Case Report: Adult proximal humeral aneurysmal bone cyst: radical resection and reconstruction with osteoconductive allograft & reverse arthroplasty-Ecuador's first reported case and functional outcomes.

INTRODUCTION: Aneurysmal bone cysts (ABCs) are rare benign lesions, with adult cases being exceptionally uncommon. This report details a multidisciplinary approach for managing a proximal humeral ABC in an adult, emphasizing functional restoration. CASE PRESENTATION: A 36-year-old male presented with progressive pain, swelling, and restricted shoulder mobility. Imaging revealed an expansile lytic lesion with "soap-bubble" morphology. Histopathology confirmed a primary ABC. After failed conservative management, radical en bloc resection was performed, followed by single-stage reconstruction us

Inferior subluxation of the humeral head after trauma or surgery.

Inferior subluxation of the humeral head can occur after shoulder trauma or surgery. One hundred consecutive patients were evaluated prospectively after shoulder surgery or injury. The radiographic incidence of inferior subluxation of the humeral head 2 weeks after rotator cuff repair was 10%. The radiographic incidence of inferior subluxation after fracture of the proximal humerus was 42%, and the incidence 2 weeks after prosthetic humeral head replacement was 60%. The immediate postoperative radiograph showed an inferior subluxation of the humeral head in 4% of patients after prosthesis inse

Reverse total shoulder arthroplasty for massive irreparable rotator cuff tears in patients younger than 65 years old: results after five to fifteen years.

BACKGROUND: The role of reverse shoulder arthroplasty (RTSA) in the relatively young individual is currently unclear. Our study evaluates the midterm to long-term results of RTSA for patients aged younger than 65 years with pseudoparalysis secondary to massive irreparable rotator cuff tears, with or without arthritis. METHODS: Between 1997 and 2006, 46 RTSAs (41 patients) were performed. Mean age was 60 years (range, 46-64 years). At the latest follow-up, 5 patients had died and 1 was lost, leaving 35 patients (40 shoulders) with a mean follow-up of 93 months (range, 60-171 months). RESULTS:

Early morbidity and mortality after one-stage bilateral shoulder arthroplasty.

PURPOSE: One-stage bilateral shoulder arthroplasty has the advantage of requiring a single hospital stay and a single anaesthesia. The topic has been little reported, unlike one stage bilateral hip and knee arthroplasty, which have demonstrated their interest. The aim of the present study was to determine peri- and early post-operative morbidity and mortality after this procedure. The study hypothesis was that peri- and early post-operative morbidity and mortality in one stage bilateral shoulder arthroplasty is low in selected patients and that satisfaction is high. METHODS: A single-centre r

[Unreduced posterior luxations and fractures-luxations of the shoulder. Apropos of 30 cases].

Twenty-seven patients (30 shoulders) presenting with an unreduced posterior dislocation or fracture-dislocation were operated on and followe … ogether with derotation osteotomy. Lastly, one patient refused to be operated on and we performed a shoulder prosthesis to the last one. Necrosis of the humeral head (9 cases) appeared to be related to the severity of the lesion (type III and IV) and the type of surgical procedure (5 derotation osteotomy out of 6 and 3 open reductions out of 7) rather than to the app …

Learning curve of augmented reality surgical application for reverse shoulder arthroplasty.

BACKGROUND: Accurate positioning of the glenoid component in reverse total shoulder arthroplasty is important since it reduces prosthesis-related complications. Conventional instrumentation is still the gold standard. However, patient-specific instrumentation using 3-D printed guides, as well as computer-assisted navigation, is gaining more and more importance. Augmented reality has been established to enable the surgeon to have more precision in the positioning and inclination of the glenoid component. A new technique called "NextAR" (©Medacta) supports the surgeon during operation by guidanc

The treatment of proximal humerus nonunions in older patients with reverse shoulder arthroplasty.

INTRODUCTION: Experience with treating proximal humeral nonunions with reverse total shoulder arthroplasty is limited. We report our results. PATIENTS: Eighteen patients with proximal humeral atrophic nonunion were treated with reverse total shoulder prosthesis. There were 12 women and 6 men, with a mean age of 78.8 years (range, 75-84 years). The mean follow up after reverse arthroplasty was 28 months (range, 24-32 months). Extensive proximal humeral bone loss that was found in six cases was treated adding a proximal humeral allograft. A long humeral stem was always used. RESULTS: The mean …

First ray reconstruction with distraction osteogenesis.

Disarticulation of the thumb at the metacarpophalangeal joint level is not beneficial either aesthetically or functionally without additional surgery because it requires a bulky and an unacceptable prosthesis to be made for this amputation level. In this study, the authors have presented our experience of 12 metacarpal distractions in thumb amputated patients. Twelve male patients who had thumb amputation due to gunshot wounds were included in the study. Before the operation, aesthetic hand prostheses were made for 5 of the 12 patients. Callus distraction was performed with the use of a mini I

Effect of lateralization and distalization on tuberosity healing and functional outcomes after reverse total shoulder arthroplasty with the Univers Revers Total Shoulder System for proximal humerus fractures: a randomized controlled trial comparing 135° and 155° uncemented stems.

