The pathogenesis of venous thrombosis is far less clear than that of arterial thrombosis. In spite of physical and medical prophylaxis, there is still a risk of thrombosis of approx. 5% left after elective abdominal surgery. Ascending phlebography cannot be dispensed with in case of an active therapeutic procedure such as fibrinolysis or venous thromectomy. In order to settle the question of thrombolysis or thrombectomy, we have to consider (1) the extent of the thrombosis, (2) the time of existence of the thrombosis, and (3) possible contra-indication of either method. The doppler ultrasound probe was used to study 138 patients after major surgery to assess the incidence of iliofemoral thrombosis in the early postoperative period. The patients were simultaneously studied with the 125-I-fibrinogen uptake test. In 207 legs with venous leg ulcers and postthrombotic syndrome proved by phlebography, the frequency of involvement of the 4 levels of deep venous system was assessed. Level A: deep veins of the lower leg. Level B: poplieal vein and gastrocranial veins. Level C: femoral veins. Level D: iliac veins. Thirty-eight patients were followed up 13 to 17 years after sustaining a fracture of the tibia. The aim was to establish the frequency of post-thrombotic venous insufficiency. Between 1979 and 1982, 47 patients with pathological process of the adrenals were surgically treated after preceding thorough non-evasive angiographic diagnosis (6 phaeochromocytomas, 20 adrenal adenomas, 8 adrenal hyperplasias, 5 adrenal carcinomas, 5 adrenal cysts and 3 adrenal metastases). Deep vein thrombosis of the upper extremity is rare. While its pathogenesis is not always evident, it often occurs in connection with tumors, thoracic outlet syndrome or intravenous catheter therapy.
