Validation of an Algorithm for the Prediction of Diabetic Retinopathy in Type 1 Diabetic Patients in a Spanish Population.

AIM: The aim of the present study was to build a clinical decision support system (CDSS) that can predict the presence of diabetic retinopathy (DR) in type 1 diabetes (T1DM) patients. MATERIAL AND METHOD: We built two versions of our CDSS to predict the presence of any-type DR and sight-threatening DR (STDR) in T1DM patients. The first version was trained using 324 T1DM and 826 T2DM patients. The second was trained with only the 324 T1DM patients. RESULTS: The first version achieved an accuracy (ACC) = 0.795, specificity (SP) = 83%, and sensitivity (S) = 65.7% to predict the presence of any-

Feasibility of Machine Learning and Logistic Regression Algorithms to Predict Outcome in Orthopaedic Trauma Surgery.

BACKGROUND: Statistical models using machine learning (ML) have the potential for more accurate estimates of the probability of binary events than logistic regression. The present study used existing data sets from large musculoskeletal trauma trials to address the following study questions: (1) Do ML models produce better probability estimates than logistic regression models? (2) Are ML models influenced by different variables than logistic regression models? METHODS: We created ML and logistic regression models that estimated the probability of a specific fracture (posterior malleolar invol

A novel machine-learning algorithm for predicting mortality risk after hip fracture surgery.

INTRODUCTION: Although several risk stratification models have been developed to predict hip fracture mortality, efforts are still being placed in this area. Our aim is to (1) construct a risk prediction model for long-term mortality after hip fracture utilizing the RSF method and (2) to evaluate the changing effects over time of individual pre- and post-treatment variables on predicting mortality. METHODS: 1330 hip fracture surgical patients were included. Forty-five admission and in-hospital variables were analyzed as potential predictors of all-cause mortality. A random survival forest (RS

Combinatorial Pharmacogenomic Algorithm is Predictive of Citalopram and Escitalopram Metabolism in Patients with Major Depressive Disorder.

Pharmacogenomic tests used to guide clinical treatment for major depressive disorder (MDD) must be thoroughly validated. One important assessment of validity is the ability to predict medication blood levels, which reflect altered metabolism. Historically, the metabolic impact of individual genes has been evaluated; however, we now know that multiple genes are often involved in medication metabolism. Here, we evaluated the ability of individual pharmacokinetic genes (CYP2C19, CYP2D6, CYP3A4) and a combinatorial pharmacogenomic test (GeneSight Psychotropic®; weighted assessment of all three gen

External Validation of an Algorithm to Predict Adjacent Musculoskeletal Infection in Pediatric Patients With Septic Arthritis.

BACKGROUND: Septic arthritis (SA) remains a potentially morbid disease in the pediatric population. Magnetic resonance imaging (MRI) is the most sensitive tool for recognizing associated osteomyelitis and intramuscular abscess, but is a limited resource. The aim of this study is to externally validate a previously developed algorithm (Rosenfeld and colleagues) to predict adjacent infection in pediatric patients diagnosed with SA. METHODS: We identified 120 children under 16 with presumed SA presenting to a tertiary referral center between 2008 and 2018. Patients without confirmed SA, those wi

Decision tree algorithms predict the diagnosis and outcome of dengue fever in the early phase of illness.

BACKGROUND: Dengue is re-emerging throughout the tropical world, causing frequent recurrent epidemics. The initial clinical manifestation of dengue often is confused with other febrile states confounding both clinical management and disease surveillance. Evidence-based triage strategies that identify individuals likely to be in the early stages of dengue illness can direct patient stratification for clinical investigations, management, and virological surveillance. Here we report the identification of algorithms that differentiate dengue from other febrile illnesses in the primary care setting

HLAMatchmaker algorithm is not a suitable tool to predict the alloreactive cytotoxic T-lymphocyte response in vitro.

Both donor-specific anti-human leukocyte antigen (HLA) antibodies and cytotoxic T lymphocytes are important mediators of graft rejection. HLAMatchmaker determines the amino acid triplets on antibody-accessible sites of the HLA molecule that are not shared between patient and donor. A previous study showed a strong positive correlation between the number of triplet mismatches and the percentage of individuals producing HLA antibodies. In the present study, we tested whether the number of triplet mismatches is predictive for the cytotoxic T-lymphocyte precursor (CTLp) frequency in vitro. The ana

Individualized prediction of non-sentinel lymph node metastasis in Chinese breast cancer patients with ≥ 3 positive sentinel lymph nodes based on machine-learning algorithms.

BACKGROUND: Axillary lymph node dissection (ALND) is a standard procedure for early-stage breast cancer (BC) patients with three or more positive sentinel lymph nodes (SLNs). However, ALND can lead to significant postoperative complications without always providing additional clinical benefits. This study aims to develop machine-learning (ML) models to predict non-sentinel lymph node (non-SLN) metastasis in Chinese BC patients with three or more positive SLNs, potentially allowing the omission of ALND. METHODS: Data from 2217 BC patients who underwent SLN biopsy at Shantou University Medical

Permeability Prediction and Potential Site Assessment for CO2 Storage from Core Data and Well-Log Data in Malay Basin Using Advanced Machine Learning Algorithms.

Establishing a potential site characterization for carbon dioxide (CO2) storage in geological formations anticipates the appropriate reservoir properties, such as porosity, permeability, and so forth. Well logs and seismic data were utilized to determine key reservoir properties, including volume of shale, porosity, permeability, and water saturation. These properties were cross validated with core data sets to ensure accuracy. To enhance permeability estimation, sophisticated machine learning (ML) methods were employed, categorizing permeability into five classes ranging from extremely good (

Exclusion of focal vertebral artifacts from spine bone densitometry and fracture prediction: a comparison of expert physicians, three computer algorithms, and the minimum vertebra.

UNLABELLED: Expert physicians and automated methods for the exclusion of vertebral levels in DXA scans containing focal artifacts were compared. All methods of vertebral exclusion led to a small improvement in fracture prediction. Computer algorithms performed at least as well as physicians. INTRODUCTION: Lumbar spine DXA is often confounded by focal artifacts. Clinical rules and automated methods for vertebral exclusion have been proposed, but their concordance, effect on diagnosis, and fracture prediction is unknown. MATERIALS AND METHODS: We analyzed clinical DXA scans of the lumbar spine

Construction of an interpretable prediction model for poor functional outcome in conservatively managed basal ganglia hemorrhage based on CT radiomics and multiple machine learning algorithms.

OBJECTIVE: Among patients with conservatively managed basal ganglia intracerebral hemorrhage (BG-ICH), a 3-month mRS ≥ 4 signals a poor functional outcome. Routine non-contrast CT reveals intralesional density heterogeneity, yet conventional clinical indicators largely overlook this information, weakening long-term prognostic accuracy. METHODS: We retrospectively enrolled 254 conservatively treated BG-ICH patients and randomly split them into training and internal validation sets (7:3). Clinical variables were compared between outcome groups, and radiomic features were extracted from admissio
