"Just Right" Perceptions and Obsessive-Compulsive Symptoms in Tourette Syndrome and Chronic Tic Disorder: Implications for Tourettic Obsessive-Compulsive Disorder.

OBJECTIVES: Perceptions that something is not "just right" and feelings of incompleteness are forms of premonitory urges that extend beyond simple bodily sensations; they involve subjective interpretations of those sensations and may therefore be regarded as mental phenomena. These phenomena may precede tics but can also trigger compulsive behaviors. Investigating these phenomena is essential for understanding intermediate clinical phenotypes between tic disorders and obsessive-compulsive disorder (OCD). METHODS: We retrospectively reviewed the medical records of 71 patients older than 10 yea

Hypoalgesia and recovery in methylmercury-exposed rats.

Methylmercury (MeHg), the causal substrate in Minamata disease, can lead to severe and chronic neurological disorders. The main symptom of Minamata disease is sensory impairment in the four extremities; however, the sensitivity of individual sensory modalities to MeHg has not been investigated extensively. In the present study, we performed stimulus-response behavioral experiments in MeHg-exposed rats to compare the sensitivities to pain, heat, cold, and mechanical sensations. MeHg (6.7 mg/kg/day) was orally administered to 9-week-old Wistar rats for 5 days and discontinued for 2 days, then ad

On the functional anatomy of the urge-for-action.

Several common neuropsychiatric disorders (e.g., obsessive-compulsive disorder, Tourette syndrome (TS), autistic spectrum disorder) are associated with unpleasant bodily sensations that are perceived as an urge for action. Similarly, many of our everyday behaviors are also characterized by bodily sensations that we experience as urges for action. Where do these urges originate? In this paper, we consider the nature and the functional anatomy of "urges-for-action," both in the context of everyday behaviors such as yawning, swallowing, and micturition, and in relation to clinical disorders in wh

Analysis of feeding and eating disorders in 191 children according to psychiatric or gastroenterological recruitment: The PEDIAFED cohort study.

OBJECTIVE: The DSM-5 classification introduced new Feeding and Eating Disorders (FED) diagnostic categories, notably Avoidant and Restrictive Food Intake Disorder (ARFID), which, like other FED, can present psychiatric and gastrointestinal symptoms. However, paediatric clinical research that focuses on children below the age of 12 years remains scarce. The aim of this study was first to investigate the clinical features of FED in a cohort of children, second to compare them according to their recruitment (gastroenterology or psychiatry unit). METHOD: This non-interventional retrospective coho

Psychosensory symptoms in combat veterans with posttraumatic stress disorder.

Psychosensory symptoms have relevance to the study of chronic posttraumatic stress disorder (PTSD), given that their presence is associated with limbic system dysfunction and that several features of chronic PTSD suggest that it, too, may be associated with limbic dysfunction. The Iowa Interview for Partial Seizure-like Symptoms (IIPSS), a measure of psychosensory symptoms, was administered to a PTSD group and a comparison group. The PTSD group generated significantly higher IIPSS scores than did the other group. Within the PTSD group, higher IIPSS scores were associated with significantly mor

Cutaneous and mucosal pain syndromes.

The cutaneous and mucosal pain syndromes are characterized by pain, burning sensation, numbness or paraesthesia of a particular part of the skin or mucosal surface without any visible signs. They are usually sensory disorders, sometimes with a great deal of psychologic overlay. In this article various conditions have been listed and are described. The possible causative mechanisms are discussed when they are applicable and the outline of their management is described.

Genomic structural equation study reveals links between anorexia nervosa and delay discounting and lack of perseverance but not other facets of impulsivity.

Anorexia nervosa (AN) is a heritable condition, characterized by a fear of weight gain and a distorted body image, for which treatments are only limited. AN is characterized by excessive control over feeding behaviors, which has been hypothesized to indicate that low impulsivity, including low emotional impulsivity (urgency), may place certain individuals at risk for AN; however, this has not been fully genetically evaluated. We used genomic structural equation modeling and genome-wide association studies (GWASs) based on individuals of European ancestry (n = 72,517-903,147) to examine the lat

Defecatory disorders in patients with type 1 diabetes and chronic constipation.

BACKGROUND AND OBJECTIVES: Chronic constipation (CC) may be caused by defecatory disorders (DDs) and associated with reduced rectal sensation. Among patients with type 1 diabetes (T1D) and CC (T1DCC patients), the prevalence of DDs and reduced rectal sensation is unknown. We sought to compare complications of T1D, anorectal dysfunction, and CC symptoms, among T1DCC patients with versus without a DD. METHODS: Anorectal pressures at rest and during squeeze and evacuation, as well as rectal sensation and rectal balloon expulsion time (BET) were measured with high-resolution anorectal manometry i

Sensory processing profiles and their association with tic severity in children with tic disorders: a cross-sectional study.

