[Late hematogenous infection of prosthetic joint].

The importance of prevention in late hematogenous infection is well understood but, because responsibility lies with general practitioners and other specialists, the orthopedic surgeon is usually not much interested. In both our and other countries, discussions are taking place on whether and to what extent antibiotic prevention should be carried out. Antibiotic prophylaxis of hematogenous infection is not indicated for all patients with joint arthroplasty, but only for a limited, defined group of patients at high risk. In these, however, the present state of knowledge suggests that prevention

Management of the Infected Endoprosthesis.

Limb salvage surgery has become the primary means of treatment for patients with malignant tumors of the extremities and pelvis. For these patients, endoprostheses have become the principal means to reconstruct the extremity following an oncologic resection because they offer an off-the-shelf-option that is modular, cost effective, and durable. As with other forms of arthroplasty, several modes of failure can occur, with infection being common. Management of an infected endoprosthesis is individualized based on numerous factors that the treating surgeon needs to consider. It is important to pr

Etiology, pathology, and host-impaired immunity in medical implant-associated infections.

Host impaired immunity and pathogens adhesion factors are the key elements in analyzing medical implant-associated infections (MIAI). The infection chances are further influenced by surface properties of implants. This review addresses the medical implant-associated pathogens and summarizes the etiology, pathology, and host-impaired immunity in MIAI. Several bacterial and fungal pathogens have been isolated from MIAI; together, they form cross-kingdom species biofilms and support each other in different ways. The adhesion factors initiate the pathogen's adherence on the implant's surface; howe

Pocket histology at cardiac implantable electronic device replacement: What's new?

BACKGROUND: Repeated procedures involving the cardiac implantable electronic device (CIED) pocket increase the infection risk, and the extent of pocket adhesions may prolong the procedure time. Few data on pocket histology at the time of CIED replacement are available. OBJECTIVE: The purpose of this study was to describe CIED pocket histology in a cohort of patients undergoing CIED replacement or upgrade. METHODS: All consecutive patients undergoing CIED replacement or upgrade at our center between November 2019 and May 2020 were enrolled. Subclinical pocket infection was ruled out by physic

[Present situation and prospect of diagnosis and treatment of periprosthetic joint infections].

Periprosthetic joint infection (PJI) as an insurmountable challenge has always represented a serious concern for orthopedic surgeons. Accurate diagnosis and reasonable treatment of PJI are the keys to deal with this challenge. The diagnosis of PJI is mainly based on clinical manifestations, joint fluid biochemistry, pathogen culture and biomarkers at present. New pathogen culture methods and specific biomarkers in recent years are hot topics in PJI research. In terms of treatment, two-stage revision is still considered to be the 'gold standard' protocol. However, more and more centers around th

Prevention of Periprosthetic Joint Infections of the Hip and Knee.

Periprosthetic joint infection (PJI) is a rare but devastating complication of arthroplasty. Research has been dedicated to minimizing the incidence of PJI, leading to the development of a comprehensive perioperative approach. Multiple preoperative, intraoperative, and postoperative factors can increase patient risk. From medical management and skin sterilization to wound sterility and blood management, multiple issues must be considered in a well-rounded prevention protocol. In this literature review, we consolidate the current information that orthopedic surgeons can use to minimize PJI afte

Letter to the Editor regarding "Risk of surgical site infection associated with intraoperative smartphone use in a sterile cover during total knee arthroplasty".

PURPOSE: To critically examine the interpretation of the reported association between intraoperative smartphone use in a sterile cover and surgical site infection after total knee arthroplasty. METHODS: We appraised the study's infection definition and surveillance period, statistical precision, interpretation of nonsignificant findings, and reporting of the institutional infection-prevention bundle. RESULTS: Defining the outcome as periprosthetic joint infection within one year may capture events less plausibly attributable to intraoperative contamination. Reporting infections separately at

Comparative effectiveness of antimicrobial implant surface coatings in preventing orthopaedic implant-associated infections: a network meta-analysis.

PURPOSE: Implant-associated infections (IAIs) remain a major challenge in orthopaedic surgery, causing substantial morbidity, mortality, and healthcare costs. Antimicrobial implant coatings have emerged as a promising preventive strategy, but their comparative clinical benefit remains unclear. This study aimed to evaluate the effectiveness of antimicrobial coatings in preventing IAIs and to compare their clinical performance to uncoated implants. METHODS: A systematic review and network meta-analysis was conducted in accordance with PRISMA guidelines. Medline, Embase, Scopus, and Web of Scienc

Above-the-knee amputation after a total knee replacement: prevalence, etiology, and functional outcome.

BACKGROUND: Despite modern surgical techniques, salvage of a failed total knee replacement remains a challenge. In certain situations, when other treatment options have been exhausted, patients with a failed total knee replacement may become candidates for above-the-knee amputation. The objective of this study was to assess the prevalence, etiology, and functional outcome of above-the-knee amputation performed proximal to an ipsilateral total knee replacement. METHODS: From 1970 to 2000, 18,443 primary total knee replacements were performed at our institution; sixty-seven (0.36%) were eventua

Evidence for osteocyte-mediated bone-matrix degradation associated with periprosthetic joint infection (PJI).

Osteomyelitis associated with periprosthetic joint infection (PJI) signals a chronic infection and the need for revision surgery. An osteomyelitic bone exhibits distinct morphological features, including evidence for osteolysis and an accelerated bone remodelling into poorly organised, poor-quality bone. In addition to immune cells, various bone cell-types have been implicated in the pathology. The present study sought to determine the types of bone-cell activities in human PJI bones. Acetabular biopsies from peri-implant bone from patients undergoing revision total hip replacement (THR) for c

Short or Long Antibiotic Regimes in Orthopaedics (SOLARIO): a randomised controlled open-label non-inferiority trial of duration of systemic antibiotics in adults with orthopaedic infection treated operatively with local antibiotic therapy.

BACKGROUND: Orthopaedic infections, such as osteomyelitis, diabetic foot infection and prosthetic joint infection, are most commonly treated by a combination of surgical debridement and a prolonged course of systemic antibiotics, usually for at least 4-6 weeks. Use of local antibiotics, implanted directly into the site of infection at the time of surgery, may improve antibiotic delivery and allow us to shorten the duration of systemic antibiotic therapy, thereby limiting the frequency of side effects, cost and selection pressure for antimicrobial resistance. METHODS: SOLARIO is a multicentre
