REM Sleep Impairment May Underlie Sleep-Driven Modulations of Tinnitus in Sleep Intermittent Tinnitus Subjects: A Controlled Study.

(1) Background: Poor sleep and fragmented sleep are associated with several chronic conditions. Tinnitus is an auditory symptom that often negatively combines with poor sleep and has been associated with sleep impairment and sleep apnea. The relationship between tinnitus psychoacoustic characteristics and sleep is still poorly explored, notably for a particular subgroup of patients, for whom the perceived loudness of their tinnitus is highly modulated by sleep. (2) Methods: For this observational prospective study, 30 subjects with tinnitus were recruited, including 15 "sleep intermittent tinn

Apical Reference Stimulation: A Possible Solution to Facial Nerve Stimulation.

OBJECTIVES: Postimplantation facial nerve stimulation is a common side-effect of intracochlear electrical stimulation. Facial nerve stimulation occurs when electric current intended to stimulate the auditory nerve, spread beyond the cochlea to excite the nearby facial nerve, causing involuntarily facial muscle contractions. Facial nerve stimulation can often be resolved through adjustments in speech processor fitting but, in some instances, these measures exhibit limited benefit or may have a detrimental effect on speech perception. In this study, apical reference stimulation mode was investig

Amplitude reduction in brainstem auditory response in term infants under neonatal intensive care.

OBJECTIVE: To examine brainstem auditory electrophysiology in term neonates under intensive care due to perinatal conditions other than hypoxia-ischemia. METHODS: Maximum length sequence brainstem auditory evoked response was studied in term neonates in an intensive care unit. The amplitudes of wave components of the response were analysed to assess brainstem auditory electrophysiology. RESULTS: The amplitudes of all wave components in the neonates under intensive care tended to be smaller than in those in normal term controls. Wave I amplitude was significantly reduced at all 91-910/s click

The relationship between inexperienced listeners' perceptions and acoustic correlates of children's /r/ productions.

This study examined inexperienced listeners' perceptions of children's naturally produced /r/ sounds with reference to levels of accuracy determined by consensus between two expert clinicians. Participants rated /r/ sounds as fully correct, distorted or incorrect/non-rhotic. Second and third formant heights were measured to explore the relationship between acoustic cues and perceptual judgments. Inexperienced listeners' agreement was greater for correct productions than for distorted or incorrect/non-rhotic productions. In addition, inexperienced listeners' differentiation of intermediate vers

Effects of simulated spectral holes on speech intelligibility and spatial release from masking under binaural and monaural listening.

The possibility that "dead regions" or "spectral holes" can account for some differences in performance between bilateral cochlear implant (CI) users and normal-hearing listeners was explored. Using a 20-band noise-excited vocoder to simulate CI processing, this study examined effects of spectral holes on speech reception thresholds (SRTs) and spatial release from masking (SRM) in difficult listening conditions. Prior to processing, stimuli were convolved through head-related transfer-functions to provide listeners with free-field directional cues. Processed stimuli were presented over headpho

Amplification of interaural level differences improves sound localization in acoustic simulations of bimodal hearing.

Users of a cochlear implant and contralateral hearing aid are sensitive to interaural level differences (ILDs). However, when using their clinical devices, most of these subjects cannot use ILD cues for localization in the horizontal plane. This is partly due to a lack of high-frequency residual hearing in the acoustically stimulated ear. Using acoustic simulations of a cochlear implant and hearing loss, it is shown that localization performance can be improved by up to 14 degrees rms error relative to 48 degrees rms error for broadband noise by artificially introducing ILD cues in the low fre

The effect of a precursor on growth of forward masking.

This study examined the effect of an on-frequency precursor on growth-of-masking (GOM) functions measured using an off-frequency masker. The signal was a 6-ms, 4-kHz tone. A GOM function was measured using a 40-ms, 2.8-kHz tone (the off-frequency masker). GOM functions were then measured with an on-frequency, fixed level precursor presented before the off-frequency masker. The precursor was 50 or 60 dB SPL, and 160 ms in duration. For the 60-dB SPL precursor, a 40-ms duration was also used. Two-line functions were fit to the GOM data to estimate the basilar membrane input-output function. The

Behavioral and evoked-potential thresholds in young and old Mongolian gerbils (Meriones unguiculatus).