BACKGROUND: Controversy remains regarding whether humeral neck shaft angle influences tuberosity healing in reverse total shoulder arthroplasty for proximal humerus fracture (PHF). This study aimed to compare the tuberosity healing rates between reverse total shoulder arthroplasty using a lateralized and a distalized configuration of the Univers Revers Total Shoulder System for PHFs. The secondary objective was to evaluate functional outcomes based on stem inclination and tuberosity healing status. METHODS: This randomized controlled trial enrolled patients ≥65 years with a Neer 3- or 4-part

3D-Printed Metacarpal Prosthesis in the Treatment of Primary Osteosarcoma of the First Metacarpal: A Novel Surgical Technique.

OBJECTIVE: Osteosarcoma at the first metacarpal is extremely rare. Reconstructing the metacarpal after tumor resection is essential, as the thumb accounts for approximately 40%-50% of hand function. Although autografts, arthroplasty, and transposition have been reported as reconstruction options, their use is limited by complications such as secondary injury, nonunion, and displacement. In this study, we present a case of a patient with first metacarpal osteosarcoma who underwent tumor resection followed by reconstruction with a 3D-printed metacarpal prosthesis. We tend to introduce a novel st

Humeral offset as a predictor of outcomes after reverse shoulder arthroplasty.

INTRODUCTION: The Grammont-style reverse shoulder arthroplasty (RSA) relies on medialization and distalization of the shoulder center of rotation. Lateralized designs have recently gained popularity. The amount of lateralization, however, remains a controversial topic. The purpose of this study was to correlate the change in humeral offset (HO) with outcomes and complications following RSA. We hypothesized that a lateralized HO following RSA would be associated with improved range of motion (ROM), better patient-reported outcomes (PROs), and fewer complications. MATERIALS AND METHODS: A conse

Axillary artery thrombosis after humeral resurfacing arthroplasty.

Shoulder arthroplasty is a very successful procedure that places significant technical demands on the surgeon. Complications, such as neural injury and postoperative fracture, have been reported in the literature. In this article, I describe 2 cases of axillary artery thrombosis that occurred after humeral resurfacing arthroplasty. Case 1 involved a 59-year-old woman who underwent humeral resurfacing arthroplasty. In the immediate postoperative period, a vascular insult was diagnosed by decreased radial pulse and perfusion. Immediate retrograde angiography revealed occlusion of the axillary ar

Humeral stem with low filling ratio reduces stress shielding in primary reverse shoulder arthroplasty.

PURPOSE: Some patients show severe humeral bone stress shielding even one year after press-fit humeral stem, which can be a risk factor for implant durability. This study compared humeral stress shielding and clinical outcomes between high and low filling ratio (HFR and LFR) stems in primary reverse shoulder arthroplasty (RSA). METHODS: From 2015 to 2020, 104 patients who underwent RSA with a non-cemented standard-length humeral stem were examined. The humeral stems included large press-fit stems (HFR group, 53 patients) or small non-press-fit stems with autogenous cancellous bone grafting (L

Lateralized glenosphere reverse shoulder arthroplasty: inlay and onlay designs have similar clinical outcomes in patients with glenohumeral osteoarthritis.

BACKGROUND: Whether or how the position of the humeral tray (inlay or onlay) in reverse shoulder arthroplasty (RSA) affects outcomes is unclear. Our goal was to compare the clinical and radiographic results of RSA systems with inlay vs. onlay designs but with similar neck shaft angles (NSAs) and lateralized glenospheres. METHODS: We screened the institutional database at our tertiary academic center for patients who underwent primary RSA (with a lateralized glenosphere and a 135° NSA) from 2009 through 2017. The indication for surgery was glenohumeral osteoarthritis with glenoid bone loss (Wa

Management of intraoperative posterior decentering in shoulder arthroplasty using anteriorly eccentric humeral head components.

BACKGROUND: Posterior humeral decentering presents a challenge in glenohumeral arthroplasty. Soft tissue releases and osteophyte resection can lead to intraoperative decentering not evident preoperatively. Inferior outcomes result if decentering is not addressed as a part of the arthroplasty. When there is >50% posterior subluxation of the humeral head on passive elevation of the arm at surgery, we have used an anteriorly eccentric humeral head component to improve centering of the humeral articular surface on the glenoid. METHODS: We reviewed the 2-year outcomes for 33 shoulder arthroplastie

Implant sonication versus intraoperative tissue sample cultures for Periprosthetic Joint Infection (PJI) of Shoulder Arthroplasty.

Introduction Periprosthetic joint infection (PJI) is the most problematic complications after shoulder arthroplasty. Many diagnostic tools have been identified to find infection, such as hystopatologic examination of tissue sections or cultures of intraoperative tissue. Implant sonication fluid culture showed good results in order to enhance diagnostic accuracy, but literature results are still controversial. Aim of our study is to compare the results of sonication with intraoperative tissue sample cultures. Patients and Methods From February 2016 to January 2018 we performed 102 revisions of
