PURPOSE OF REVIEW: People living with HIV (PLWH) on contemporary antiretroviral therapy experience high rates of overweight/obesity, which predisposes to cardiometabolic disease and multiple other conditions with negative health consequences in this aging population. We aim to summarize the epidemiology and pathophysiology of obesity in PLWH and review recent advances in the therapeutic management of obesity.
RECENT FINDINGS: The prevalence of overweight/obesity in PLWH mirrors long-standing trends in the general population. Obesity and weight gain have a complex, multifactorial pathogenesis

BACKGROUND/OBJECTIVES: Recent advances in prostate cancer management have improved overall survival and quality of life for those afflicted. However, these patients continue to suffer from a high burden of cardiovascular disease through a variety of mechanisms, including both the cancer itself as well as prostate cancer-specific treatments.
SUBJECTS/METHODS: Cardiovascular disease is now the leading cause of death in this population, and current data suggests that many patients remain sub-optimally controlled from a risk factor standpoint. There is a lack of consensus and standardized guideli

BACKGROUND: Mental illness is the single most common reason for patient visits to a general practitioner (GP). Prevalent mental illnesses include major depression disorder and dysthymia, anxiety disorders, bipolar disorders and schizophrenia. Patients with severe mental illness have a 10-20-year shorter life expectancy when compared with the general population, primarily due to physical chronic disease.
OBJECTIVE: The aim of this article is to provide a concise update regarding the evidence-based clinical lifestyle interventions in the management of both mental illnesses and the physical chro

More than a quarter of the world's population is at risk of infection with the soil-transmitted helminths Ascaris lumbricoides, hookworm (Ancylostoma duodenale and Necator americanus), Trichuris trichiura, and Strongyloides stercoralis. Infected children and adults present with a range of medical and surgical conditions, and clinicians should consider the possibility of infection in individuals living in, or returning from, endemic regions. Although safe and effective drugs are donated free to endemic countries, only half of at-risk children received treatment in 2016. This Seminar describes t

PURPOSE OF THE REVIEW: The number of master athletes (MAs) is steadily increasing, reflecting broader societal trends in healthy aging and competitive sports participation beyond the age of 35. This work presents an up-to-date, evidence-based framework for evaluating the cardiovascular and general health of master athletes, integrating current guidelines with sport-specific considerations, and focusing primarily on cardiovascular prevention and risk management. It also acknowledges conditions that are more prevalent in this population-accelerated coronary calcification/coronary artery disease,

UNLABELLED: In the last few decades, the impact of chronic health conditions on health systems, as well as on the quality of life, frailty, and dependence of those affected, has been brought to light. The objective of this study was to describe the population care needs of highly complex chronic patients (HCCPs).
METHODS: An epidemiological observational study was conducted.
RESULTS: A total of 13,262 patients were identified, 51% of which were elderly women. Among all patients, 84.4% had received a nursing assessment related to health patterns. Three diagnoses were established in 25% of the

We sought to identify factors associated with being a reservoir district for wild poliovirus in Pakistan. Differences between reservoir and non-reservoir districts were identified using acute flaccid paralysis surveillance data, population census statistics and data from a survey of district health officials (DHOs). Of the 11 poliovirus reservoir districts identified, population density was significantly higher (median 550 persons/km2) than the non-reservoirs (median 175 persons/km2). DHOs from reservoir districts more often reported that planning was affected by refugees and they had more fre

Periodontal diseases remain highly prevalent worldwide and represent a significant public health concern, particularly among special populations. These groups, including geriatric and pediatric patients, pregnant women, individuals with disabilities, medically compromised patients, and socioeconomically disadvantaged populations, experience a disproportionately higher burden of disease due to complex interactions between systemic, behavioral, and environmental factors. This review provides a comprehensive overview of periodontal health in these diverse populations, highlighting key risk determ

The prevalence of obesity in older adults is rapidly increasing due to the demographic shift occurring globally. Older adults with obesity face health care challenges due to multimorbidity, functional dependency, and disability. This population often has prior history of struggles through weight-loss attempts during the middle years of life. Therefore, weight-loss intervention needs a personalized approach, taking into consideration weight-loss attempts, relapse, and life events, including bariatric surgery, functional limitations, psychosocial status, and current residential environment. OBJECTIVE: The nation's 2862 local health departments (LHDs) are the primary means for assuring public health services for all populations. The objective of this study is to assess the effect of organizational network analysis on management decisions in LHDs and to demonstrate the technique's ability to detect organizational adaptation over time.
DESIGN AND SETTING: We conducted a longitudinal network analysis in a full-service LHD with 113 employees serving about 187,000 persons. Network survey data were collected from employees at 3 times: months 0, 8, and 34. At time 1 the initial analysis

