Sonography of the knee is a well-established method for the diagnosis of palpable tumors. However, evaluation of the menisci, cruciate ligaments, and hyaline cartilage is controversial. Despite the variable echogenicity and extension of different diseases, many distinct sonographic signs are different in cysts, solid neoplasms, and vascular diseases. The investigator must be familiar with various patterns of popliteal cyst formation: crescent-shaped, cross-shaped, slitlike, and grape-like cysts. Because of diagnostic and economic reasons the following concept should provide guidance when diagn…

The paper reports an echographic study of the popliteal region designed to assess the utility of ultrasonography in the differential diagnosis of the various popliteal pathologies. In a year, 459 soft tissue samples were examined; 250 concerned the knee 138 patients gave negative results, while 112 cases presented this pathology with the following diagnosis: 94 cysts (21 appeared duplicated and monolateral, 28 duplicated and counterlateral), 2 aneurysms, 3 hematomas, 10 solid tumefactions (9 were benign, 1 malignant), 1 venous thrombosis and 2 chondromatosis. Of these 112 cases, 25 had been op

INTRODUCTION: Mycotic popliteal aneurysms are not a common phenomenon. They can initially be easily confused with other more trivial conditions such as a Baker's cyst. We present a case of a patient presenting with a progressively worsening leg swelling which was initially misdiagnosed. Only until symptoms rapidly progressed was a popliteal aneurysm diagnosed. To our knowledge this is the only identified case of a Streptococcus sanguinis mycotic popliteal aneurysm.
CASE PRESENTATION: An 81-year-old gentleman presented to the surgical assessment unit with a six-week history of a painful, diffu

Popliteal (Baker) cysts, meniscal cysts, proximal tibiofibular joint cysts, and cruciate ligament ganglion cysts are cystic masses commonly found about the knee. Popliteal cysts form when a bursa swells with synovial fluid, with or without a clear inciting etiology. Presentation ranges from asymptomatic to painful, limited knee motion. Management varies based on symptomatology and etiology. Meniscal cysts form within or adjacent to the menisci. These collections of synovial fluid are thought to develop from translocation of synovial cells or extravasation of synovial fluid into the meniscus th…

Cystic adventitial disease (CAD) of the popliteal artery is a rare vascular disease of unknown etiology in which a mucin-containing cyst develops in the adventitial layer of the artery. We report the case of a 26-year-old male with CAD of the right popliteal artery diagnosed non-invasively with 3 Tesla cardiovascular magnetic resonance and confirmed on post-operative histopathology.

The prevalence of peripheral artery disease (PAD) has been increasing in parallel with the increasing prevalence of the atherosclerotic disease. Therefore, we have to be familiar with the diagnostic approach used for ischemic symptoms in the lower limbs. Adventitial cystic disease (ACD) is rare but not negligible as one of the differential diagnoses of intermittent claudication (IC). Although duplex ultrasound and magnetic resonance imaging (MRI) are helpful tools for the diagnosis of ACD, further imaging modality is needed to avoid misdiagnosis. Cystic adventitial disease is a rare cause of calf claudication and is characterized by the development of a mucinous, cystic mass within the outer layer (adventitia) of an artery. The popliteal artery is most commonly affected. Several theories and hypotheses exist regarding the aetiology of this disorder with no clear unifying cause accepted in the literature to date. We describe a case of a 32-year-old female with a one-year history of medial right knee pain and intermittent claudication who was diagnosed with a medial meniscal tear and a large parameniscal cyst communicating with the popli

A 22-year-old male underwent an arthroscopic meniscal repair, using meniscal arrows, due to bucket-handle medial meniscus tear. Magnetic resonance images (MRI) at 3 years revealed a large parameniscal cyst protruding into the extra-capsular popliteal space. Arthroscopic partial cystectomy, removing only the intra-articular portion of the cyst was performed, leaving the extra-articular part of the cyst. Sixteen months postoperatively MRI revealed no cyst remnants, and the patient was free of symptoms.