Age-dependent hearing loss has been well documented in gerbils exceeding 2 years of age using physiological methods (e.g. [Mills et al. (1990) Hear. Res. 46, 201-210]). We determined behavioral thresholds for broad-band noise and pure-tone pulses in gerbils as a function of age. Contrary to expectations based on previously published physiological data, we found no significant (broad-band noise and 10 kHz) or only a very small hearing loss (7 dB at 2 kHz) in 30-36-month-old animals. In animals over 3 years of age we observed an increased spread of thresholds and threshold shifts exceeding 20 dB

Psychophysical suppression effects for tonal and speech signals.

This experiment assessed the benefits of suppression and the impact of reduced or absent suppression on speech recognition in noise. Psychophysical suppression was measured in forward masking using tonal maskers and suppressors and band limited noise maskers and suppressors. Subjects were 10 younger and 10 older adults with normal hearing, and 10 older adults with cochlear hearing loss. For younger subjects with normal hearing, suppression measured with noise maskers increased with masker level and was larger at 2.0 kHz than at 0.8 kHz. Less suppression was observed for older than younger subj

Consensus Statements on Tinnitus Assessment and Treatment Outcome Evaluation: A Delphi Study by the Korean Tinnitus Study Group.

BACKGROUND: Tinnitus is a multifactorial condition with no universally accepted assessment guidelines. The Korean Tinnitus Study Group previously established consensus statements on the definition, classification, and diagnostic tests for tinnitus. As a continuation of this effort, this study aims to establish expert consensus on tinnitus assessment and treatment outcome evaluation, specifically tailored to the Korean clinical context. METHODS: A modified Delphi method involving 26 otology experts from across Korea was used. A two-round Delphi survey was conducted to evaluate statements relat

Simulation of hearing loss can induce pitch shifts for complex tones.

BACKGROUND: Most studies on pitch shift provoked by hearing loss have been conducted using pure tones. However, many sounds encountered in everyday life are harmonic complex tones. In the present study, psychoacoustic experiments using complex tones were performed on healthy participants, and the possible mechanisms that cause pitch shift due to hearing loss are discussed. METHODS: Two experiments were performed in this study. In experiment 1, two tones were presented, and the participants were asked to select the tone that was higher in pitch. Partials with frequencies less than 250, 500, 75

Temporal Sequence of Hearing Impairment and Cognition in the Baltimore Longitudinal Study of Aging.

BACKGROUND: Hearing impairment (HI) could be a risk factor for cognitive decline, but cognition could plausibly also affect psychoacoustic assessment of hearing with audiometry. We examined the temporal sequence of hearing and cognitive function among nondemented, community-dwelling older adults. METHODS: Hearing and cognition were assessed between 2012 and 2015 and 2 years thereafter in 313 nondemented participants aged ≥60 years in the Baltimore Longitudinal Study of Aging. Poorer hearing was defined by pure-tone average of 0.5-4 kHz tones in the better-hearing ear. Cognitive measures with

Tinnitus and other auditory problems - occupational noise exposure below risk limits may cause inner ear dysfunction.

The aim of the investigation was to study if dysfunctions associated to the cochlea or its regulatory system can be found, and possibly explain hearing problems in subjects with normal or near-normal audiograms. The design was a prospective study of subjects recruited from the general population. The included subjects were persons with auditory problems who had normal, or near-normal, pure tone hearing thresholds, who could be included in one of three subgroups: teachers, Education; people working with music, Music; and people with moderate or negligible noise exposure, Other. A fourth group i

Auditory and cognitive factors underlying individual differences in aided speech-understanding among older adults.

