[Measuring contrast sensitivity using visual acuity tests in retinal and optic nerve diseases].

The luminance contrast needed to discern various test types was measured with monochromatic and achromatic light to detect discrete function … ding on the color of the test light and the diagnosis. The amount of contrast needed is greatest in retinal diseases, and it is therefore possible to a certain extent to distinguish these from diseases of the optic nerve.

Retinal phenotyping of variants of Alzheimer's disease using ultra-widefield retinal images.

BACKGROUND: Posterior cortical atrophy (PCA) is the most common atypical variant of Alzheimer's disease (AD). Changes associated with PCA in the brain affect the visual cortex, but little is known about retinal changes in PCA. In this study, we explored retinal phenotypic variations in typical AD (tAD) and PCA. METHODS: Retinal phenotyping was carried out on ultra-widefield (UWF) images of 69 control, 24 tAD, and 25 PCA participants. RESULTS: Individuals with tAD (odds ratio [OR] = 2.76 [confidence interval (CI):1.24 to 6.10], P = .012) and PCA (OR = 3.40 [CI:1.25 to 9.22], P = .016) were mo

Case Report: Peripheral Retinal Ischemia and Retinal Neovascularization in von Willebrand Disease.

SIGNIFICANCE: Although von Willebrand disease is the most common inherited bleeding disorder, there are only a few published reports of ocular complications. To our knowledge, this is the first case of peripheral retinal ischemia and retinal neovascularization in a patient with von Willebrand disease. PURPOSE: This study aimed to demonstrate the value of multispecialty care when exploring a diagnosis for bilateral retinopathy. CASE REPORT: A 55-year-old African American woman presented with peripheral retinal hemorrhages on routine examination. She was asymptomatic and did not have any perso

Visual Deficits in Diabetic Retinal Disease.

PURPOSE: To demonstrate the current level of visual deficits due to DRD. METHODS: This cross-sectional study used the Sight Outcomes Research Collaborative (SOURCE) repository to identify all patients with DRD, diabetic macular edema (DME), and proliferative diabetic retinopathy (PDR). Cohorts for each disease state were created using ICD10 codes based on the first date of diagnosis. Patients had their vision assessed at the index date, then, when available, at years 1,2,3, and 5. The primary outcome was the percentage of eyes with vision deficits to the level of 20/70 and 20/200 or worse. R

Cone Opsins and Inherited Retinal Disease.

Opsins are the light receptors in retinal rod and cone photoreceptor cells that initiate vision in response to a light stimulus. Rhodopsin is the opsin in rods, and the influence of mutations that disrupt its structure and function has been characterized in detail. Less is known about cone opsins, the opsins in cones, and their gene arrays. Cones are responsible for color vision and high visual acuity and operate under most lighting conditions. There are up to three types of cone opsins (L-, M-, and S-opsin) in most vertebrates, each defined by their distinct spectral sensitivities. Disruption

Diabetic retinal disease: A sensory neuropathy.

Diabetic retinal disease (DRD), diabetic neuropathy (DN), and nephropathy are increasing in worldwide prevalence, but current methods to identify early abnormalities and predict progression of DRD are limited. DN and DRD share many common risk factors and pathophysiologic features, including altered amino acid, lipid, and carbohydrate metabolism. Of these, we propose that defective insulin receptor signaling, and consequent dysregulated energetic and biosynthetic processes are common initiators of DRD and DN. Of relevance, neurons have high basal metabolic requirements that render them vulnera

Stem cell therapy in retinal disease.

Stem cell therapy presents a new solution to cure degenerative diseases of the retina, which normally have limited ability to regenerate after injury. Stem cells are cells capable of self-renewal and differentiation into more specialized cells, and can be derived from both embryonic and adult sources. This definition encompasses a heterogeneous group of cells possessing varied potency and characteristics. Current attempts in applying stem cell technology to the retina have primarily focused on regenerating the retinal pigment epithelium (RPE) and light-sensing photoreceptor cells. They have no

Potential Retinal Biomarkers in Alzheimer's Disease.

