Microsurgical replacement of the right hepatic artery with the donor superior mesenteric artery in cadaveric donor pediatric liver transplantation.

PURPOSE: To present our experience of using the pediatric donor superior mesenteric artery (SMA) as the interpositioning vessel to reconstruct the hepatic artery in pediatric deceased donor liver transplantation. METHODS: Pediatric patients who received pediatric deceased donor livers from December 2019 to June 2021 were enrolled. RESULTS: A total of 43 pediatric recipients received pediatric deceased donor livers during the study period. Thirty-four (79.1%) pediatric donors had normal hepatic artery anatomy, while nine (20.9%) showed variant anatomies. The SMAs of the pediatric donors were

Transmission of Balamuthia mandrillaris through solid organ transplantation: utility of organ recipient serology to guide clinical management.

A liver, heart, iliac vessel and two kidneys were recovered from a 39-year-old man who died of traumatic head injury and were transplanted into five recipients. The liver recipient 18 days posttransplantation presented with headache, ataxia and fever, followed by rapid neurologic decline and death. Diagnosis of granulomatous amebic encephalitis was made on autopsy. Balamuthia mandrillaris infection was confirmed with immunohistochemical and polymerase chain reaction (PCR) assays. Donor and recipients' sera were tested for B. mandrillaris antibodies. Donor brain was negative for Balamuthia by i

Updates in Lower Extremity Reconstruction: Post Sarcoma.

Lower extremity (LE) reconstruction presents unique challenges due to complex wound environments, compromised vascularity, and high functional demands. Compared with other anatomic regions, free flap reconstruction in the LE demonstrates higher complication and failure rates. Early microsurgical intervention improves outcomes, though negative pressure wound therapy can bridge to delayed coverage. In oncologic patients, radiation and chemotherapy impair tissue quality and recipient vessels, necessitating meticulous planning and vessel selection outside the zone of injury. Anatomic location infl…

How I do it: internal maxillary artery to middle cerebral artery bypass to manage giant thrombosed internal carotid artery aneurysm.

BACKGROUND: Internal maxillary artery (IMA) bypass has become popularized due to its medium-to-high blood flow, short graft length, and well-matched arterial caliber between donor and recipient vessels. METHOD: We described an open surgery of a NEW "workhorse," the IMA bypass, to treat a giant, thrombosed cerebral aneurysm. The extracranial middle infratemporal fossa (EMITF) approach was used to unveil the pterygoid segment of the IMA for cerebral revascularization. CONCLUSION: Although this technique is technically challenging, the variations in IMA can be effectively identified and suffici…

Free Flaps in Lower Extremity Reconstruction.

Free tissue transfer to the lower extremity for limb salvage remains challenging. A comprehensive approach includes patient selection, flap selection, selection of the recipient vessels, flap dissection, flap preparation, microvascular anastomosis, flap inset, immediate postoperative care, intermediate postoperative care, and further follow-up care. Each step in this comprehensive approach has its unique considerations and should be executed equally to ensure an optimal outcome. Once acquired, some clinical experience along with adequate microsurgical skill, good surgical judgment, well instru…

Partial esophageal transplantation is possible as part of a multivisceral graft.

We report a case of a 9-year-old female with inflammatory myofibroblastic tumor (IMT), which involved the upper retroperitoneum, visceral vessels, stomach and distal esophagus. Complete resection of the tumor required a multivisceral (MV) transplant. Due to tumor involvement, resection of the distal third of recipient esophagus was necessary. Gastrointestinal continuity was subsequently established via esophagoesophagostomy to donor esophagus en bloc with a standard MV graft. After 1.9 years of follow-up, the patient has no symptoms of dysphagia or reflux. This case illustrates the feasibility

Divergent vascular courses in the submental lymph node flap revealed by cadaveric study.

Vascularized submental lymph node (VSLN) flap transfer is a promising treatment for lymphedema, yet its venous anatomy remains underexplored. This study investigated variations of the submental artery and vein and the courses of hilar arteries and veins connecting lymph nodes in 40 VSLN flaps from 20 Thai cadavers. Colored latex was injected in both sides of the internal carotid arteries, external carotid arteries, internal jugular veins, and external jugular veins of each cadaver. The mandibles were removed en-bloc and then sliced coronally for investigation under stereomicroscopy. The submen

Murine Model of Venous Bypass Grafting to Study Determinants of Graft Patency.

The investigation of patency and remodeling in venous grafts requires a reproducible animal model that enables the study of the underlying cellular and molecular processes. Here, we describe a simplified mouse model for venous bypass grafts, in which the inferior thoracic vena cava (IVC) of a donor mouse is interposed into the common carotid artery of a recipient mouse using a cuff technique. The procedure is divided into four basic protocols, which includes the harvesting of the inferior vena cava from donor mice (Basic Protocol 1), the preparation of the common carotid artery (Basic Protocol

Two-step hepatic artery reconstruction for a hepatic artery lacking in length for the use of a microclamp in living donor liver transplantation.

