Root perforation repair: surgical and nonsurgical management.

Root perforation repair has historically been an unpredictable treatment modality with an unacceptably high rate of clinical failure. Recent developments in the techniques and materials utilized in root perforation repair have dramatically enhanced the prognosis of both surgical and nonsurgical procedures. This article presents a review of the literature pertaining to root perforation repair and illustrates, through clinical case presentations, the principles of extraradicular surgical repair and non-surgical internal repair of root perforation using mineral trioxide aggregate (MTA).

Laparoscopic temporary uterine artery occlusion combined with en bloc lesion resection for type III cesarean scar pregnancy: surgical technique.

This article presents a refined laparoscopic surgical technique for the management of type IIIb cesarean scar pregnancy (CSP), as classified by Ban et al.'s 2023 criteria [residual myometrial thickness <3 mm, maximum sac diameter (MSD) ≥3 cm, bladder protrusion], in a 35-year-old female with vaginal bleeding and a history of two cesarean sections. The core protocol integrates temporary bilateral uterine artery occlusion (simple ligation of the main trunk with No. 1 absorbable Vicryl suture at 2 cm from the uterine wall) immediately before resection, adhesiolysis, en bloc excision of the ectopi

Operative technique and results of core decompression and filling with bone grafts in the treatment of osteonecrosis of femoral head.

BACKGROUND: The aim of this paper is to present the operative technique and results of core decompression and clearing of a necrotic zone in the femoral head followed by filling the defect with impacted autologous or synthetic bone grafts. This procedure allows for mechanical prevention of further collapse of the femoral head and loss of its spherical shape. It also creates conditions for the incorporation of the biological graft or synthetic bone. MATERIALS AND METHODS: A series of 63 patients (72 hips) were treated for aseptic osteonecrosis of the femoral head between 1996 and 2009. This gr

The effects of chlormethiazole and nimodipine on cortical infarct area after focal cerebral ischaemia in the rat.

Focal ischaemia in the rat cerebral cortex was produced by means of a photochemically induced thrombosis of cerebral arteries. This was achieved by intravenous infusion of the photosensitive dye Rose Bengal and illumination of the skull with focused green light. Initial experiments justified the use of tetrazolium staining as an index of infarct damage. Using this technique it was demonstrated that chlormethiazole (200 mg/kg, i.p.) given 5 min post ischaemia markedly reduced the area of infarcted cortical tissue. A second experiment replicated this observation and showed that, in contrast, nim

Benchmark ab initio and dynamical characterization of the stationary points of reactive atom + alkane and SN2 potential energy surfaces.

We describe a composite ab initio approach to determine the best technically feasible relative energies of stationary points considering additive contributions of the CCSD(T)/complete-basis-set limit, core and post-CCSD(T) correlation, scalar relativistic and spin-orbit effects, and zero-point energy corrections. The importance and magnitude of the different energy terms are discussed using examples of atom/ion + molecule reactions, such as X + CH4/C2H6 and X- + CH3Y/CH3CH2Cl [X, Y = F, Cl, Br, I, OH, etc.]. We test the performance of various ab initio levels and recommend the modern explicitl

Mapping of blood flow and slow speckle tissue dynamics using laser speckle contrast imaging.

Laser speckle contrast imaging (LSCI) is a wide-field optical technique commonly used to monitor cerebral blood flow (CBF). Recently, we have discovered that besides the fast-decorrelating signals from blood flow, LSCI can also detect slow-decorrelating signals associated with cellular activity. The ability to image these signals has significant implications for various research areas such as ischemic stroke, as it enables longitudinal monitoring of both vascular and cellular dynamics, offering new biomarkers for tissue viability, injury progression, and therapeutic response. Here, we demonstr

Evaluation of the Activities of Thyroid Hormones Among Pre- and Post-menopausal Euthyroid Women: A Cross-sectional Study from a Tertiary Care Teaching Hospital in India.

Introduction Aging brings about several changes in humans that include both physiological and anatomical changes. As individuals' age, the activity of the thyroid gland and its hormones decline, causing significant metabolic disorders. Most thyroid gland disorders have been noted among young and middle-aged women. Very little is known regarding the activities of thyroid hormones among older aged women. Methods The study included 350 young to middle-aged pre-menopausal women between 25 and 49 years and 350 older post-menopausal women above 50 years of age. The study was conducted in the departm

Ubiquitin-like modifications in adipose tissue development and obesity: New insights.

Obesity involves abnormal adipose tissue remodeling, including increased fat cell number (hyperplasia) and size (hypertrophy), along with dysregulated adipogenesis, function, and turnover of white, beige, and brown fat cells. These changes cause chronic inflammation and insulin resistance, leading to metabolic diseases. Post-translational modifications (PTMs), especially ubiquitin and ubiquitin-like modifications (UBLs), regulate adipose development by attaching to proteins, affecting their activity, stability, and localization through a cascade involving E1, E2, and E3 enzymes. Although UBL m

Advances in mechanisms, imaging and management of the unstable plaque.

Post-mortem observations demonstrated that plaque fissure was the final event leading to coronary thrombosis and occlusion in about two-thirds of cases of sudden coronary death. Plaques prone to fissure have, therefore, been defined "vulnerable plaques" and are identified by specific anatomic features including thin inflamed fibrous cap, large lipidic core and positive remodeling. Accordingly, elegant imaging modalities have been developed in order to identify this "holy grail". However, the results of prognostic studies based on the identification of vulnerable plaques have not been encouragi

Combined orthodontic, surgical, and restorative approach to treat a complicated crown-root fracture in a maxillary central incisor.

This case report shows the treatment of a severe traumatic tooth injury. For the maxillary right central incisor, the trauma was considered a complicated crown-root fracture. The level of the fracture line, the length of the remaining root segment, and the presence and condition of the tooth fragment determined the type of therapy. Traumatized teeth with fractures below the alveolar crest are often considered hopeless. As this report shows, the treatment of a complicated crown-root fracture in the esthetic region can be challenging. Orthodontic extrusion and crown-length surgery were performed
