PURPOSE: To report a case of complication after inadvertent Nd:YAG laser in a phakic eye.
OBSERVATIONS: A 65-year-old man presented with a sudden vision loss on his right eye after undergoing Nd:YAG laser for posterior capsulotomy. Best-corrected visual acuity (BCVA) was hand motion and slit-lamp examination revealed a dense posterior cataract with 3 circular holes on posterior capsule and on anterior segment OCT confirmed a posterior capsule rupture (PCR). Due to scattered fine echoes on eye ultrasound, suggesting remnants of cataract, the patient underwent phacoemulsification with posterior…

A consecutive group of 67 patients (70 eyes) with senile cataract underwent extracapsular cataract extraction and implantation of a Shearing posterior chamber intraocular lens. After two years of follow-up, there was an 11% +/- 2% decrease in central cell density (P less than .001), a 20% +/- 2% decrease in superior cell density (P less than .001), and a 5% +/- 3% decrease in inferior cell density (P less than .005). Complications included corneal edema requiring a corneal transplant (one patient), a sterile nonhealing ulcer that required a conjunctival flap (one patient), and secondary glauco

Fifty-one soft hydrogel posterior chamber intraocular lenses (IOLs) were implanted in the ciliary sulcus after planned extracapsular cataract extractions. In nine cases the IOLs were positioned with one haptic in the capsular bag and the other in the ciliary sulcus. During a one-year follow-up, Nd:YAG capsulotomy was performed on five eyes and cystoid macular edema was present in two cases. No other serious complications were encountered. At one year, visual acuity was 20/30 or better in all eyes that did not have preexisting secondary eye problems. To prevent postoperative rotation, decentrat

Neodymium-doped:yttrium-aluminum-garnet (Nd:YAG) laser capsulotomy is considered the gold standard treatment for visually significant posterior capsule opacification (PCO), which remains the most common long-term complication following cataract surgery. However, Nd:YAG laser treatment carries the risk of inducing intraocular lens (IOL) damage, including pits, cracks, and optical degradation, particularly affecting the quality of premium hydrophobic acrylic and small-aperture IOLs. To systematically review the morphology, material dependence, optical, and clinical consequences, as well as preve

The author considered the following important points:(1) Anterior capsular synechia to a corneal incision (made by a keratome) after the evacuation of a traumatic cataract. This might be detached early by the use of a blunt-ended knife following a perforation of the cornea with a sharp-pointed knife, much like a tenotome.(2) The involuntary prolapse of capsule with a cataract incision.(a) The danger of this was demonstrated as the cause of glaucoma, especially if it be found necessary to divide opaque capsular membrane after the extraction.(b) The danger of sympathetic ophthalmia.Prolapse migh

A 43-year-old man with high myopia developed unilateral pigmentary glaucoma with recurrent episodes of painless blurred vision in the left eye following uneventful phacoemulsification. Bilateral cataract surgery was performed with capsular bag implantation of a +4.00 diopter AcrySof MA60 intraocular lens (IOL) (Alcon Laboratories) followed by a neodymium:YAG laser capsulotomy. Secondary iatrogenic dispersion syndrome in the left eye with subsequent intraocular pressure elevation was suspected in the presence of anterior chamber pigmented cells, circular epithelial iris loss around the pupil, a
