Adrenal venous sampling using Dyna-CT--a practical guide.

Primary hyperaldosteronism due to aldosterone secreting adrenal adenomas is an important and potentially curable cause for hypertension. The differentiation between unilateral or bilateral adrenal adenomas is crucial, as unilateral adenomas can easily be cured by surgery whereas bilateral adenomas have to be treated conservatively. Exact diagnosis can be made when unilateral or bilateral hormone production is proven with adrenal vein sampling. We present an effective step-by-step technique how to perform an adrenal vein sampling with a special emphasis on how to reliably catheterize the right

Newly designed sheaths for gastroduodenal intervention: an experimental study in a phantom and dogs.

OBJECTIVE: To evaluate the usefulness of newly designed sheaths for gastroduodenal intervention in a gastric phantom and dogs. MATERIALS AND METHODS: A regular sheath was made using a polytetrafluoroethylene tube (4 mm in diameter, 90 cm long) with a bent tip (4 cm long, 100 degree angle). For the supported type of sheath, a 5 Fr catheter was attached to a regular sheath to act as a side lumen. To evaluate their supportability, we measured the distance of movement of the sheath's tip within a silicone gastric phantom for three types of sheath, the regular type, supported type, and supported t

[Interventonal radiology in the splanchnic arteries].

We present a review of interventional radiology in the splanchnic arteries, focusing on what has been achieved and the remaining challenges. Embolization is currently the treatment of choice for hemorrhagic events due to anastomotic leakage after intestinal anastomosis, although occasional association with liver necrosis poses a hazardous problem yet to be overcome. For splanchnic artery aneurysms, we should recognize that we are not, in reality, well informed about their natural history. The indications for the embolization of aneurysms is limited depending on the morphology of the aneurysm a

Transcatheter obliteration of gastric varices: Part 2. Strategy and techniques based on hemodynamic features.

Balloon-occluded retrograde transvenous obliteration (BRTO) has become the treatment of choice for gastric varices at many institutions in Japan. However, in some cases that involve complex types of afferent or draining veins, the use of standard BRTO for the treatment of gastric varices may be associated with several difficulties that can lead to unfavorable results. In such cases, additional techniques are required for successful treatment. These techniques include stepwise injection of the sclerosing agent, selective injection of the agent via a microcatheter, coil embolization of the affer

Arthroscopically assisted retrograde fixation of patellar osteochondritis dissecans using fluoroscopic guidance: a case report and technical note.

Osteochondritis dissecans of the patella is a relatively rare condition. Treatment recommendations vary significantly from nonoperative treatment to surgical intervention. Surgical management varies from excision of the lesion to a variety of methods of stabilization. We present a case of patellar osteochondritis dissecans successfully treated with arthroscopically assisted retrograde fixation using fluoroscopic guidance. This surgical technique, which we describe in detail, avoids articular cartilage penetration and the need for an arthrotomy and allows for compression of the lesion with the

Endoscopic transvaginal approach to the rabbit uterine cervix: improvement in the technique of selective tubal catheterization.

RATIONALE AND OBJECTIVES: The authors compared two methods of selective fallopian tube catheterization in female rabbits. METHODS: Technical success rates and procedure times in two groups of animals were compared. Group 1 consisted of 20 female rabbits catheterized with a fluoroscopic technique. Group 2 consisted of 55 rabbits catheterized with a combination of endoscopic and fluoroscopic guidance. RESULTS: The technique used in group 2 allowed a statistically significant reduction in the time required for cervical catheterization and the total procedural time (P < .01). There was a paralle

[Percutaneous radiologic techniques in therapy of choledocholithiasis].

Interventional radiology offers a variety of techniques for percutaneous manipulation of intrahepatic and extrahepatic bile duct stones via the T-tube be tract and the transhepatic route, including extraction, fragmentation, transpapillary expulsion of stones into the duodenum, and chemolysis. These techniques can be combined with extracorporeal lithotripsy, balloon dilatation and electroincision of stenoses and implantation of metallic stents. Removal of stones via the T-tube tract is just as successful as endoscopic stone manipulation and leaves the papilla of Vater intact. Percutaneous tran

Silicone implant for chin augmentation mimicking a low-grade liposarcoma.