The existence of a popliteal Baker's cyst was regarded as a contraindication for radiosynoviorthesis of the knee joint since decades. A so-c … potential hazard, knee joint ultrasound is mandatory prior to radiosynoviorthesis to check for any popliteal cysts. New studies however decline the risk of a radiogenic cyst rupture after an appropriately performed radiosynoviorthesis of the knee joint. In case of a preexistent cyst rupture, the risk of a radiogenic tissue damage remains an issue and magnetic re …

Adventitial cystic disease is a rare form of non-atherosclerotic stenosis of the popliteal artery. It is caused by synovial-like cysts in the subadventitial tissue layer of the arterial wall causing compression of the lumen. The cysts contain mucinous material similar to that found in joint-ganglia. Popliteal artery stenoses causing claudication in young patients is the leading symptom of this disease. The classical therapy is the surgical excision of the diseased artery and interposition grafting with saphenous vein. As an alternative method we present the technique of complete circumferentia

Cystic adventitial disease (CAD) of the popliteal artery is a rare but well-known cause of intermittent claudication, especially in young patients. The etiology of the disease is still controversial and the literature reports various hypotheses for its origin. Diagnosis starts with thorough history taking and physical examination; non invasive diagnostic studies comprise color duplex scanner (ECD), computed tomography (CT), better if elicoidal (3D CT) and magnetic resonance imaging (MRI), which can aid in establishing correct recognition of the disease in most cases. BackgroundBaker's cysts can cause significant vascular complications including popliteal vein compression and deep vein thrombosis (DVT), yet no validated tool exists to predict which patients are at highest risk. We aimed to develop and internally validate a clinical nomogram for predicting vascular complications in patients with Baker's cysts.MethodsWe conducted a retrospective cohort study of 1,364 consecutive patients diagnosed with Baker's cysts between January 2015 and December 2024 at a single tertiary center. The primary outcome was a composite of vascular complications (symptomatic po

A 50-year old female patient with unilateral knee pain demonstrated a recurrent ultrasound-proven popliteal cyst (Baker's cyst). Even though a proper differential diagnosis was done, the MRT of the knee showed in a secondary step a tibial fissure as the cause of the treatment-refractory knee pain and Baker's cyst. A fracture of the tibia is a rare cause for a symptomatic Baker's cysts. Mechanical, degenerative or inflamed diseases of the joint are more frequent associated with a Baker's cyst.

Ultrasonography was performed on 54 patients with lesions of the extremities and pelvic girdle. Computed tomography (CT) was employed in 8 patients. Focal space-occupying lesions such as tumors, abscesses, hematomas, aneurysms, and popliteal cysts (dissecting or otherwise) were delineated by ultrasound and their extent defined. Differentiation from diffuse changes such as lymphedema, cellulitis, or phlebitis and diagnosis of aneurysms were possible. Popliteal cysts were usually quite characteristic, while correlation with the clinical features suggested the correct diagnosis in the case of the…

Intra- or para-articular ganglion cysts are frequent and can develop at some distance from the joints. The authors describe examples of typical and atypical ganglion cysts, namely meniscal cyst, intra-articular cyst, intraosseous cyst, intraneural cyst of tibial nerve, adventitial cyst of popliteal artery, and para-articular cyst of the hip, filled with gas of presumed articular origin. The diagnosis of a ganglion cyst can be difficult when there is no evident articular communication. If demonstration of communication is necessary (in case of uncertain diagnosis or for preoperative assessment)

INTRODUCTION: Adventitial cystic disease of the popliteal artery (PA) is an uncommon and unique entity characterized by a mucinous cyst located in the arterial adventitia. As the cyst enlarges, it provokes vascular compression with stenosis or occlusion, the first only during the knee flexion, and then in all leg position. Atkins and Key (1946) were the first who described this disease in the external iliac artery [1]. Eirup and Hiertonn (1956) described the disease in the PA, which is the place of its most common localization. The aim of the paper is the presentation of our 10 cases of PA adv