This study was designed to address individual differences in aided speech understanding among a relatively large group of older adults. The group of older adults consisted of 98 adults (50 female and 48 male) ranging in age from 60 to 86 (mean = 69.2). Hearing loss was typical for this age group and about 90% had not worn hearing aids. All subjects completed a battery of tests, including cognitive (6 measures), psychophysical (17 measures), and speech-understanding (9 measures), as well as the Speech, Spatial, and Qualities of Hearing (SSQ) self-report scale. Most of the speech-understanding m

Tonal Tinnitus Does Not Interfere with Tone Detection at the Tinnitus Pitch-Matched Frequency.

Individuals with tinnitus hear sounds that are not present in the external environment. Whereas hearing difficulties at frequencies near those matching the tinnitus pitch are a common complaint for individuals with tinnitus, it is unclear to what extent the internal tinnitus sounds interfere with the detection of external sounds. We therefore studied whether pure-tone detection at the estimated frequency corresponding to the tinnitus pitch (ftp) was affected by confusion with the tinnitus percept. Signs of confusion would be a high false alarm rate or a shallower slope of the psychometric func

The insular auditory field receives input from the lemniscal subdivision of the auditory thalamus in mice.

Here we studied the auditory thalamic input to the insular cortex using mice as a model system. An insular auditory field (IAF) has recently been identified in mice. By using retrograde neuronal tracing, we identified auditory thalamic neurons projecting to the IAF, primary auditory cortex (AI), and anterior auditory field (AAF). After mapping the IAF, AAF, and AI by using optical imaging, we injected a distinct fluorescent tracer into each of the three fields at frequency-matched locations. Tracer injection into the IAF resulted in retrogradely labeled cells localized ventromedially in the le

Auditory and cognitive effects of aging on perception of environmental sounds in natural auditory scenes.

PURPOSE: Previously, Gygi and Shafiro (2011) found that when environmental sounds are semantically incongruent with the background scene (e.g., horse galloping in a restaurant), they can be identified more accurately by young normal-hearing listeners (YNH) than sounds congruent with the scene (e.g., horse galloping at a racetrack). This study investigated how age and high-frequency audibility affect this Incongruency Advantage (IA) effect. METHOD: In Experiments 1a and 1b, elderly listeners ( N = 18 for 1a; N = 10 for 1b) with age-appropriate hearing (EAH) were tested on target sounds and aud

Shape-specific activation of occipital cortex in an early blind echolocation expert.

We have previously reported that an early-blind echolocating individual (EB) showed robust occipital activation when he identified distant, silent objects based on echoes from his tongue clicks (Thaler, Arnott, & Goodale, 2011). In the present study we investigated the extent to which echolocation activation in EB's occipital cortex reflected general echolocation processing per se versus feature-specific processing. In the first experiment, echolocation audio sessions were captured with in-ear microphones in an anechoic chamber or hallway alcove as EB produced tongue clicks in front of a conca

Noise-induced annoyance from transportation noise: short-term responses to a single noise source in a laboratory.

An experimental study was performed to compare the annoyances from civil-aircraft noise, military-aircraft noise, railway noise, and road-traffic noise. Two-way within-subjects designs were applied in this research. Fifty-two subjects, who were naive listeners, were given various stimuli with varying levels through a headphone in an anechoic chamber. Regardless of the frequency weighting network, even under the same average energy level, civil-aircraft noise was the most annoying, followed by military-aircraft noise, railway noise, and road-traffic noise. In particular, penalties in the time-a

Sensitivity to interaural time difference with bilateral cochlear implants: Development over time and effect of interaural electrode spacing.

Sensitivity to interaural time difference (ITD) in constant-amplitude pulse trains was measured in four sequentially implanted bilateral cochlear implant (CI) subjects. The sensitivity measurements were made as a function of time beginning directly after the second ear was implanted, continued for periods of months before subjects began wearing bilateral sound processors, and extended for months while the subjects used bilateral sound processors in day-to-day listening. Measurements were also made as a function of the relative position of the left/right electrodes. The two subjects with the sh