BACKGROUND: Sensory symptoms have been increasingly recognized as core characteristics of tic disorders (TD). This study aimed to systematically investigate the core features of sensory processing abnormalities in Chinese children with tic disorders, analyze their association with tic severity, and explore potential subtypes based on sensory profiles. METHODS: This cross-sectional study recruited 151 children diagnosed with TD (encompassing Tourette syndrome, chronic motor tic disorders, and provisional tic disorders) along with 100 age-matched healthy controls (HC). [Deglutition disorders].

Dysphagia is related to the impairment of food passage from the mouth to the stomach. Globus pharyngis implies the frequent and often painful sensation of a lump in the throat that usually does not interfere with swallowing and may even be relieved by food intake. The diagnosis is based upon a careful history, clinical examination, endoscopy, dynamic imaging (videofluoroscopy, cinematography, videosonography) and electrophysiologic procedures (including pharyngoesophageal manometry, electromyography and pH determinations). Structural lesions of the cervical spine such as diffuse idiopathic ske

Restless head syndrome: A retrospective study.

BACKGROUND: Restless legs syndrome (RLS) is characterized by an urge to move with an unpleasant sensation in the lower limbs. RLS typically affects the legs. However, it can also affect several other body regions, such as the arms, abdomen, face, neck, head, and genital area. There are only a few reports of the RLS variant affecting the head. AIM: To assess the epidemiological, clinical, and other aspects of the RLS variant affecting the head. METHODS: We conducted a retrospective study of 17 adult patients (> 18 years) who met the RLS criteria and simultaneously experienced RLS-like symptom

Exploration of sensory impairments associated with C6 and C7 radiculopathies.

BACKGROUND CONTEXT: Cervical radiculopathy is a common disorder caused by compression of the cervical nerve roots and is characterized by arm pain and altered sensory-motor function. Incongruity in the locations of C6 and C7 dermatomes in competing versions of historical dermatome maps has plagued interpretation of impaired sensation associated with C6 and C7 radiculopathies. Magnetic resonance imaging (MRI) allows accurate identification of the C6 or C7 nerve root compression and therefore makes it possible to explore sensory findings that are associated with compression of specific nerve roo

There might be a distinctive clinical phenotype of constipation with non-cardiac chest pain which responds to combination laxatives: A retrospective, longitudinal symptom analysis.

Proton pump inhibitor (PPI)-refractory non-cardiac chest pain (NCCP) is often resolved when constipation was relieved. This study aimed to investigate the clinical features of patients with both NCCP and constipated functional bowel disorders (FBD).Among 692 consecutive patients diagnosed with functional constipation or irritable bowel syndrome with constipation and underwent anorectal manometry (ARM) in our hospital, PPI-refractory NCCP was present in 37. The clinical course of various torso symptoms including NCCP and ARM findings were retrospectively evaluated. The mean age was lower in the

Clinical and biochemical footprints of inherited metabolic disease. V. Cerebral palsy phenotypes.

Cerebral palsy is the most common physical disability of childhood describing a heterogeneous group of neurodevelopmental disorders that cause activity limitation, but often are accompanied by disturbances of sensation, perception, cognition, communication and behavior, or by epilepsy. Inborn errors of metabolism have been reported in the literature as presenting with features of cerebral palsy. We reviewed and updated the list of metabolic disorders known to be associated with symptoms suggestive of cerebral palsy and found more than 150 relevant IEMs. This represents the fifth of a series of

Trihexyphenidyl Misuse in Delusional Disorder.

Trihexyphenidyl is an anticholinergic medication that is routinely used for the management of extrapyramidal symptoms in patients who receive antipsychotic medications. Trihexyphenidyl has been reported to be abused by some patients, who start to take it in increasing doses and tend to report a sensation of relaxation or pleasure with this medication. Hence, whether trihexyphenidyl should be considered a psychoactive substance and whether nonprescription misuse of this medication should be considered under the purview of substance use disorders need further clarity. We present here two cases o

Chest muscle activity and panic anxiety: a preliminary investigation.

This report represents a pilot investigation of the role of chest muscle electromyographic (EMG) activity in developing panic episodes. Chest EMG activity was obtained as part of a larger study examining ventilatory differences between panic sufferers and normal controls. Frontalis EMG, heart rate, and minute ventilation (breathing rate and tidal volume) were also obtained during the study. The ventilatory procedure involved exposing the subjects to three periods of carbon dioxide gas inhalations (1%, 3%, 5%; balance oxygen). Subjective measures of frightening cognitions and body sensations we
