BACKGROUND AND OBJECTIVES: The labyrinthine structures obstruct the surgical view of the deep petroclival region in the transpetrosal approach. Historically, labyrinthectomy and removal of all 3 semicircular canals, with resultant deafness, was used in patients with ipsilateral functional hearing deficits to improve access. The advent and systematization of superior and posterior semicircular canal removal (transcrusal approach) with good rates of hearing preservation has allowed a redefinition of the possibility of partial labyrinthectomy in patients without previous hearing deficits. The pre…

Lateral semicircular canal (LSCC) malformations represent one of the most common types of inner ear malformation. As many cases of LSCC malformations are asymptomatic, detailed vestibular functions in such cases remain unclear. We present a case with bilateral LSCC malformations for whom the function of each vestibular organ was evaluated by caloric testing, video Head Impulse Test (vHIT) and vestibular evoked myogenic potentials (VEMP). Caloric testing showed canal paresis of the left side, whereas vHIT showed bilateral normal semicircular canal function. The results of VEMP indicated left sa

In children with profound deafness, bilateral cochlear implant (CI) is an effective, established procedure. However, its safety on vestibular function has recently been debated. The goal of this study is to evaluate the long-term lateral semicircular canal high-frequency vestibulo-oculomotor reflex (LSC HF VOR) in children with CI by video head impulse testing (vHIT). This is a cross-sectional study assessing a cohort of children who received either a unilateral (12) or a bilateral (12) cochlear implant (CI), compared with a control group of 12 normal-hearing children. No significant LSC HF VO

In this case report, the air-conducted cervical vestibular evoked myogenic potentials (AC cVEMP) test was only sensitive for the left superior semicircular canal dehiscence (SCD), even though the contralateral SCD was of equal length (2.5 mm). Furthermore, a lysis of the processus lenticularis incudis caused a real conductive hearing loss in the left ear. A diminished left AC cVEMP was thus expected, but the opposite was shown (increased corrected amplitude, lowered detection threshold). The patient only experienced hearing loss, so middle ear surgery was performed to repair the lysis. The pos

Semicircular canal dehiscence is a bony abnormality in the otic capsule especially involving the superior semicircular canal. Since its identification in 1998, there is significant research regarding the pathology in the adult population. This condition generates a third window effect that is well-described in the literature. However, the entity is rare in the pediatric population with limited research. Difficulties encountered in children are obtaining a direct history that is essential for the diagnosis followed by neurovestibular tests that may be difficult to perform. This study presents o

IMPORTANCE: Nystagmus can occur spontaneously from multiple causes. Direction-changing positional nystagmus on the supine roll test is a characteristic clinical feature in horizontal semicircular canal benign paroxysmal positional vertigo. One of several mechanisms of spontaneous nystagmus is plugging of the otoconia, which has been described as a canalith jam.
OBSERVATIONS: We evaluated a 52-year-old woman with a history of geotropic variant of horizontal semicircular canal benign paroxysmal positional vertigo on the right side who had been treated with a modified Lempert maneuver 3 months e

We developed a modeling procedure using CT scans or MRI data for exploring the bony and lymphatic canals of vestibular patients. We submitted 445 patients with instability and spatial de-orientation to this procedure. Out of the 445 patients, 95 had scoliosis, some of them, because malformations were suspected also had CT-scan modeling and functional tests. We focused on a never described, abnormal connection between the lymphatic lateral and posterior canal (LPCC) with a frequency of 67/445 (15%). In the scoliosis subgroup, the frequency was 52/95 (55%). Three scoliotic patients had CT scans.