BACKGROUND: Osteoporosis has emerged as an important comorbidity for people living with HIV (PLWH) as life expectancy for this population has increased in the anti-retroviral therapy era. In countries lacking a robust primary care system such as China, chronic co-morbidities of HIV such as osteoporosis and fractures may be particularly under-recognized. Understanding barriers and opportunities for integration of bone health promotion strategies into current HIV care settings can aid in bone health program development and may provide a model of primary/secondary preventive care that is currentl

BACKGROUND: Adequate self-management behaviours may improve health outcomes, but dialysis patients often struggle to adopt and maintain these behaviours. Therefore, we aimed to 1) identify self-management barriers in dialysis patients, 2) assess their relation to patient characteristics, and 3) assess their effect on clinical and patient-reported outcomes.
METHODS: We conducted a multicentre, longitudinal cohort study in adult incident dialysis patients who filled in the Self-Management Screening questionnaire. Descriptive analyses were performed to assess the extent of self-management barrie

Our work develops procedures and useful tools for the screening, assessment and management of prevalent infectious or parasitic diseases (tuberculosis, measles, chickenpox, scabies, bed bugs) among asylum seekers and detainees in the canton of Vaud, populations living in similar closed settings. Its aim is to support health professionals in their work, to maintain the health of the target population and to protect the health of the community. Through a literature review and a focus group with experts, it is proposed to harmonize the existing procedures in asylum seekers centres end prisons of

Foodborne botulism is a life-threatening disease caused by the ingestion of food containing preformed botulinum neurotoxins, the most potent natural poisons known to humans. On the basis of the new challenges in management of the diseases as well as considering the potential use of botulinum toxins as biological weapons, foodborne botulism is still considered a public health emergency. Each suspected case should be immediately notified to public health authorities with the aim of preparing a prompt response. With the aim of improving botulism surveillance systems, health authorities as well as

BACKGROUND: Integrated care has been advanced by the World Health Organization as a strategic response to population ageing. Digital health platforms are increasingly central to this agenda, particularly for monitoring intrinsic capacity among older adults within community-based integrated care. This scoping review synthesizes the implementation and practice evidence on digital health platforms used to monitor intrinsic capacity in this population.
METHODS: This review followed the methodological framework proposed by Arksey and O'Malley. Six databases-PubMed, Web of Science, the Cochrane Lib

Odontoid fractures are the most common fracture of the axis and the most common cervical spine fracture in patients over 65. Despite their frequency, there is considerable ambiguity regarding optimal management strategies for these fractures in the elderly. Poor bone health and medical comorbidities contribute to increased surgical risk in this population; however, nonoperative management is associated with a risk of nonunion or fibrous union. We provide a review of the existing literature and discuss the classification and evaluation of odontoid fractures. The merits of operative vs nonoperat

PURPOSE: Autosomal recessive deficiencies of DNMT3B or ZBTB24 account for two-thirds of cases of immunodeficiency, centromeric instability and facial dysmorphism (ICF syndrome). This primary immunodeficiency (PID) is characterized mainly by an antibody deficiency, facial abnormalities and centromeric instability. We analyzed the national cohort of patients with ICF syndrome with the aim of providing a more detailed description of the phenotype and management of patients with ICF syndrome.
METHODS: Demographic, genetic, immunological, and clinical features were recorded for each patient.

Guatemala faces a growing burden of chronic noncommunicable diseases, with hypertension and type 2 diabetes mellitus being the leading contributors to cardiovascular mortality and progression to chronic kidney disease. In 2022, the Ministry of Public Health and Social Assistance began implementing the Pan American Health Organization/World Health Organization's HEARTS technical package to strengthen the prevention and clinical management of these conditions in low- and medium-complexity health care facilities. Between 2022 and 2025, the strategy was implemented in 11 Departmental Directorates

The impact of COVID-19-related mental health effects on health behaviors among people living with HIV (PLWH) remains unknown. Our qualitative study explored the relationship between the mental health consequences of the COVID-19 pandemic and self-management of HIV and cardiovascular disease (CVD) among PLWH in the United States. Twenty-four PLWH enrolled in a multicenter, cardiovascular, clinical trial completed one-on-one semistructured interviews to assess changes in mood, health behaviors, and comorbid CVD management during the COVID-19 pandemic. The majority of participants (ages 43-70 yea
