Scarred scalp reconstruction with a rectangular expander.

Scalp reconstruction is challenging because the scalp is inelastic, stiff, and has hair follicles. Tissue expansion offers aesthetically pleasing outcomes with minimal donor-site morbidity. However, in a scarred scalp, the extent of possible dissection for the expander insertion may be limited and surgeons must make use of the limited scalp tissue. We successfully reconstructed scarred scalps using rectangular expanders. This report presents two cases: a 4× 3 cm chronic defect with widespread scarring and osteomyelitis and an 11× 7.5 cm scar tissue following a skin graft. Tissue expanders were…

Pseudomycetoma of the scalp caused by Microsporum canis.

Pseudomycetoma is an extremely rare deep mycosis, caused by dermatophytic fungi that penetrate the tissue from infected follicles of tinea capitis. Both clinically and histopathology are similar to eumycetoma, being distinguished through the isolation of the fungus, which in the case of pseudomycetoma can be Microsporum spp. or Trichophyton spp. genre. We present a 24-year-old man with an exuberant tumor in the occipital region with fistula, whose histopathological examination evidenced grains composed of hyaline hyphae and the culture for fungi isolated the agent Microsporum canis. Combined t

An Intracranial Tuberculoma with Discharging Scalp Sinus.

Our report highlights a case of intracranial tuberculoma in an elderly woman with chronic discharging sinus. A 77-year-old woman had a mass lesion extending to the scalp through a chronic discharging sinus for 2 years with an intermittent, non-radiating, dull, low-grade headache. Based on our survey of central nervous system tuberculosis, this case is a rare event of documented intracranial tuberculoma with concomitant discharge of the scalp sinus similar to the periodic geyser eruptions in developing countries.

Scalp microbiota alterations in children with pediculosis.

Pediculosis is a disease caused by the insect Pediculus humanus capitis that mainly occurs in childhood. A comparative study was carried out evaluating groups of schoolchildren with (group A) and without pediculosis (group B) to analyse the characteristics of the scalp microbiota. Samples were collected by swab using Stuart transport medium and incubate in Sabouraud dextrose agar with tetracycline to analyse the fungal microbiota and in blood agar to assess the bacterial microbiota. The isolates identity was confirmed by sequencing of the 16S and 18S regions of the ribosomal DNA gene for bacte…

Steatocystoma Simplex on the Scalp: A Case Report.

Steatocystoma is a benign adnexal tumor arising from the pilosebaceous duct junction. It can be classified into two groups (steatocystoma simplex and steatocystoma multiplex). Steatocystoma simplex, which presents as a solitary, non-herited very rare lesion in adult. It occurs most commonly in the axillae, in the sternal region and on the arms. Scalp is an extremely rare location. We report a case of 49-years-old male admitted in Department of Surgery, Dhaka Medical College & Hospital, Dhaka, Bangladesh on February 2017 presented with slowly progressing painless, soft, yellowish, single smooth…

Pemphigus Vegetans Confined to the Face and Scalp.

A 58-year-old man presented to our department with multiple chronic vegetant plaques on the face and scalp that had been present for 4 months. The plaques were well-defined hypertrophic areas, measuring between 3 cm × 3 cm and 3 cm × 4 cm, that were located mainly on the face symmetrically and on the forehead (Figure 1). The patient did not report itching or pain. There were also dry, crusty, well-defined plaques on the scalp. No other lesions were present on the skin or mucous membranes.

Scalp involvement in pemphigus: a prognostic marker.

INTRODUCTION: Scalp involvement in the course of pemphigus is observed in 16-60% of patients. AIM: To determine the prognostic significance of scalp involvement in pemphigus vulgaris and pemphigus foliaceus. MATERIAL AND METHODS: A total of 75 patients (46 with pemphigus vulgaris, 29 with pemphigus foliaceus) were included into this prospective study. The following clinical data were analyzed: Pemphigus Disease Area Index, time to complete clinical remission and duration of complete clinical remission. Indirect immunofluorescence and enzyme-linked immunosorbent assay were performed to monito…

A case of pulsatile scalp swelling in a child.

Arteriovenous fistulas (AVF) are an entity most commonly seen in the head and neck region. An AVF is a direct connection between the arterial feeders and the draining veins without intervening capillary beds (unlike an arteriovenous malformation in which a nidus intervenes between the arteries and veins). We describe a case of traumatic AVF in a child aged 9 years, who presented with a pulsatile scalp swelling following blunt trauma to the head. It turned out to be an AVF between the superficial temporal artery and vein. Correct diagnosis of an AVF involves Doppler examination with digital sub…

Case for diagnosis. A bluish nodule on the scalp.

Acrospiroma, also known as hidradenoma, is a rare cutaneous tumor that has several histological characteristics. As a consequence, a high index of suspicion is necessary for its diagnosis. Here we report a case that illustrates the importance of a good clinical-pathologic correlation in order to recognize this disease.

Trichoscopic Features of Linear Morphea on the Scalp.

Linear scleroderma en coup de sabre (LSCS) is the most common form of morphea on the scalp and its trichoscopic features have not been described yet. We report 2 adult women with LSCS with distinct dermoscopic findings. They both had an atrophic well-defined linear patch of alopecia on the frontotemporal scalp. Dermoscopy showed loss of follicular openings on a whitish skin surface; scattered black dots, broken hairs, and pili torti; and short thick linear and branching tortuous vessels on the periphery of the lesion. Pathology confirmed the diagnosis of LSCS in the 2 cases. This description o…
