe Labor (2002-2008) data (n = 203 990; 1.6% with abrupti…

The Psychiatrist's Role in Treating Perinatal Opioid Use Disorder and Reducing Maternal Mortality.

Drug overdose is a leading cause of maternal mortality. Psychiatrists can play a critical role in reducing these deaths by delivering effective evidence-based treatments for perinatal opioid use disorder (POUD), including the use of buprenorphine. Medications for POUD (i.e., buprenorphine, methadone) are life-saving treatments, but only half of those who are diagnosed as having POUD will receive this treatment, which can result in an increased risk for return to opioid use, overdose, and death. Psychiatrists are well positioned to prescribe buprenorphine given the Drug Enforcement Administrati

Weight discordance and perinatal mortality in twin pregnancy: systematic review and meta-analysis.

OBJECTIVES: The primary aim of this systematic review was to explore the strength of association between birth-weight (BW) discordance and perinatal mortality in twin pregnancy. The secondary aim was to ascertain the contribution of gestational age and growth restriction in predicting mortality in growth-discordant twins. METHODS: MEDLINE, EMBASE, CINAHL and ClinicalTrials.gov databases were searched. Only studies reporting on the risk of mortality in twin pregnancies affected compared with those not affected by BW discordance were included. The primary outcomes explored were incidence of int…

[High perinatal mortality associated with triple anthelmintic resistance in a German sheep flock].

High perinatal mortality, low milk yields and occasional ewe deaths were investigated in a Dorper sheep flock in Southern Germany. Parasitic gastroenteritis due to Trichostrongylus spp. associated with severe weight loss despite regular anthelmintic treatments of the flock was identified as the underlying cause. A faecal egg count reduction (FECR) test revealed zero reduction after treatment with ivermectin or albendazole, respectively, and a FECR of 57.9% following treatment with levamisole. These results indicate a lack of, or considerably reduced efficacy of substances from all three classi…
