Artificial intelligence and target trial emulation: toward scalable and credible real-world evidence.

BACKGROUND AND OBJECTIVES: Randomized controlled trials remain the gold standard for causal inference but are often resource-intensive, time-consuming, and operationally complex to conduct. The growing availability of real-world data has created new opportunities for generating real-world evidence (RWE), and target trial emulation (TTE) provides a principled framework for designing observational studies that approximate a target trial. At the same time, artificial intelligence (AI) is increasingly applied in health research, yet its application within TTE has received limited systematic attent

Energy metabolism, adenosine, and glutamate signaling reprogramming by decanoic acid in Glut1 disorder syndrome.

Glut1 deficiency syndrome (Glut1DS) leads to neurological and cognitive symptoms and is primarily treated using carbohydrate-restricted ketogenic diets. However, a recent clinical trial of a less restrictive, non-ketogenic, medium chain triglyceride (MCT) diet with a high decanoic acid content suggests efficacy in Glut1DS treatment. Here, we employ human Glut1DS-derived iPSCs to investigate a role for these medium chain fatty acids in the regulation of gene expression as a proxy for metabolic reprogramming. We show that the new high decanoic blend reproduces many therapeutic changes in energy

Late-onset asystole associated with vagal nerve stimulation.

Vagal nerve stimulation (VNS) is a form of neuromodulation that is an established adjunct in the management of drug-resistant epilepsy. The common side effects of VNS are cough, dysphonia, or shortness of breath, which are often mild and do not require discontinuation of therapy. However, serious adverse effects such as bradyarrhythmia and asystole can occur rarely in patients who have had VNS implanted several years ago due to certain technical issues, patient-related risk factors, and unknown causes that necessitate the deactivation of VNS. We describe the late onset of bradyarrhythmia in a

The significance of multimodality approach in the management of non-lesional drug-resistant focal parietal lobe epilepsies.

Due to extensive connectivity of the parietal lobe, non-lesional drug-resistant (DRE) parietal lobe epilepsies (PLEs) are difficult to localize and often imitate other epilepsies. Therefore, patients with PLEs have low rates of seizure freedom following epilepsy surgery. Previous studies have highlighted the need to combine EEG and semiology for more accurate localization of PLEs. As sophisticated tools for localization become more available, the use of multiple different neuroimaging and neurophysiologic diagnostic tests may more readily identify PLE. We hereby report a unique case of a compl

Clinical dimensions of people with co-occurring obsessive-compulsive and related disorders and multiple sclerosis: a scoping review protocol.

INTRODUCTION: Multiple sclerosis (MS) is an immune-mediated demyelinating disease with a significant burden of neuropsychiatric sequelae. These symptoms, including depression and anxiety, are predictors of morbidity and mortality in people with MS. Despite a high prevalence of obsessive-compulsive disorder in MS, potentially shared pathophysiological mechanisms and overlap in possible treatments, no review has specifically examined the clinical dimensions of people with obsessive-compulsive and related disorders (OCRD) and MS. In this scoping review, we aim to map the available knowledge on th

Leveraging multiple approaches for the detection of pathogenic deep intronic variants in developmental and epileptic encephalopathies: A case report.

About 50% of individuals with developmental and epileptic encephalopathies (DEEs) are unsolved following genetic testing. Deep intronic variants, defined as >100 bp from exon-intron junctions, contribute to disease by affecting the splicing of mRNAs in clinically relevant genes. Identifying deep intronic pathogenic variants is challenging and resource intensive, and interpretation is difficult due to limited functional annotations. We aimed to identify deep intronic variants in individuals suspected to have unsolved single gene DEEs. In a research cohort of unsolved cases of DEEs, we searched

"You're Someone Different Now": An Autoethnography on Identity and Occupational Identity Disruption After Traumatic Brain Injury.

IMPORTANCE: Identity disruption and occupational identity disruption are common after traumatic brain injury (TBI), but the relationship between these two phenomena is underexplored. Occupational therapy practitioners should be knowledgeable about identity challenges after TBI and ways to reconstruct a sense of self. OBJECTIVE: To sensitize readers to the experience of identity disruption and occupational identity disruption and describe how those two experiences are interrelated and transactional in nature. DESIGN: We completed an autoethnography, because this method privileges the insider

Clinical efficacy of low-dose Perampanel correlates with neurophysiological changes in familial adult myoclonus epilepsy 2.

Familial adult myoclonus epilepsy (FAME) management relies on antiseizure medications (ASMs), which inadequately address myoclonus and cortical tremor. This study evaluates Perampanel (PER), an AMPA-receptor antagonist, for treating FAME symptoms. Fifteen FAME2 patients participated in an observational prospective study. They received up to 6 mg daily of PER and underwent Unified-Myoclonus-Rating-Scale (UMRS) before and after treatment. Neurophysiological evaluations, including somatosensory evoked potentials (SEPs) and transcranial magnetic stimulation (TMS), assessed PER's impact on cortical

Quality of Life in Adult and Pediatric Patients with Tropomyosin Receptor Kinase Fusion Cancer Receiving Larotrectinib.

Neurotrophic tyrosine receptor kinase (NTRK) gene fusions lead to chimeric tropomyosin receptor kinase (TRK) fusion proteins, which act as primary oncogenic drivers in diverse tumor types in adults and children. Larotrectinib, a highly selective and central nervous system-active TRK inhibitor, has shown high objective response rates, durable disease control, and a favorable safety profile in patients with TRK fusion cancer. The impact of larotrectinib on health-related quality of life (HRQoL) was evaluated in adult and pediatric patients in two phase I/II clinical trials (NAVIGATE; NCT02576431

Treatment burden and adherence in adults receiving long-term anti-VEGF intravitreal injections for retinal disease: a qualitative evidence synthesis protocol.

INTRODUCTION: Retinal diseases such as neovascular age-related macular degeneration and diabetic macular oedema are leading causes of irreversible vision loss and require intravitreal anti-vascular endothelial growth factor (VEGF) injections over many years. Repeated treatment imposes physical, psychological, social and financial burdens and is associated with reduced adherence and poorer real-world outcomes. Existing research is largely quantitative and does not explain, from patients' perspectives, why people persist with, struggle against or discontinue burdensome treatment. This review wil
