Suprasellar arachnoid cysts: an extension of the membrane of Liliequist.

A case of suprasellar arachnoid cyst and hydrocephalus diagnosed by computed tomography (CT) and positive-contrast ventriculography is presented. Transfrontal exploration of the 3rd ventricle revealed findings consistent with the cyst being, in fact, a large forward and upward diverticulum of the arachnoidal membrane of Liliequist. The authors suggest that some of the other similarly located arachnoid cysts are extensions of this membrane imperforate by previous infection, hemorrhage, or maldevelopment.

[Von Recklinghausen's neurofibromatosis with intracranial stenosis of the internal carotid artery and orbito-sphenoidal bone dysplasia (author's transl)].

A patient with fibromatosis of the skull and a so-called "parophthalmicus syndrome" is described in whom there was hypoplasia and stenosis of the homolateral internal carotid artery and aplasia of the ophthalmic artery. The vascular changes in this phakomatosis are discussed. We suggest that the vascular changes in the carotid be included in the extended parophthalmicus syndrome.

Joubert syndrome: episodic hyperpnea, abnormal eye movements, retardation and ataxia, associated with dysplasia of the cerebellar vermis.

A unique syndrome consisting of episodic hyperpnea, abnormal eye movements, ataxia and mental retardation, associated with agenesis of the cerebellar vermis, has been delineated in four siblings by Joubert et al. (1969). We describe three other children with this clinically recognizable condition which we suggest to call Joubert syndrome. There is good evidence that it is inherited as autosomal recessive. Two of out patients were brothers, the third child's parents were related. Recognition of this syndrome is important in view of prognosis and for genetic counseling.

The radiology of arachnoid pouches.

The radiographic appearances in 14 cases with arachnoid pouches are reviewed. Their clinical summaries, operative findings and anatomical distribution are tabulated.

Porencephaly diagnosed by isotope cisternography.

The diagnosis of porencephaly by isotope cisternography is described. In the three cases presented, porencephaly was associated with non-resorptive hydrocephalus. The communicating hydrocephalus caused the isotope to enter the ventricular system and visualize the cyst, and the diagnosis of both disorders was established by RIHSA cisternography. The method is simple and non-traumatic and provides information about abnormalities which air may fail to demonstrate.

[Radiologic diagnosis of acoustic neurinoma. Apropos of 130 patients].

Diagnostic value of radiologic examinations was assessed in a homogeneous series of 130 cases of neurinoma of acoustic nerve seen over the last 10 years. Radiologic exploration is conceived and used at the present time for two purposes. Firstly, it enables selection of patients with suspected acoustic nerve neurinoma. Radiotomography appears to be currently an accessory examination because of the discriminative performance of early auditory evoked potentials tests on the brain stem. Secondly, radiology is necessary for confirmation of diagnosis. Visualization of the tumoral syndrome can be obt

Contribution to the study of craniostenosis: disturbance of the cerebrospinal fluid flow in oxycephaly.

In 6 cases of oxycephaly, isotope (RIHSA) cisternography showed an altered CSF circulation with ventricular reflux or cisternal block and accumulation of the contrast at lumbosacral level. These changes express increased absorption of the CSF by the spinal arachnoid villi to compensate for reduced or nonexistent absorption by the subarachnoid villi of the vault, obstructed by chronic intracranial hypertension secondary to craniosynostosis. The possible clinical implications are outlined.