Alzheimer's disease (AD) represents a major diagnostic challenge, as early detection is crucial for effective intervention. This review examines the diagnostic challenges facing current AD evaluations and explores the emerging field of retinal alterations as early indicators. Recognizing the potential of the retina as a noninvasive window to the brain, we emphasize the importance of identifying retinal biomarkers in the early stages of AD. However, the examination of AD is not without its challenges, as the similarities shared with other retinal diseases introduce complexity in the search for …

Ribbon Synapses and Retinal Disease: Review.

Synaptic ribbons are presynaptic protein complexes that are believed to be important for the transmission of sensory information in the visual system. Ribbons are selectively associated with those synapses where graded changes in membrane potential drive continuous neurotransmitter release. Defective synaptic transmission can arise as a result of the mutagenesis of a single ribbon component. Visual diseases that stem from malfunctions in the presynaptic molecular machinery of ribbon synapses in the retina are rare. In this review, we provide an overview of synaptopathies that give rise to reti

The Role of Hsp90 in Retinal Proteostasis and Disease.

Photoreceptors are sensitive neuronal cells with great metabolic demands, as they are responsible for carrying out visual phototransduction, a complex and multistep process that requires the exquisite coordination of a large number of signalling protein components. Therefore, the viability of photoreceptors relies on mechanisms that ensure a well-balanced and functional proteome that maintains the protein homeostasis, or proteostasis, of the cell. This review explores how the different isoforms of Hsp90, including the cytosolic Hsp90α/β, the mitochondrial TRAP1, and the ER-specific GRP94, are

Secondary Glaucoma in the Context of Retinal Disease.

A variety of retinal diseases can lead to the development of glaucoma. The most common type of these secondary glaucomas is neovascular glaucoma (NVG), which constitutes the main subject of this review. NVG is a severe condition with a poor prognosis. Treatment becomes increasingly challenging as the disease progresses. Thus emphasis is put on early diagnosis and therapy adapted to the disease stage. The review also covers other less frequent secondary glaucomas, such as glaucomas due to intraocular tumours or associated with retinal detachment (Schwartz-Matsuo syndrome) as well as late onset …

Functional evaluation in inherited retinal disease.

Functional assessments are a fundamental part of the clinical evaluation of patients with inherited retinal diseases (IRDs). Their importance and impact have become increasingly notable, given the significant breadth and number of clinical trials and studies investigating multiple avenues of intervention across a wide range of IRDs, including gene, pharmacological and cellular therapies. Moreover, the fact that many clinical trials are reporting improvements in vision, rather than the previously anticipated structural stability/slowing of degeneration, makes functional evaluation of primary re…

Diabetic and Retinal Vascular Eye Disease.

The retinal vasculature is the only neurovascular system directly visible to the human eye, easily evaluated by fundoscopy and many imaging modalities. This window allows physicians to diagnose and treat retinal pathologies and detect systemic diseases including diabetes, hypertension, hypercoagulable/hyperviscosity syndromes, and vasculitis. Diabetic retinopathy is the most common retinal vascular disease, followed by retinal vein and artery occlusion. Patients with these conditions require medical optimization to prevent further damage to the eyes and to the other organs. Both the internists

Retinal miRNA Functions in Health and Disease.

The health and function of our visual system relies on accurate gene expression. While many genetic mutations are associated with visual impairment and blindness, we are just beginning to understand the complex interplay between gene regulation and retinal pathologies. MicroRNAs (miRNAs), a class of non-coding RNAs, are important regulators of gene expression that exert their function through post-transcriptional silencing of complementary mRNA targets. According to recent transcriptomic analyses, certain miRNA species are expressed in all retinal cell types, while others are cell type-specifi

Telemedicine Applications in Pediatric Retinal Disease.

Teleophthalmology is a developing field that presents diverse opportunities. One of its most successful applications to date has been in pediatric retinal disease, particularly in screening for retinopathy of prematurity (ROP). Many studies have shown that using telemedicine for ROP screening allows a remote ophthalmologist to identify abnormal findings and implement early interventions. Here, we review the literature on uses of telemedicine in pediatric retinal disease and consider future applications.
