Sensory stimulation for upper limb amputations modulates adaptability of cortical large-scale systems and combination of somatosensory and visual inputs.

Touch-like phantom limb sensations can be elicited through targeted transcutaneous electrical nerve stimulation (tTENS) in individuals with upper limb amputation. The corresponding impact of sensory stimulation on cortical activity remains an open question. Brain network research shows that sensorimotor cortical activity is supported by dynamic changes in functional connections between relevant brain regions. These groups of interconnected regions are functional modules whose architecture enables specialized function and related neural processing supporting individual task needs. Using electro…

Chronic pain in lower limb amputees: Is there a correlation with the use of perioperative epidural or perineural analgesia?

BACKGROUND: Chronic pain is common in patients who undergo lower limb amputation. The use of epidural or perineural catheters seems to reduce acute pain after surgery but their effects in a longer follow up are unknown. OBJECTIVE: To evaluate the long-term prevalence of phantom limb sensation (PLS), phantom limb pain (PLP), and residual limb pain (RLP) and their correlation with perioperative use of epidural or perineural catheters. METHODS: Postal survey. Patients with trans-femoral, trans-tibial or hemi-pelvectomy amputation were asked to partake in the study. The Prosthetic Evaluation Que…

Phantom pain as an adverse effect after maggot (Lucilia sericata) debridement therapy: a case study.

Maggot debridement therapy, also known as larval therapy, is a well known method of treatment for hard-to-heal and intractable wounds. This case study describes severe phantom pain as a rare adverse event of maggot therapy in patients after post-traumatic amputation of the leg. We also hypothesise and discuss the possibility that the phantom pain may be a result of maggot activity, not only through tissue debridement but also through nerve nourishment and nerve regeneration.

Transcutaneous electrical nerve stimulation after major amputation.

We studied the effect of transcutaneous electrical nerve stimulation (TENS) on stump healing and postoperative and late phantom pain after major amputations of the lower limb. A total of 51 patients were randomised to one of three postoperative treatment regimens: sham TENS and chlorpromazine medication, sham TENS only, and active low frequency TENS. There were fewer re-amputations and more rapid stump healing among below-knee amputees who had received active TENS. Sham TENS had a considerable placebo effect on pain. There were, however, no significant differences in the analgesic requirements

The appearance of new phantom fingers post-amputation in a phocomelus.

We report the unusual case of a woman with right upper limb phocomelia who, post-amputation of her right hand following trauma, sprouted a phantom hand that contained five digits, including a phantom thumb and index finger that had been absent since her birth. These two phantom digits were initially half normal size, however, more than three decades later, with mirror visual feedback treatment, she was able to elongate them to normal length. This suggests that a hardwired representation of a complete hand had always been present in her brain, but inhibited by the presence of afferents from the

Extra-neural signals from severed nerves enable intrinsic hand movements in transhumeral amputations.

Robotic prostheses controlled by myoelectric signals can restore limited but important hand function in individuals with upper limb amputation. The lack of individual finger control highlights the yet insurmountable gap to fully replacing a biological hand. Implanted electrodes around severed nerves have been used to elicit sensations perceived as arising from the missing limb, but using such extra-neural electrodes to record motor signals that allow for the decoding of phantom movements has remained elusive. Here, we showed the feasibility of using signals from non-penetrating neural electrod

The Role of Preemptive Perioperative Analgesia in Prevention of Chronic Phantom Pain: A Systematic Review and Meta-analysis.

BACKGROUND: Phantom limb pain (PLP) is a debilitating condition leading to the experience of pain in a limb that has been amputated. Pharmacological interventions have been proposed to prevent chronic PLP. However, results of these interventions are still controversial. This systematic review and meta-analysis clarifies the effectiveness of preemptive pharmacological interventions in prevention of chronic phantom pain by evaluating incidence and intensity of PLP, residual limb pain (RLP), quality of life (QoL), depression, and anxiety. METHODS: We systematically searched the PubMed, Embase, S…

Targeted Muscle Reinnervation Improves Pain and Ambulation Outcomes in Highly Comorbid Amputees.

BACKGROUND: Approximately 200,000 people undergo a lower extremity amputation each year. Following amputation, patients suffer from chronic pain, inability to ambulate, and high mortality rates. Targeted muscle reinnervation is a nerve transfer procedure that redirects transected sensory and mixed nerves into motor nerves to treat neuroma and phantom limb pain. This study evaluates outcomes with prophylactic targeted muscle reinnervation at the time of below-knee amputation. METHODS: This is a cohort study comparing 100 patients undergoing below-knee amputation with primary targeted muscle re…

Epidemiology and treatment of neuropathic pain: the UK primary care perspective.

The true incidence of neuropathic pain is unknown, but it is believed to be under-diagnosed and treated inadequately, despite the availability of drugs with proven efficacy. Our objective was to report the epidemiology and drug treatment of neuropathic pain as managed by UK primary care physicians. A descriptive analysis of the epidemiology of incident post-herpetic neuralgia (n=12,386); trigeminal neuralgia (8268); phantom limb pain (451) and painful diabetic neuropathy (4719) and prescription treatment at diagnosis from computerised UK general practice records (GPRD): January 1992 to April 2…

Outcomes of Ertl Osteomyoplasty Transtibial Amputation With Primary Targeted Muscle Reinnervation and Regenerative Peripheral Nerve Interface.

BACKGROUND: Traditional transtibial amputation (TTA) often results in inadequate residual limb stability and increased risks of phantom limb pain and residual limb pain. The Ertl osteomyoplastic TTA technique with primary targeted muscle reinnervation and regenerative peripheral nerve interface may offer solutions to improve structural stability and reduce peripheral nerve pain. We hypothesize that this advanced TTA technique may reduce phantom limb pain and residual limb pain, while improving residual limb stability, contributing to a higher quality of life compared with traditional methods. …
