[Anxiety disorders in the elderly--diagnostic problems and therapeutic prospects].

Anxiety disorders in elderly patients have up to now represented a neglected gerontopsychiatric field of medical research, although they represent epidemiologic quite significant disorders also on this age. Diagnosis of an anxiety disorder in the elder age group may be more difficult than in other age groups, as anxiety symptoms may manifest more on the somatic base or they are unclear as in the phobic disorder. An integrative disorder model, in which the etiologic relevant biologic and psychologic causes are united, seems to be more effective under theoretic-heuristic as well as therapeutic a

A case of phobic anxiety related to the inability to smell cyanide.

This report describes a case of phobic anxiety relating to cyanide in a process operator who is unable to smell hydrogen cyanide. This case demonstrates that hazardous substances in the workplace can provoke this mental disorder in individuals who are unable to detect by special senses whether or not a specific hazard is present. The clinical management of such individuals is complicated since they must be able to perceive the feared object or substance in order to overcome their anxiety.

Genetics of Anxiety Disorders.

PURPOSE OF REVIEW: Anxiety disorders are among the most common mental disorders with a lifetime prevalence of over 20%. Clinically, anxiety is not thought of as a homogenous disorder, but is subclassified in generalized, panic, and phobic anxiety disorder. Anxiety disorders are moderately heritable. This review will explore recent genetic and epigenetic approaches to anxiety disorders explaining differential susceptibility risk. RECENT FINDINGS: A substantial portion of the variance in susceptibility risk can be explained by differential inherited and acquired genetic and epigenetic risk. Ava

Application of structural equation mixture modeling to characterize the latent structure of eating pathology.

OBJECTIVE: Several theoretical models describe the structure of eating disorders (EDs), and a burgeoning empirical literature has sought to identify whether eating pathology is conceptualized best as categorical (presence or absence of disorder), dimensional (continuous), or a hybrid of categories and dimensions. METHODS: This study used structural equation mixture modeling (SEMM) to identify the latent structure of EDs. Items from the Eating Pathology Symptoms Inventory (EPSI) were administered to individuals with EDs (N = 344). Select EPSI scales and body mass index were indicators in subse

The Fear of Facing Drives and Desires: Is It Still Appropriate to Reduce Anorexia and Bulimia to Eating Disorders?

This paper reflects a perspective the author has developed over twenty years of practice as a clinical psychoanalyst and as an academic researcher in the field of eating disorders. Anorexia and bulimia are discussed through the lens of psychoanalytic theory, with the support of empirical evidence in clinical psychology research. The author proposes a new way to consider anorexic and bulimic pathology, assigning primary importance to drive repression and object avoidance and stimulating reflections on the phobic-obsessive displacement onto hunger and food. A brief illustrative vignette is inclu

Keeping your eye on the process: body image, older women, and countertransference.

Research on body image and older women has grown in the past decade. However, there is a gap in the literature regarding body image, older women, and countertransference. This article provides 7 case examples of racially and ethnically diverse women over 60, drawn from MSW student and agency staff supervision, and participant feedback from a national conference on aging workshop. Themes related to loss and grief, adult daughter and aging mother issues, incest, anger, disability, personality disorders, phobic reactions, and shame are discussed. Recommendations and implications for social work p

The adult children of alcoholics in a psychiatric population.

The issues confronting adult children of alcoholics (ACOA) are well publicized but their empirical basis remains limited. The screening of 250 consecutive psychiatric admissions to a general hospital revealed a significant prevalence of ACOAs across diagnoses. Compared to the other patients, the ACOA group was younger but with no other socioeconomic difference. The diagnoses of substance abuse and phobic disorders were more frequent. The group was psychiatrically hospitalized at an earlier age. The impact of an ACOA status varied with the parental biological relation and gender, and with the p…

Alprazolam treatment for panic disorders.

Office records of 27 private patients who were treated with alprazolam for panic disorders were evaluated by a second psychiatrist. Alprazolam treatment led to a complete remission of panic attacks in 85% of the patients; panics ceased within an average of 6 days, at a mean dose of 2.2 mg/day. In addition, alprazolam was found to bring about a complete absence of phobic avoidance behavior in 21 of the 23 agoraphobic patients in the group. This is the first study to demonstrate the efficacy of medication alone as a treatment for agoraphobia.

Diazepam and propranolol in panic disorder and agoraphobia.

The response to diazepam and propranolol hydrochloride was compared in 21 patients who (with one exception) met DSM-III criteria for panic disorder and agoraphobia. Each drug was administered for two weeks in double-blind fashion according to a crossover design. The response to diazepam was significantly superior on all measures. By observer rating, 18 patients showed at least moderate improvement with diazepam compared with seven receiving propranolol. Panic attacks and phobic symptoms responded to diazepam, but not to propranolol. The results suggest that benzodiazepines constitute effective

Differential diagnosis and treatment of panic attacks and phobic states.

Recent advances in our understanding of and ability to treat panic disorders and various types of phobia have been reviewed. A case of agoraphobia was presented in detail to illustrate clinical issues and serve as a basis for discussing differential diagnosis. The presence or absence of spontaneous panic attacks has crucial implications for classifying phobic states. Data and speculations concerning the pathophysiology of agoraphobia, social phobia and simple phobia were also presented. Finally, a variety of pharmacological and behavioral treatment approaches, some solidly established, other s

Application of electroconvulsive therapy in treatment of retinitis pigmentosa comorbid with major depressive disorder and panic disorder.

