NT, NPY and PGP 9.5 presence in myomeytrium and in fibroid pseudocapsule and their possible impact on muscular physiology.

The uterine myoma pseudocapsule is a neurovascular bundle surrounding fibroid, containing neuropeptides, probably involved in uterine scar h … these neuropeptides are present in the uterine muscle and can affect muscle contractility, uterine peristalsis and muscular healing. A myomectomy respecting the pseudocapsule neurofibers should facilitate smooth muscle scarring and promote restoration of normal uterine peristalsis with a possible positive influence on fertility.

Prenatal diagnosis of gastroesophageal reflux by color and pulsed Doppler ultrasonography in a case of congenital pyloric atresia.

A case of pyloric atresia diagnosed in the third trimester is described. Real-time ultrasonographic examination revealed polyhydramnios, enlarged stomach, evident gastric peristalsis and esophageal dilatation. Color and pulsed Doppler studies of the gastroesophageal junction revealed the presence of a biphasic flow pattern that was consistent with gastroesophageal reflux.

Three-Dimensional ultrasound parameters in assessing endometrial receptivity: a prospective study of novel scoring systems in frozen embryo transfer cycles.

Accurate prediction of endometrial receptivity remains a key challenge in assisted reproductive technology (ART), with implantation failure … e pathologies were excluded. Endometrial parameters including thickness, echogenicity, vascularity, peristalsis, uterine artery pulsatility index and 3D endometrial volume were evaluated on the day of FET using standardised ultrasonography. The primary outcome was clinical pregnancy, confirmed by visualisation of a gestational sac at six weeks. The study was …

Mixed attention ensemble for esophageal motility disorders classification.

Esophageal motility disorders result from dysfunction of the lower esophageal sphincter and abnormalities in esophageal peristalsis, often presenting symptoms such as dysphagia, chest pain, or heartburn. High-resolution esophageal manometry currently serves as the primary diagnostic method for these disorders, but it has some shortcomings including technical complexity, high demands on diagnosticians, and time-consuming diagnostic process. Therefore, based on ensemble learning with a mixed voting mechanism and multi-dimensional attention enhancement mechanism, a classification model for esopha…

CFD investigation of multiple peristaltic waves in a 3D unobstructed ureter.

Ureters are essential components of the urinary system and play a crucial role in the transportation of urine from the kidneys to the bladde … um velocity is observed in the single and double wave motion due to the contraction produced by the peristalsis motion. The pressure gradient is maximum at the inlet of the ureter during the single bolus motion. The contraction produces a high jet of velocity due to neck formation and also helps in urine trapping in the form of a bolus, which leads to the for …

An analytical and comparative study of swallowing in a tumor-infected oesophagus: a mathematical model.

This study discusses non-steady effects encountered in peristaltic flows in oesophagus. The purpose of this communication is to evolve a mechanism to diagnose tumor in an oesophagus mathematically. The tumor is modelled by generic bump function of certain height and width. The method of solution follows long wavelength and low-Reynolds number approximations for unsteady flow, while integrations have been performed numerically in order to plot graphs, which reveal various characteristics of the flow. The goal is to assess how pressure varies across the tumor's width. The spatial, as well as tem

Esophageal high-resolution manometry demands visual interpretation in addition to mathematical software-based analysis.

PURPOSE: Esophageal high-resolution manometry (HRM) revolutionized esophageal function testing due to the intuitive colorful and agreeable-t … tion may be useful in cases of hypomotility, premature waves, artifacts, segmental abnormalities of peristalsis, and extra-luminal non-contractile findings. CONCLUSION: These extra findings can be reported apart from the conventional parameters.

[Achalasia Update].

