Under-prescribing of Prevention Drugs and Primary Prevention of Stroke and Transient Ischaemic Attack in UK General Practice: A Retrospective Analysis.

BACKGROUND: Stroke is a leading cause of death and disability; worldwide it is estimated that 16.9 million people have a first stroke each year. Lipid-lowering, anticoagulant, and antihypertensive drugs can prevent strokes, but may be underused. METHODS AND FINDINGS: We analysed anonymised electronic primary care records from a United Kingdom (UK) primary care database that covers approximately 6% of the UK population. Patients with first-ever stroke/transient ischaemic attack (TIA), ≥18 y, with diagnosis between 1 January 2009 and 31 December 2013, were included. Drugs were considered under-

Lymphedema microsurgical preventive healing approach: a new technique for primary prevention of arm lymphedema after mastectomy.

BACKGROUND: The purpose of this manuscript is to assess the efficacy of direct lymphatic venous microsurgery in the prevention of lymphedema following axillary dissection for breast cancer. METHODS: Nineteen patients with operable breast cancer requiring an axillary dissection underwent surgery, carrying out LVA between the blue lymphatics and an axillary vein branch simultaneously. The follow-up after 6 and 12 months from the operation included circumferential measurements in all cases and lymphangioscintigraphy only in 18 patients out of 19 cases. RESULTS: Blue nodes in relation to lymphat

Routine immunization of adults in Canada: Review of the epidemiology of vaccine-preventable diseases and current recommendations for primary prevention.

Vaccination is one of the greatest achievements in public health of the 20th century. However, the success of vaccine uptake and adherence to immunization guidelines seen in pediatric populations has not been observed among adult Canadians. As a result of the disparity in susceptibility to vaccine-preventable disease, there has been an increasing shift of vaccine-preventable childhood diseases into adult populations. Accordingly, morbidity and mortality due to vaccine-preventable illnesses now occur disproportionately in adults. All Canadians, irrespective of age, should have immunity to measl

Prevention of skin cancer in primary healthcare: an evaluation of three different prevention effort levels and the applicability of a phototest.

BACKGROUND/OBJECTIVE: The high skin cancer incidence in western society, and its known association with sun exposure habits, makes the area an important target for prevention. We investigated, in a primary healthcare setting, differentiated levels of prevention efforts directed at the propensity of the patient to change his/her sun habits, sun protection behaviour, and attitudes, after information intervention. Additionally, the impact of the performance of a phototest to determine individual sun sensitivity was evaluated. METHODS: 308 patients visiting a primary healthcare centre in southern

The case for primary prevention of obesity in the era of GLP-1 therapies.

Obesity rates continue to rise across Europe, driven by obesogenic environments shaped by food systems, urban design, socioeconomic inequali … aper argues that the growing availability of medications actually reinforces the need to prioritize primary prevention. While pharmacotherapy can improve individual outcomes, it cannot lower population-level risk or deliver the wider societal, environmental, and economic benefits of prevention. The paper also outlines key policy implications to reaffirm prevention a …

Telehealth for primary and preventive care among food-insecure individuals.

No preclinical experimental approach enables the study of voluntary oral consumption of high-concentration Δ9-tetrahydrocannabinol (THC) and its intoxicating effects, mainly owing to the aversive response of rodents to THC that limits intake. Here, we developed a palatable THC formulation and an optimized access paradigm in mice to drive voluntary consumption. THC was formulated in chocolate gelatin (THC-E-gel). Adult male and female mice were allowed ad libitum access for 1 and 2 hr. Cannabimimetic responses (hypolocomotion, analgesia, and hypothermia) were measured following access. Levels o

[Preventing pain chronification : the key role of primary care physicians].

Chronic pain affects approximately 1.5 million people in Switzerland, representing a major burden for patients and society. Family physicians are uniquely positioned to identify patients at risk of pain chronification and implement preventive interventions. This article presents the conceptual framework of primary, secondary, and tertiary pain prevention, the key biopsychosocial risk factors for pain chronification, validated screening tools for clinical practice, and evidence-based intervention strategies. Using paradigmatic examples, such as low back pain, the most frequent painful condition

Aspirin for Primary Prevention of Cardiovascular Events in Relation to Lipoprotein(a) Genotypes.

BACKGROUND: The role of aspirin in reducing lipoprotein(a)-mediated atherothrombotic events in primary prevention is not established. OBJECTIVES: This study sought to assess whether low-dose aspirin benefits individuals with elevated plasma lipoprotein(a)-associated genotypes in the setting of primary prevention. METHODS: The study analyzed 12,815 genotyped individuals ≥70 years of age of European ancestry and without prior cardiovascular disease events enrolled in the ASPREE (ASPirin in Reducing Events in the Elderly) randomized controlled trial of 100 mg/d aspirin. We defined lipoprotein(a…

Primary prevention of overweight and obesity in adolescents: An overview of systematic reviews.

The aim of this overview of systematic reviews was to summarize evidence from up-to-date reviews of the effectiveness of interventions aimed at preventing overweight and obesity in adolescents aged 10 to 19 years. We searched nine databases for systematic reviews published between January 2008 and November 2019. We used A Measurement Tool to Assess Systematic Reviews (AMSTAR) 2 to assess the quality of reviews, excluding those of critically low quality, and the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) tool to grade the certainty of included evidence. We inclu

[Prevention of impaired mobility in the elderly in primary care: a brief report].

The loss autonomy in the aging population is a major public health issue. Mobility impairment, which precedes loss of autonomy, could yet be accessible to multi-modal and personalised programs to enhance balance and physical performances, and avoid loss of autonomy. Moreover, such preventive programs are likely to be more effective when the patients are taken in charge before presenting with difficulties in daily living. The aging demographics and the consequences of the loss of autonomy clearly indicate a need for better addressing these patients with lowering mobility, even though they will

Prevention of parastomal hernia by placement of a polypropylene mesh at the primary operation.

INTRODUCTION: Parastomal hernias occur frequently after placement of a permanent colostomy. Preliminary reports have shown a beneficial effect of placing a mesh at the primary operation to prevent the formation of a parastomal hernia. We studied the safety and prophylactic effect of placing a newly designed polypropylene mesh in an onlay position at the primary operation. METHODS: This was a prospective study that included 25 patients scheduled for elective colorectal surgery. Risk factors for development of parastomal hernia were recorded before surgery. A prepared lasercut polypropylene mes

Cost-effectiveness of a program to prevent depression relapse in primary care.

OBJECTIVE: Evaluate the incremental cost-effectiveness of a depression relapse prevention program in primary care. MATERIALS AND METHODS: Primary care patients initiating antidepressant treatment completed a standardized telephone assessment 6-8 weeks later. Those recovered from the current episode but at high risk for relapse (based on history of recurrent depression or dysthymia) were offered randomization to usual care or a relapse prevention intervention. The intervention included systematic patient education, two psychoeducational visits with a depression prevention specialist, shared de