Vibration-induced nystagmus, as clinical sign, was recently introduced in outpatient clinical practice for the study and evaluation of otoneurological patients. This response, which can only be evoked by bone conducted vibratory stimulation in the mastoid region or at the location on the forehead in the midline at the hairline, was essentially designed for patients with persistent unilateral vestibular deficit and was interpreted as the result of excitatory functional activity of the vestibular system on the non-affected side. Vibratory stimulation is, in fact, considered to reach both systems

A consecutive series of 51 patients (34 females and 17 males) with posterior semicircular canal benign positional paroxysmal vertigo (BPPV) were treated with the modified Epley particle positioning manoeuvre (PRM). Follow-up data were available on all 51 patients. After one manoeuvre, 42 patients had a negative Dix-Hallpike test, and after a second manoeuvre, 8 of the remaining 9 patients had a negative Dix-Hallpike test (testing was conducted 1 to 2 weeks after the PRM was performed). Therefore, the overall short-term success rate after two manoeuvres was 50 of 51 patients (98%), which is sim

HYPOTHESIS AND BACKGROUND: Quantitative caloric testing is considered to be one of the most sensitive parameters in the diagnosis of peripheral vestibular disorders. In the past, because of limitations in the methods, the evaluation of the caloric response was restricted to mainly lateral semicircular canal functions. In this study, the authors tried to extend caloric testing to the function of all semicircular canals by using three-dimensional (3-D) analysis techniques.
METHODS: The authors studied in seven normal subjects 3-D eye movement responses to air caloric of the right ear with the s…

BACKGROUND: Vestibular schwannoma Gamma Knife radiosurgery (GKRS) achieves high tumor control rates but is occasionally associated with the new onset of acute vestibular dysfunction. The potential association of vestibulopathy and radiation delivery to the semicircular canals (SCCs) during GKRS has not been explored.
METHODS: We performed a retrospective single-center study of 286 patients who underwent vestibular schwannoma GKRS. Baseline clinical, treatment, and dosimetric data were analyzed. The primary endpoint was the new onset of acute imbalance, vertigo, and/or dizziness within 7 days …

Sudden sensorineural hearing loss (SNHL) following non-otologic surgery is a rare but potentially devastating complication. We report a case of bilateral SNHL occurring after elective excision of a frontal bone osteoma under general anaesthesia. A 52-year-old woman with no prior hearing impairment developed profound bilateral hearing loss immediately on emergence from anaesthesia, with partial spontaneous improvement over hours and subsequent right-sided predominance with tinnitus. Imaging excluded ischaemic pathology. Audiometry demonstrated a transient bilateral noise-induced pattern, with p

OBJECTIVE: To establish a high-fidelity finite element method (FEM) model of the human inner ear and explore the biomechanical effects of superior semicircular canal dehiscence (SCD) on both cochlear and vestibular function.
METHODS: A detailed FEM model of the entire human ear was reconstructed from high-resolution computed tomography (CT) data. The model was validated through comparison with established experimental data, including basilar membrane (BM) displacement patterns, cochlear tonotopy, inner ear impedance, and middle-ear transfer function. After validation, the model was adapted to

BACKGROUND Superior semicircular canal dehiscence is an inner-ear pathology which presents with vertigo, disequilibrium, and hearing loss. Although the exact etiology of superior semicircular canal dehiscence is unknown, it is thought that an increase in middle-ear pressure disrupts a thin overlying temporal bone. Superior semicircular canal dehiscence is frequently seen in association with dehiscence of the tegmen tympani, which overlies the middle ear. Here, we present a case report of a 52-year-old Puerto Rican man with vertigo, dizziness, vomiting, and mild hearing loss associated with sup

The tectorial membrane is crucial in the physiology of the auditory neuroepithelium. Mutations in one of its functional molecules, α-tectorin, lead to autosomal dominant and recessive congenital mid-frequency, non-syndromic hearing loss.Typically, α-tectorin mutations are not accompanied by any morphological abnormalities of the labyrinth. For the first time, we present a case of a toddler boy with congenital hearing loss due to TECTA gene mutation and concomitant bilateral dilation of the lateral semicircular canals. The expression of glycoproteins, like α-tectorin, varies between the distinct…
