This study aimed to examine the prevalence of molar roots protruding into the maxillary sinus and to determine the panoramic radiographic signs as correlated with cone beam computed tomography (CBCT). CBCT images of 354 roots were assessed and classified into three types, according to the relationship between the root and maxillary sinus. The prevalence of root protrusion into the maxillary sinus was calculated then the panoramic images assessed. After excluding some unidentified roots on the panoramic images, 200 were investigated for panoramic signs, including (i) projection of the root apex

BACKGROUND: In sinus augmentation, when remaining bone height is ≤5 mm, a lateral window approach is often the preferred choice; nonetheless, patients prefer to have a less invasive approach such as crestal sinus augmentation (CSA). Prior case reports have described the use of various staged approaches of a CSA technique in cases of limited bone height. The aim of this report was to describe the results of a case series in which a two-stage CSA technique was used in patients with 4 to 6 mm of bone height.
METHODS: Nineteen subjects with 28 sinuses of initial vertical bone height of 4 to 6 mm

Several systematic reviews have shown that maxillary sinus augmentation is a predictable and effective procedure for augmentation of an atrophic posterior maxilla. However, we know of no reviews that have covered all the clinical aspects. We searched the PubMed, EMBASE, Cinhal, and Cochrane databases up to January 2015 to select relevant studies that cover the different objectives of this review, including the anatomy of the maxillary sinus, surgical techniques, biomaterials used in the sinus augmentation, and potential complications.

Maxillary sinus augmentation is now considered a routine procedure; however, it can rapidly become a challenging surgery susceptible to multiple complications. It is widely acknowledged that lack of preparation or inadequate technical expertise is a primary cause of such complications. This procedure can become a source of significant stress for the clinician and morbidity for the patient if not properly planned preoperatively. Despite this, insufficient attention is often paid to the physiological, psychological, and environmental factors that may influence unforeseen and adverse surgical out

Sinus floor elevation (SFE) by transcrestal approach has been proven to be a predictable and minimally invasive treatment that augments posterior maxilla with insufficient bone height, allowing the prosthetic rehabilitation of this area with dental implants. However, precise and sufficient elevation of the Schneiderian membrane without perforation is challenging through this blind technique especially in the presence of anatomical restrictions. This note describes a novel technique combining a surgical template and an absorbable collagen sponge (ACS) strip for transcrestal SFE in the oblique s

The aim of this paper was to search for reported cases of sinus infection following reduction malarplasty and present guidelines for the prevention of sinusitis. Two cases of maxillary sinusitis that developed after reduction malarplasty has been reported, which were treated with endoscopic sinus surgery. Histologically, thickness of the mucosal lining of the maxillary sinus (Schneiderian membrane) was 0.41 mm at sinus floor, and 0.38 mm at 2 mm above the floor. In functional endoscopic sinus surgery (FESS), the uncinate process is removed, exposing the hiatus semilunaris. The anterior ethmoid

The aim of this case report was to present the preliminary results of a novel microsurgical approach to sinus floor elevation and bone augmentation. This technique was used to treat four patients in whom an implant could not be placed in the maxillary first molar position because of insufficient bone height. The maxillary first molar was extracted, and a sinus access window was created in the palatal area of the bony interradicular septum. The sinus membrane with the palatal septum fragment was elevated, and the sinus space between and above the roots was filled with xenograft. Alveolar preser

Maxillary sinus grafting is generally a safe procedure. However, intraoperative complications, as well as early and late postoperative complications, may occur. Included in the latter group are graft infections that can be triggered by peri-implantitis. The aim of the present study was to report three cases of late maxillary sinus graft infections and to histologically evaluate the effects of peri-implantitis in the grafted area. In peri-implantitis cases in grafted sinuses, the sole removal of the implant along with accompanying debridement of the infected area may not be sufficient to resolv

OBJECTIVES: Sinus augmentation is frequently used to maintain implant stability when there is severe alveolar bone loss. The aim of this study was to determine the thicknesses and histologic features of the sinus lateral wall and Schneiderian membrane in embalmed cadavers.
DESIGN: This study included 35 hemimaxillae from 25 cadavers (19 males and 6 females with a mean age at death of 59 years). Specimens obtained from the first premolar to the second molar were embedded in paraffin, stained with hematoxylin-eosin, and observed under a light microscope. The thicknesses of the lateral wall and

We report the case of a patient presenting with severe headache and progressive aphasia due to a large brain abscess. End-stage peri-implantitis on a dental implant in the upper jaw with contact to the maxillary sinus floor causing maxillary and frontal sinusitis could be identified as the source of infection. Multidisciplinary treatment included broad-spectrum antibiotics, craniotomy for abscess drainage, sinus surgery, and the removal of the implant. Despite all the care taken, the patient died, underlining the importance of oral hygiene and a frequent recall in patients with dental implants

This retrospective analysis assessed a possible correlation of perforation of the maxillary sinus floor during apical surgery of maxillary molars and the distances from the treated apices/periapical radiolucencies to the sinus floor. The material included 168 apical surgeries of maxillary first or second molars performed from 1999 to 2016. In 22 (out of 33) perforation cases, a preoperative cone beam computed tomography (CBCT) was available. These cases were defined as the test group. From the pool of operated cases without sinus floor perforation, matching cases - that is, same treated tooth,

This technical note introduces a novel instrument to facilitate lateral window sinus membrane elevation. The fabrication and use of the instrument are reported. The features of this instrument include easy control, good tactile sensation, flexibility in adjusting the instrument, and simple to replicate. Since its introduction, it has been the preferred instrument of our implant surgical trainees. Most importantly, it has proved to be an invaluable teaching tool, as it has helped not only to boost trainee confidence in the procedure, but also in their use of other sinus elevation instruments.

PURPOSE: To analyze risk factors affecting sinus membrane perforation (SMP) during sinus floor elevation (SFE) procedures using the lateral window technique (LWT).
MATERIALS AND METHODS: For patients with SFEs using the LWT, patient-related risk factors (age/sex/smoking/diabetes) and surgical-anatomical-related risk factors (stage approach/sinus side/residual ridge height/sinus membrane thickness/previous surgical interventions) were compared between perforated and nonperforated sites and were evaluated for their influence affecting SMP. Additionally, SMPs were further subdivided into small/m

Sinus floor augmentation (SFA) is the most predictable treatment option in the atrophic posterior maxilla. However, exposure of the apical implant body into the maxillary sinus cavity is an occasionally observed phenomenon after SFA. Although most penetrating dental implants remain completely asymptomatic, they may induce recurrent rhinosinusitis or implant loss. Removal of the implant should be considered if there is significant implant exposure that results in prolonged treatment and increased costs. This case report demonstrates a recovery approach using sinus floor reaugmentation without i…

Several systematic reviews and many clinical articles have shown that maxillary sinus floor elevation through lateral window approach (LSFE) is a predictable and reliable procedure, which has been routinely performed for more than 30 years. The complication rate is low, but some cases may require additional surgery, and the outcome of implant may be affected if not treated properly. The survival rates of implants are still high after early diagnosis and proper early treatments. The purpose of this article is to discuss the intraoperative and postoperative complications of LSFE, including perfo