Achalasia Update Abstract. The neurodegenerative disease achalasia (obsolete: "cardiac spasm") is the second most common functional disease of the esophagus after reflux disease. It is associated with an extremely high level of suffering for the patient. Pathophysiologically, it is a combination of a lack of swallowing-reflex relaxation at the gastric entrance and disturbed peristalsis of the tubular esophagus. The gold standard in diagnostics is high-resolution manometry. The disease cannot be cured, the therapeutic spectrum that alleviates the disease includes pharmaceutical, endoscopic-inte…

Hard to Swallow Results.

The American Neurogastroenterology and Motility Society (ANMS) proposed quality measures (QMs) for performance and interpretation of esophag … (n = 39) having at least one missing item based on the CC. The most missed diagnosis was fragmented peristalsis (30%, n = 29). Among the 39% (n = 21) of surgery referrals, 24% (n = 5) were incorrectly referred. Our study shows poor data interpretation by community gastroenterologists without formal motility training despite adequate performance by nursing sta …

Esophageal motor dysfunction in an adult with a congenital tracheoesophageal fistula.

A preoperative barium esophagogram and esophageal manometry in an adult patient with an H-type congenital tracheoesophageal fistula revealed the typical esophageal motor abnormalities previously described following operative repair of esophageal atresia in infants. The upper esophageal sphincter contracted normally, the lower esophageal sphincter relaxation was weak and incomplete, and there was no active peristaltic wave within the esophagus. These findings support the theory that the abnormal esophageal motor function reported after repair of congenital tracheoesophageal fistula and esophage

Quantitation of pharyngeal motor function in normal human subjects.

A need exists for accurate pressure recording of pharyngeal motor events. Results of this study indicate that accurate quantitation of pharyngeal motor activity is not possible using a water-filled catheter system, even when high infusion rates are used. An intraluminal strain gauge system, however, achieves high-fidelity recording. Quantitation of pharyngeal peristalsis using the intraluminal strain gauge system reveals peristaltic pressure amplitudes higher than those hitherto recorded. In normal subjects, peristaltic amplitude averages about 200 mmHg in the hypopharynx, complexes in one sub…

Transient versus surgically managed small bowel intussusception in children: Role of ultrasound.

BACKGROUND: To evaluate and compare the ultrasound (US) features of transient small bowel intussusception (SBI) with those which required su … susceptions were associated with the lead point while none of the transient SBI had any lead point. Peristalsis was absent in all surgically managed intussusceptions. CONCLUSION: Transient SBI is associated with a shorter length of intussusception, smaller transverse diameter, thin walls, absence of the lead point and visible peristalsis. All these findings …

[Is complex therapy of achalasia using botulinum toxin combined with balloon dilatation an effective approach?].

BACKGROUND: Achalasia (ACHL) is not a very frequent disease and its etiology is still unclear. In addition to the oldest therapeutical approach represented by myotomy, two conservative methods are commonly used--balloon dilatation and application of botulotoxin. So far, both methods have been used only separately, and their effects have been compared. Literature provides no evidence of the renewal of oesophageal propulsive peristalsis in result of conservative treatment. OBJECTIVES: The aim is to use both approaches subsequently within a short period of time, in order to potentiate their effe…

Esophageal motor function in primary Sjögren's syndrome: correlation with dysphagia and xerostomia.

The incidence of dysphagia in patients with primary Sjogren's syndrome (pSS) has been underestimated and all too often ascribed to xerostomi … le with that of controls, with no difference between patients with and without dysphagia. Defective peristalsis, ie, the presence of simultaneous contractions in more than 30% of wet swallows was detected, however, in the distal tract of the esophagus of six patients (22.2%) and in the proximal tract of three (11.1%). All these patients had severe dysphagia a …

A case of perazine-induced enteritis--allergy or pseudoallergy?

Almost all adverse reactions to neuroleptic drugs in the gastro-intestinal system can be explained by their peripheral cholinergic blocking action with subsequent inhibition of intestinal tone and peristalsis. As a rare condition, severe diarrhoea due to enteritis may occur. A case with this complication is presented, the etiology and pathogenetic factors are discussed.
