A 13-year-old boy with hyperimmunoglobulin E (hyper-IgE) syndrome presented with headache, blurred vision, photophobia and bilateral papilledema due to cryptococcal meningitis. Treatment with amphotericin B, and S-fluorocytosine for several weeks and repeated lumbar punctures did not reduce the intracranial pressure, and a myeloperitoneal shunt was performed. The child was maintained on fluconazole for an additional six months. Patients with hyper-IgE syndrome are at increased risk of opportunistic fungal infections such as cryptococcal meningitis.

The efficacy of topical sodium cromoglycate and medrysone was evaluated in 38 patients suffering from atopic keratoconjunctivitis. It was found that medrysone (1%) significantly improved the symptoms of itching, watering, photophobia and hyperaemia, while sodium cromoglycate (2%) was found to be ineffective. Tear prostaglandin E2 and histamine levels did not decrease significantly after sodium cromoglycate therapy. The findings of the present study reveal that sodium cromoglycate has no significant role in the management of atopic keratoconjunctivitis.

A male patient with tuberculous lymphadenopathy was treated with a four-fold therapy of ethambutol, isoniacide, rifampicin and pyracinamide. After 10 weeks the patient suffered from photophobia. Although ethambutol was discontinued vision decreased and visual field defects occurred as well as signs of myelopathy. Isoniacide was then discontinued and in the subsequent phase the vision was slowly restored over a period of 36 months. The combined toxicity of ethambutol and isoniacide seems to have been the main cause of the severe and protracted optic neuropathy.

A case of acute disseminated encephalomyelitis in 13-year-old boy associated with enterovirus is described. The patient had symptoms of severe headache and photophobia for 2 days. Diagnosis was made on the basis of diffuse high intensity white matter lesions in the left frontoparietal region seen on magnetic resonance imaging, and positive enterovirus polymerase chain reaction in cerebrospinal fluid. His symptoms improved substantially without specific therapy, and he recovered without neurologic sequelae.

PURPOSE: To present a case of increased systemic hypertension and pupil dilation related to low dose atropine eyedrops.
OBSERVATIONS: A thirteen-year-old male with progressive myopia received atropine 0.05% ophthalmic drops to slow down myopia progression. He exhibited systemic systolic hypertension, photophobia, and bilateral nonreactive mydriasis. The atropine drops were discontinued, and his blood pressure and pupillary function normalized.
CONCLUSIONS AND IMPORTANCE: This case demonstrates sensitivity to low dose atropine with increased systemic blood pressure and pupillary dilation.

Severe chemical burns can damage the periocular area causing exposure keratopathy and resulting in ocular pain, foreign body sensation, epiphora, photophobia, and vision loss. A custom fit large diameter therapeutic scleral lens can protect the ocular surface by preventing desiccation and providing immediate improvement in comfort. This case describes the management of a patient with extensive chemical burn with complete lid loss and exposure keratopathy who has maintained excellent visual acuity and comfort with therapeutic scleral lens use.

A 61-year-old woman presented with photophobia, pain, epiphora, and decreased vision as a result of two iatrogenic corneal perforations sustained during carbon dioxide laser blepharoplasty. The management and course of the case are outlined. Laser blepharoplasty complications are reviewed. This case highlights the potential occurrence of corneal penetration with laser blepharoplasty. Meticulous awareness of the laser effects on periocular tissue must be kept in mind at all times during laser blepharoplasty to avoid unnecessary intraocular side effects and undesirable postoperative outcomes.

VM is a common yet debilitating migraine variant that has taken many monikers in the past. As a relatively new diagnostic entity, public and provider awareness of this disorder can be improved. Symptoms include vertigo episodes in addition to photophobia, phonophobia, nausea, and headache. Diagnosis is primarily based on clinical history as pathognomonic signs via testing are not reliable. Standardized treatment algorithms have yet to be created and current recommendations have been adopted from migraine guidelines.

A Japanese infant had bilateral hypopigmented macules in a whorled or marble-cakelike configuration on her trunk and extremities. Her irides were bluish gray and her hair was dark brown. She had photophobia and macrocephaly with developmental delay. Analysis of peripheral lymphocytes and dermal fibroblasts disclosed a normal chromosomal pattern. A new finding was that the eumelanin content of the hypopigmented epidermis was decreased. We suggest that at least a portion of patients with this disorder have a defect in eumelanogenesis in hypopigmented skin.