A substantial proportion of retinitis pigmentosa (RP) patients have depression/anxiety and a phobic pathology that may be related to changes in melatonin secretion. We discuss electroconvulsive therapy (ECT) in a patient with RP comorbid with depression and panic disorder. A 51-year-old man was admitted because of major depression, panic disorder, and RP. Ultrabrief pulse (0.3 millisecond) right unilateral ECT was performed 9 times in total. The symptoms relieved, and patient tolerated the treatments well. Electroconvulsive therapy increases serum melatonin, providing therapeutic effects in de

[Nocturnal distress crisis in agoraphobics].

The aim of this study was to measure the incidence of nocturnal panic attacks in a sample of 60 referrals who met DSM-III-R criteria for panic disorders. We also examine the relationship of nocturnal panic and clinical variables and family history. Probands with or without nocturnal panic were compared in regard to the existence of anxiety, affective, personality and alcohol-dependence disorders using the SCID-P and SCID-II interviews for DSM-III-R criteria and the FISC for the family study. Severity was assessed by means of age of onset, percentage of remissions, subtype of agoraphobia and Gl

Psychopathology and Urine Toxicology in Methadone Patients.

Several studies reported high rates of psychiatric commorbidity among methadone patients. We examined the relationships of measures of psychopathology to outcomes of screening urine tests for cocaine, opiates, and benzodiazepines in a sample of 56 methadone patients. They also completed the Symptom Check List-90-Revised (SCL-90-R). The highest scales in the SCL-90-R profile of our patients were those indicating somatic discomfort, anger, phobic anxiety, paranoid ideation, and also obsessive-compulsive disorder symptoms (scores above the 39(th) percentile). The only significant correlations bet

Prevalence, onset, and clinical recognition of panic states in hospitalized male alcoholics.

Alcoholism has been associated with a high prevalence of anxiety and phobic disorders. The authors ascertained the current prevalence of panic disorder and the lifetime prevalence of infrequent panic attacks in 154 male alcoholics in an inpatient alcohol treatment program. Thirteen percent (N = 20) gave a lifetime history of panic attacks and 45% (N = 9) of these had current panic disorder. Panic attacks preceded or coincided with the onset of problem drinking in 50% (N = 9) of the 18 patients with both diagnoses. Only two patients with histories of panic had been previously diagnosed, and non…

Symptoms of offensive type Taijin-Kyofusho among Australian social phobics.

This study investigated the culture specificity of Taijin-Kyofusho (TK) offensive type by examining whether symptoms of the disorder covary with social phobia and determining the proportion of those who meet criteria for a diagnosis of TK offensive type among Australian socially phobic individuals. The study included a total of 94 participants who met the DSM-IV criteria for social phobia and 39 normal controls who did not meet criteria for any mental disorder. All participants were born in Western countries and resided in Australia. Results showed that levels of offensive worry were significa

Short-term full kindling of the amygdala dissociates natural and periaqueductal gray-evoked flight behaviors of the rat.

Panic attacks present a high comorbidity with agoraphobia, separation anxiety and generalized anxiety disorder. Nevertheless, while panic attacks have been frequently equated to fear, the relationship of panic disorder with specific phobias remains uncertain. The combination of experimental models of panic and phobias could but afford valuable information about both the comorbidity and causation of these disorders. As it regards, while the defensive behaviors produced by stimulation of dorsal periaqueductal gray matter (DPAG) resemble a panic attack, resistance to capture (RC) behaviors that e

Familial aggregation of anxiety-related quantitative traits in generalized social phobia: clues to understanding "disorder" heritability?

Social phobia, particularly the generalized form, is strongly familial. Given the probable continuity from extremes of normative personality (e.g., shyness) to social phobia to personality disorder (e.g., avoidant personality disorder), it is unlikely that social phobia itself, at the level of an Axis I disorder, is transmitted. Rather, it seems more parsimonious, and in keeping with current notions about the structure and heritability of mental disorders, that one or more temperamental risk factors for social phobia is transmitted. The goal of this study was to explore this possibility by exa

Afobazole was administered to 32 patients, aged 18-60 years, with cardio-vascular diseases and neurotic stress-related psychopathologies and somatoform disorders (ICD-10 F40-F45). The drug was assigned in dosages from 30 to 60 mg daily (mean 50 mg) during 6 weeks, along with basic somatotropic therapy. Twenty-nine patients completed the course. Therapeutic efficacy was assessed by the Hamilton anxiety scale (HAM-A) and a subscale of the Global Clinical Impression scale (CGI). The total number of responders was 21 (70%). The improvement began from the 1st week of the treatment and increased to

The impact of sexual and physical abuse on eating disordered and psychiatric symptoms: a comparison of eating disordered and psychiatric inpatients.

The authors compared rates of physical and sexual abuse in women with eating disorders (N = 102) and general psychiatric disorders (N = 49). Relationships between sexual abuse and severity of eating disordered and psychiatric symptoms were also examined. While high rates of sexual abuse were found in the eating disordered sample, these rates were not significantly higher than those found in the general psychiatric population. No relationship between a history of sexual abuse and severity of eating disordered symptoms was found. However, within the eating disordered group, sexually abuse subjec
