Regrowth of splenic tissue after splenectomy for trauma and splenectomy for idiopathic thrombocytopenia purpura have been reported. However, rupture of splenic tissue, either spontaneous or traumatic, that requires a second surgical intervention for hemoperitoneum caused by a ruptured splenic nodule or splenosis has rarely been reported. We report the case of a 43-year-old man in whom hemoperitoneum developed 25 years after he underwent an open splenectomy, after a motor vehicle accident, that required removal of a recurrent ruptured splenotic nodule.

The extensor mechanism of the knee consists of the quadriceps muscle and tendon, the patella, the patellar tendon, and the tibial tubercle. Disruption of any of these can lead to an extensor mechanism rupture and render an otherwise perfectly good total knee replacement (TKR) useless. Quadriceps tears associated with TKR are difficult to treat and associated with a poorer prognosis than in the native knee. Transosseous sutures tied over the distal pole of the patella are recommended. Patella fractures may heal in continuity or may involve a disruption of the extensor mechanism. The blood suppl…

Globe rupture is one of the most severe open globe injuries, permanently impairing visual acuity or leading to blindness. The risk of globe rupture is increased after previous intraocular surgery (27-fold), in myopia, older age, females, and after sudden falls. The differentiation between an occult globe rupture and severe ocular contusion may be complicated by pronounced subconjunctival hemorrhage with conjunctival swelling. In case of doubt, a rupture of the eyeball should be ruled out after a severe blunt ocular trauma. Limbal and scleral exploration after 360 degrees peritomy leads to the …

OBJECTIVE: To study the incidence, risk factors, clinical presentation, maternal morbidity and mortality, and perinatal mortality in cases with ruptured gravid uterus.
METHODS: All cases with diagnosis of uterine rupture at Saudi Hospital at Hajjah, Yemen during 5-years period from April 1999 to March 2004 were studied. Detailed informations were obtained by reviewing hospital records.
RESULTS: Out of the total number of deliveries during the period (N=5547), 60 cases had uterine rupture giving a hospital incidence of one in 92 deliveries (1.1%). Forty-three cases (71.7%) with unscarred uter…

We report two cases of tendon rupture associated with ciprofloxacin. One patient had a complete rupture of an Achilles tendon 6 months after taking the medication. The other case involved a partial rupture of the subscapularis tendon. Both ruptures occurred with minimal mechanical stress on the tendons, suggesting that the fluoroquinolone increased the susceptibility to rupture. We also review the literature describing the association between fluoroquinolones and tendon rupture and discuss the mechanisms explaining the heightened risk of tendon rupture associated with these drugs.

Tracheobronchial ruptures in children after blunt trauma are rare, with an incidence at our clinic of 0.17% of juvenile thoracic injuries in the years 1985 to 1994. The symptoms are variable, and these injuries frequently occur in conjunction with other thoracic injuries, leading to delays in diagnosis. Diagnosis, conservative and operative treatment, and late complications are discussed by presenting four different cases of children with lesions of the tracheobronchial tree after blunt trauma.

There are many potential reasons for an acute abdomen. Very rare is a rupture of a cystadenoma of the pancreas. We describe such a case of an 85-year old woman, who was treated with an internal drainage of the cyst with a Roux-Y-loop, as at the time of the emergency operation the histological diagnosis was not known. As for the age of the patient and her general condition a secondary procedure with total resection of the mucinous cyst was not carried out. The post-operative course was uneventful up to three months, when the patient died in a geriatric home due to a antibiotic-resistant uroseps…

Common sports, involving raising the arms above the head, i.e., throwing, racquet games and swimming, often result in rotator cuff tendinitis. During the throwing motion, the humeral head and its overlying biceps tendon and rotator cuff must pass rapidly under the coraco-acromial arch. Damage to these structures can occur by several mechanism. First, an increase in the size of the structures passing underneath the arch may lead to impingement. This can occur either by way of hypertrophy of the musculotendinous cuff or by way of inflammation of the cuff. Second, a decreased space available unde

A series of 57 esophageal perforations from the period 1963-1982 is reported. They comprised 42 iatrogenic, instrumental perforations and 15 non-iatrogenic, mainly spontaneous and postemetic ruptures. The clinical manifestations were mostly pain, fever, subcutaneous and mediastinal emphysema, pleural effusion and pneumothorax. Esophageal leak was demonstrated in 73% of contrast studies. The overall mortality rate was 25%. It was 21% in cases with primary suture closure and 50% when treatment consisted of drainage. The mortality rate was 19% when treatment was begun within 24 hours and 35% when…

The surgeon who is faced with the management of a neonate with a prenatally ruptured omphalocele or a gastroschisis has a challenging problem. These anomalles are some of the most serious, lifethreatening neonatal surgical emergencies.

A case of peliosis of the spleen without apparent involvement of the liver or other organs is presented. The peliosis was discovered following rupture of the spleen after minimal trauma and was successfully treated by surgery. None of the common associations of peliosis was present (i.e., tuberculosis or intake of steroid compounds). Drugs addiction and chronic alcoholism were the only coexisting debilitating conditions exhibited by this patient.

Four cases of fatal diaphragmatic rupture in the horse are described. In 2 cases there was no history of injury but the other 2 animals had sustained recent thoracic trauma. Three of the horses had signs of colic and bowel obstruction complicated, in 2 cases, by respiratory embarrassment; the fourth died rapidly, presumably of shock. Previously published cases are reviewed and the causes, clinical effects, diagnosis and treatment of the disorder are discussed.

Three hundred and thirty-five patients with ruptured uterus were reviewed. The incidence of this condition is increasing, probably owing to poor antenatal care and lack of facilities. The most common site of rupture in the previously unscarred uterus was found to be longitudinal on the lateral aspects of the lower and upper segments. When the ruptures were in these areas, total hysterectomy proved to be best. Repair of the rupture should only be considered when the tear is simple, transverse in the lower segment, and in the absence of infection.
