[Physiology of ventilatory regulation during sleep].

During light slow-wave sleep, ventilation is principally regulated by automatic metabolic control system. An instability in the respiratory control may be the predominant disturbance leading to very irregular or periodic breathing. During deep sleep, ventilation is progressively more stable. During REM sleep, automatic regulation is abolished and ventilation is particularly dependent on the compartmental control system. The reduction in airways and respiratory muscles tone favors the occurrence of obstructive apneas. The elevation in arousal threshold leads prolongation of the obstructive even

Breathing-based meditation decreases posttraumatic stress disorder symptoms in U.S. military veterans: a randomized controlled longitudinal study.

Given the limited success of conventional treatments for veterans with posttraumatic stress disorder (PTSD), investigations of alternative approaches are warranted. We examined the effects of a breathing-based meditation intervention, Sudarshan Kriya yoga, on PTSD outcome variables in U.S. male veterans of the Iraq or Afghanistan war. We randomly assigned 21 veterans to an active (n = 11) or waitlist control (n = 10) group. Laboratory measures of eye-blink startle and respiration rate were obtained before and after the intervention, as were self-report symptom measures; the latter were also ob

Idiopathic bilateral diaphragmatic paresis.

We report the case of a patient with severe dyspnea upon reclining. Lung disease, neuromuscular disorders and heart disease were ruled out. However, during the course of the investigation, bilateral diaphragmatic paresis was discovered. A key sign leading to the diagnosis was evidence of paradoxical respiration in the dorsal decubitus position. When the patient was moved from the orthostatic position to the dorsal decubitus position, oxygenation and forced vital capacity worsened. The orthostatic fluoroscopy was normal. Maximal inspiratory pressure was severely reduced. The responses to transc

Sleep apnea syndromes (SAS) of specific etiology: review and incidence from a sleep laboratory.

Sleep apnea syndrome (SAS) results from modification in the control of respiration and of upper airway caliber during sleep. Although there is some overlap between central (CSAS) and obstructive (OSAS) sleep apnea syndromes, each syndrome has specific pathological associations. The first part of this review concerns the pathophysiology of OSAS, including periodic breathing and upper airway collapse. In the second part, each specific etiology is examined, and the respective contribution of anatomic narrowing and neuromuscular dysfunction of the upper airway is mentioned. Our experience with abo

Effects of awake tidal breathing, swallowing, nasal breathing, oral breathing and the Müller and Valsalva maneuvers on the dimensions of the upper airway. Evaluation by ultrafast computerized tomography.

Determinations of the size and dynamics of the upper airway during respiration are important in individuals with sleep-related breathing disorders. Ultrafast computerized tomography can acquire eight 8-mm axial-slice thicknesses of the upper airway in 224 ms. If this sequence is acquired every 0.7 second over an entire respiratory cycle and played back in movie mode, the dynamic changes in the airway's size can be evaluated and measured. This report defines the size of the upper airway during normal tidal breathing and describes the changes that occur with swallowing, isolated nasal breathing,

Measurement of oxygen consumption rate in mouse aortic tissue.

Thoracic aortic aneurysm and dissection (TAD) is a life-threatening vascular disorder, and smooth muscle cell mitochondrial dysfunction leads to cell death, contributing to TAD. Accurate measurements of metabolic processes are essential for understanding cellular homeostasis in both healthy and diseased states. While assays for evaluating mitochondrial respiration have been well established for cultured cells and isolated mitochondria, no optimized application has been developed for aortic tissue. In this study, we generate an optimized protocol using the Agilent Seahorse XFe24 analyzer to mea

Automated detection of obstructive sleep apnoea at different time scales using the electrocardiogram.

An automated classification algorithm is presented which processes short-duration epochs of surface electrocardiogram data derived from polysomnography studies, and determines whether an epoch is from a period of sleep disordered respiration (SDR) or normal respiration (NR). The epoch lengths considered were 15, 30, 45, 60, 75, and 90 s. Epochs were labeled as 'NR' or 'SDR' by a human expert, based on standard polysomnography interpretation rules. The automated classification algorithm was trained and tested on a database of 70 overnight ECG recordings from subjects with and without obstructiv

Respiratory disorders of Parkinson's disease.

Parkinson's disease (PD) is characterized by the progressive loss of dopaminergic neurons in the substantia nigra, mainly affecting people over 60 yr of age. Patients develop both classic symptoms (tremors, muscle rigidity, bradykinesia, and postural instability) and nonclassical symptoms (orthostatic hypotension, neuropsychiatric deficiency, sleep disturbances, and respiratory disorders). Thus, patients with PD can have a significantly impaired quality of life, especially when they do not have multimodality therapeutic follow-up. The respiratory alterations associated with this syndrome are t

Pathophysiology of Neuromuscular Respiratory Diseases.

Gas exchange between the atmosphere and the human body depends on the lungs and the function of the respiratory pump. The respiratory pump consists of the respiratory control center located in the brain, bony rib cage, diaphragm, and intercostal, accessory, and abdominal muscles. A variety of muscles serve to fine-tune adjustments of ventilation to metabolic demands. Appropriate evaluation and interventions can prevent respiratory complications and prolong life in individuals with neuromuscular diseases. This article discusses normal function of the respiratory pump, general pathophysiologic i
