Granulomatous peritonitis due to Ascaris lumbricoides.

A child with granulomatous peritonitis caused by ascariasis is described. Laparotomy revealed no obvious evidence of intestinal perforation. It is postulated that a worm escaped through the appendicular tip, causing transient peritonitis followed by adhesions and granulomatas in the peritoneal cavity.

Clostridium difficile colitis mimicking acute peritonitis.

Five patients receiving penicillin V potassium or a cephalosporin antibiotic for 18 hours to 22 days developed fever, marked leukocytosis, and signs and symptoms that suggested right-lower-quadrant peritoneal irritation. All underwent emergency laparotomy, at which dilatation and inflammation of the ascending colon were found. Only one of the patients had profuse diarrhea, and two patients had no diarrhea prior to laparotomy. Postoperatively, Clostridium difficile colitis was diagnosed by stool toxin assay and was confirmed in one case by proctosigmoidoscopic biopsy results. Antibiotic-associa

Group A streptococcal peritonitis in infancy.

Primary peritonitis in infancy is a rare condition. It is usually associated with a severe pathological process, such as nephrotic syndrome. When it affects previously healthy infants it causes diagnostic and therapeutic difficulties. Pneumococci, streptococci and Haemophilus influenzae are possible causative agents, other causes are rare. The clinical course of two infants with group A streptococcal peritonitis is described.

Granulomatous peritonitis caused by glove starch.

Corn starch particles are used as a surgical glove lubricant. At present there is no better alternative for this lubricant. Implantation of corn starch particles into the peritoneal cavity can induce foreign body reactions, starch peritonitis and starch granulomata, and may cause adhesions and intestinal obstruction. Starch peritonitis should be treated conservatively.

[Ulcero-necrotic enterocolitis complicated by peritonitis].

Among 217 patients with acute peritonitis there were 12 patients with the peritonitis developed against the background of segmentary enteritis or colitis. The article describes this form of peritonitis characterized by great severity, rapid development and high death rate. On the detection of a necrotic segment of the small intestine, a one-step resection within the limits of healthy tissues is indicated. Two-steps operations are expedient for necrosis of the colon.

[Postoperative peritonitis. Patients, causes, therapy, prognosis].

1. The most frequent causes of postoperative peritonitis are circumscribed abscesses, the postoperative ileus and the anastomotic leakage or breakdown of the digestive suture. 2. The mortality is 33%; procedures in upper abdominal surgery are accompanied by higher mortality than in colorectal surgery in cases of postoperative peritonitis. 3. The most frequent cause of death after reoperation is the generalized sepsis. 4. A mixed flora of spores are found in most cases; obligate anaerobic spores are found mostly after colorectal procedures, while typical "hospitalized spores" were present equal…

Hydrocolpos with peritonitis in the newborn.

Two cases of hydrocolpos are described presenting soon after birth. One infant who died on the first day had an intrauterine peritonitis caused by compression of the caecum on the pelvic brim. The other surviving infant developed a caecal perforation secondary to Hirschsprung's disease. Neither infant had rectal atresia, which is the commonest cause of intestinal obstruction in these infants. The association of hydrocolpos with polydactyly in one of these cases may represent an autosomal recessive trait.

[Peritonitis by ruptured pyonephrosis (author's transl)].

The authors have observed one case of peritonitis by ruptured lithiasis pyonephrosis. The elective treatment of this unusual (13th case in the literature) complication of the urinary lithiasis is the nephrectomy.

[Postoperative peritonitis. A contribution on diagnosis].

Symptoms and signs of 18 patients with postoperative bacterial peritonitis were compared with the findings in uncomplicated postoperative cases. Their diagnostic values were examined: 1. Elevated temperature, hyperventilation, and somnolence are relevant, being indicative of bacterial peritonitis and already occurring before the typical findings on abdominal examination. 2. In those patients with peritonitis, hemodynamics, and metabolism are characterized by hyperdynamic circulation, premature arterial hypotension with dry warm skin, and lactate accumulation. 3. Laboratory data often reveal th…

Balantidium peritonitis diagnosed on cytologic material.

Balantidium coli were detached in a routine cytologic smear of ascitic fluid. The morphologic features were clearly discernible in the Papanicolaou stained smears.

Concomitant pneumococcal appendicitis, peritonitis, and meningitis.

A 9-year-old boy developed pneumococcal meningitis and peritonitis following appendectomy. Subsequent pathologic examination showed Gram-positive diplococci in the appendix. Cultures of the peritoneal fluid, blood, and spinal fluid showed Diplococcus pneumoniae. The experience illustrates the danger of assuming that all pneumococcus peritonitis is the primary variety and the advisability of routine Gram stain of the peritoneal fluid at operation in order to select the appropriate antibiotic.

Primary peritonitis in infancy and childhood.

Primary peritonitis, rarely diagnosed preoperatively, is an uncommon disease accounting for 2.1% of all pediatric abdominal emergencies. It is often associated with urinary or hepatic pathology, the former the source of the infecting organism in the majority of cases, and presents with characteristic symptoms depending upon whether it occurs in infancy or childhood. The symptoms and signs which allow for a positive prospective diagnosis are illustrated by comparing this disease to those entities with which it is most often confused, e.g. diffuse peritonitis of other etiologies, and include a s…