INTRODUCTION: We describe successful two-step hepatic artery reconstruction in a patient whose graft site hepatic artery was too short for the use of a microclamp in living donor liver transplantation. PRESENTATION OF CASE: A 57-year-old woman was diagnosed as having hepatitis C and liver cirrhosis. Her 26-year-old son was the living liver donor. The living donor underwent right lobectomy. The dissected graft hepatic artery was too short for the use of a microclamp. The recipient right hepatic artery was cut and used as an arterial graft. The graft right hepatic artery was sutured to the righ

Pancreatic islet blood flow after syngeneic pancreaticoduodenal transplantation in rats. Differences between the blood perfusion of the native and the transplanted gland.

Inbred male Sprague-Dawley rats were transplanted with syngeneic pancreaticoduodenal grafts. Diabetes was induced in some of the recipient animals by an i.v. injection of streptozotocin one week before transplantation, while the remaining control rats were untreated, and thus had both an intact native and a transplanted pancreas. The left kidney of the recipient was removed and its blood vessels were used for the graft vascular anastomosis. The graft duodenum was sutured end-to-side to the ileum of the recipient. Two weeks after transplantation the rates of blood flow through both the native a

Computed Tomographic Angiography (CTA)-guided precision reconstruction in complex lower leg wounds.

BACKGROUND: Reconstruction of complex lower leg wounds presents significant challenges due to extensive soft tissue damage, vascular variations, and potential neurovascular injuries. While flap transfer, particularly free flaps, remains the preferred method for reconstruction, the lack of precise preoperative vascular information often complicates surgical planning. This study introduces a Computed Tomographic Angiography (CTA)-guided approach for precision reconstruction to address these challenges. PURPOSE: This study aims to elucidate the critical role of precise preoperative assessment of

Hybrid operating rooms for the treatment of complex middle cerebral artery aneurysms: A technical note.

BACKGROUND: The recent emergence of hybrid operating rooms (H-OR) broadens treatment options for complex intracranial aneurysms. Combining endovascular options enabled by the H-OR with refined microsurgical techniques could provide less arduous treatment alternatives for large, distal, and fusiform MCA aneurysms. RESEARCH QUESTION: To describe the technical nuances of a hybrid treatment consisting of microcatheter-guided flash fluorescence to enable revascularization bypass and endovascular parent vessel occlusion (PVO) in three different cases of large, distal and fusiform MCA aneurysms. Viable testis after retroperitoneal mass cord ligation in internal ring of inguinal canal in 15 kidney recipients: five years of experience.

INTRODUCTION: Sometimes in spermatic cord handling procedures like varicocelectomy, hernioraphy, and vasectomy, there is concern about perfusion of the testis. Herein we have presented our experience with retroperitoneal mass ligation of the cord in kidney recipients. Between 2001 and 2006, we performed mass ligation of the spermatic cord, including vas deferens and all spermatic vessels, in 15 kidney recipients (older than 55 years) who gave informed consent. During retroperitoneal preparation of the Iliac fossa for allografting, we performed this maneuver next to the internal ring of the ing

Pathology and etiology of chronic rejection of the heart.

As there are other lesions in the cardiac allografts of long-term survivors, it is best to refer to the vascular lesion as "graft vascular disease". Graft vascular disease may be evident as early as 3 months after cardiac transplantation and may cause death in the recipient as late as 22 years post-transplant. The disease affects infants, children, and adults as well as the recipients of combined heart-lung transplants. It is one of the most discouraging aspects of long-term survival in combined heart-lung and heart transplantation, accounting for 18% of the deaths among the 201 cardiac transp

Comment on "Effects of Perioperative Management on Short-Term Renal Transplant Outcome".

In a recent retrospective cohort study by Wang et al., the perioperative hemodynamic variables were found to be associated with short-term graft function in pediatric renal transplant recipients, especially mean arterial pressure (MAP) before and after the graft reperfusion. However, a set age-based MAP threshold does not necessarily reflect the individual renal allograft perfusion needs. The time spent below a target MAP may not be equivalent between patients, as the MAP threshold may change depending on the recipient and donor blood vessel characteristics, and the baseline hemodynamics, as w

Supercharged reverse pedicle anterolateral thigh flap in reconstruction of a massive defect: A case report.

Secondary reconstruction of lower extremity defects using local tissues is demanding and fraught with potential complications. Reconstructive efforts may be challenged by pre-existing scarring, paucity of recipient vessels, and patient co-morbidities limiting tolerance for prolonged and extensive surgery. We present a case of an 81-year-old male with a recurrent malignant melanoma invading the proximal and middle third of the tibia, who previously underwent reconstruction with the medial gastrocnemius muscle and a skin graft. After wide local re-excision and tibia fixation, a 12 cm x 28 cm rev…
