Double immunodiffusion (DID) is a useful, inexpensive antibody screening test that is more specific than complement fixation for blastomycosis. Seven of 10 patients with proved blastomycosis had positive results on DID assays for antibody to the A antigen of Blastomyces dermatitidis. Sputum cytologic examination and skin or other tissue biopsies may be diagnostic if the organism is demonstrated. Culture remains the sina qua non of diagnosis, and a negative DID test result for A precipitin does not rule out this infection. Forty-seven men on the Isthmus of Panama were exposed to histoplasmosis in an old bunker inhabited by bats. The resulting epidemic was studied with serial clinical, serological, and radiological examinations. Ethanol-precipitable culture filtrate antigens of 100 strains of 75 species of the Sporothrix-Ceratocystis-Europhium-Graphium complex and 1 species of Botrytis were examined for neutral sugar components and for serological cross-reactivity with S. schenckii rabbit antiserum and human sporotrichosis sera by capillary precipitin and double immunodiffusion assay. A survey of laboratory records was made to assess the value of the precipitin test and isolation methods in the diagnosis of farmer's lung disease and also to determine its prevalence in the farming population of Somerset. Two cases of an acute chest illness occurring in mushroom workers are described. In one patient, who was severely ill, a lung biopsy was obtained which showed a pattern of alveolitis and interstitial fibrosis.