The silicone implant has been widely used in facial aesthetic surgery for chin augmentation. A case of a 71-year-old woman with a lesion in the anterior mandibular region detected by radiographies and suggestive of benign soft tissue tumor is presented. The patient denied previous surgical intervention in the mental area. A biopsy was performed and initially interpreted as low-grade liposarcoma. After an immunohistochemistry study showing CD-68-positive cells and complementary information regarding a silicone implant for chin augmentation, the definitive diagnosis was silicone granuloma. This

A Young Man with Hematemesis.

Primary aortoenteric fistula (AEF) is a rare but fatal cause of upper gastrointestinal bleed. A 27-year-old previously healthy man presented with massive hematemesis, melena, severe anemia and hemodynamic instability. Point-of-care ultrasound and chest radiography suggested thoracic aortic pathology, and CT aortography confirmed a descending thoracic aneurysm with contained rupture and fistulous communication with the esophagus. This case highlights the need to consider rare vascular causes in massive hematemesis, where early imaging can facilitate rapid diagnosis and timely intervention.

Mesenteric volvulus in the dog: a retrospective study of 12 cases.

Mesenteric volvulus was diagnosed in 12 dogs over a nine-year period. Each case was presented with abdominal distension and shock. Haematochezia, which is frequently reported in association with mesenteric volvulus, was present in only two of the dogs. The diagnosis, which in all cases was based on radiography, was followed by immediate treatment for shock and surgical intervention whenever possible. In five of the 12 cases, the volvulus was treated successfully and these patients survived. This indicates that the prognosis for mesenteric volvulus might be better than is currently believed, an

Phrenic nerve injury after image-guided insertion of a tunnelled right internal jugular central venous catheter.

Central venous catheters (CVC) are now commonly inserted by radiologists. Although complications are infrequent, they must be avoided where possible and recognized when they occur. We present a 10-year-old boy who developed right hemidiaphragmatic paralysis, requiring surgical plication, following US-guided insertion of a tunnelled right internal jugular CVC. The needle trajectory for internal jugular puncture must be planned to avoid the phrenic nerve.

Population exposure to ionizing radiation from medical examinations in France.

This study was carried out to update data concerning both the nature and the frequency of X-ray diagnostic procedures and to reassess the associated per caput effective dose in France, given that the only nationwide survey was carried out over 15 years ago. Relevant data concerning examinations in conventional radiology, computed tomography, interventional radiology and nuclear medicine were obtained for the year 2002 from two main sources: the main health insurance records for private practices and the statistics of healthcare establishments on hospital activity. Doses associated with differe

Intracoronary Imaging to Guide PCI in Acute Coronary Syndromes: A Report From the BMC2 Registry.

BACKGROUND: Intracoronary imaging (ICI) to guide percutaneous coronary intervention (PCI) in acute coronary syndromes (ACS) carries a Class I recommendation in US guidelines. AIMS: This analysis evaluated the frequency of ICI use among patients with ACS. METHODS: Using data from a large statewide registry, all patients undergoing PCI between January 1, 2019, and December 31, 2024, were studied. The primary measure of interest for this analysis was the use of ICI to guide PCI, defined as the use of ICI after the decision was made to proceed with PCI. The frequency of ICI use among patients wi

Pitfalls in image guided tissue sampling in the head and neck.

Image-guided tissue sampling is becoming increasingly important for management of head and neck cancers. Ultrasound-guided fine-needle aspiration (UG-FNA) is safe, effective, and has many advantages compared with palpation-guided FNA and computed tomography-guided FNA. The technique of UG-FNA is highly operator and experience dependent; however, understanding the complex anatomy, disease processes, and patterns of nodal spread in the head and neck make this technique ideal for the neuroradiologist. Proper technique and recognition of pitfalls are critical to successful UG-FNA. Computed tomogra
