[The uvulopalatopharyngoplasty operation for obstructive sleep apnea syndrome].

We performed uvulopalatopharyngoplasty operations on 216 patients and conducted 6-month to 2-year follow-ups. 40 patients have had polysomnograms before and after operations. The clinical results showed 50 percent markedly improved, 28 percent somewhat improved, and 22 percent unimproved. The total success rate was 78%. The surgical procedures and polysomnogram methods have been introduced, and the pathogenesis, diagnostic criteria and results were discussed.

Obstructive sleep apnea syndrome in acid maltase deficiency.

A patient with adult acid maltase deficiency (AMD) developed severe obstructive sleep apnea (OSA) and respiratory insufficiency. Weaning failure was followed by diffuse pneumonia and death. At autopsy, profound muscle replacement by fibrofatty tissue was noted in the tongue and diaphragm, while the accessory and nonrespiratory muscles were variably preserved. To our knowledge, this case represents the first detailed clinical description of OSA associated with adult AMD. In addition, we suggest that severe tongue weakness due to fatty metamorphosis, in concert with macroglossia, provides a uniq

[Obstructive sleep apnea syndrome caused by craniofacial abnormalities].

Patients with obstructive sleep apnoea syndrome frequently present distinct changes in the craniofacial skeleton that result in a structural narrowing of the pharynx and promote pharyngeal collapse during sleep. The evaluation of the clinical data of patients with and without craniofacial dysmorphia reveals that mean age, mean Broca index and the number of additional diseases is decreased in patients with craniofacial dysmorphia. Young patients or patients with normal weight and OSAS usually present such skeletal changes. The number of sleep-related breathing disorders and the extent of noctur

Surgical treatment of obstructive sleep apnea. Technical variations.

OBJECTIVE: To determine the effectiveness of the two basic surgical types of uvulopalatopharyngoplasty (UPPP), as classified by us. The two basic surgical variations of UPPP differ for the width of the anteroposterior palatal resection, being 1 to 2 cm (type 1) and 0.5 to 1.0 cm (type 2). DESIGN: Randomized, retrospective study. PATIENTS: Eighty-seven patients with obstructive sleep apnea hospitalized in our department from 1983 to 1991. All patients had a complete preoperative and postoperative polysomnographic evaluation. RESULTS: The operation was found to be an effective treatment of ob

Obstructive sleep apnea in patients with pharyngeal tumours.

Six consecutive patients with pharyngeal tumours were investigated regarding disordered breathing during sleep. In addition to pulse oximetry, the investigations involved oral and nasal airflow as well as thoracic respiratory movements in four of the patients in whom the oxygen desaturation index (ODI, number of desaturations per hour of estimated sleep) was on average 24 (range 10-58). An almost total predominance of obstructive apnea was found in evaluated cases. In the 2 cases in whom only pulse oximetry was performed, numerous desaturations were found throughout the night (ODI 19 and 88, r

Obstructive sleep apnea in children with Down syndrome.

Children with Down syndrome have many predisposing factors for the obstructive sleep apnea syndrome (OSAS), yet the type and severity of OSAS in this population has not been characterized. Fifty-three subjects with Down syndrome (mean age 7.4 +/- 1.2 [SE] years; range 2 weeks to 51 years) were studied. Chest wall movement, heart rate, electroculogram, end-tidal PO2 and PCO2, transcutaneous PO2 and PCO2, and arterial oxygen saturation were measured during a daytime nap polysomnogram. Sixteen of these children also underwent overnight polysomnography. Nap polysomnograms were abnormal in 77% of c

[Dentofacial morphology of obstructive sleep apnea syndrome patients].

Sleep apnea syndrome is a condition characterized by recurrent interruption of breathing during sleep. Triad of symptoms for the disease are insomnia, daytime sleepiness and snoring. Recently, the patients complained of these symptoms have progressively increased. And so serious attention has been given to investigate the entity of this new clinical syndrome in medical and dental aspects. Three types of sleep apnea are classified; central, obstructive and mixed type. Most of patients identified this syndrome include obstructive or mixed types of sleep apnea. Obstructive sleep apnea has been pr

Cephalometric airway analysis in obstructive sleep apnea syndrome.

In obstructive sleep apnea syndrome, polysomnograms characterize the nature and severity of the disorder but, rarely, its specific site. Therefore, 90 patients with documented obstructive sleep apnea were evaluated by cephalometric technique, with special attention paid to the size and position of the soft palate and uvula, volume and position of the tongue, mandibulo-maxillary relationship, hyoid position, and size of the pharyngeal airway space. Results showed major differences between control patients and patients with obstructive sleep apnea in over 50% (30/52) of the objectively measured

Symptomatic response to CPAP in obstructive sleep apnea versus COPD- obstructive sleep apnea overlap syndrome: Insights from a large national registry.

BACKGROUND: The symptomatic response to continuous positive airway pressure (CPAP) therapy in COPD-obstructive sleep apnea overlap syndrome (OVS) compared to OSA syndrome (OSA) alone has not been well studied so far. The aim of this study is to explore main differences in the clinical response to CPAP treatment in OVS compared to OSA alone. STUDY DESIGN AND METHODS: Using prospective data from the French National Sleep Apnea Registry, we conducted an observational study among 6320 patients with moderate-to-severe OSA, available spirometry, and at least one follow-up visit under CPAP therapy.

Determinative sleep traits associated with dyslipidemia in obstructive sleep apnea patients.

BACKGROUND: Obstructive sleep apnea (OSA) is recognized to increase the risk of dyslipidemia; however, the specific sleep traits in OSA that influence dyslipidemia are poorly understood. This study sought to determine which sleep traits are independently associated with dyslipidemia and serum lipid profiles in patients with OSA. METHODS: In this cohort study, 5239 participants were included from the Sleep Heart Health Study. Further, OSA was diagnosed via polysomnography with an AHI ≥ 5 events/h. Sleep traits were assessed using polysomnographic data and questionnaires. Then, logistic regress

Comparison of awake respiratory control versus sleep obstructive sleep apnea endotypes.

Most approaches to advance simplified physiology-based precision medicine strategies for obstructive sleep apnea (OSA) focus on sleep parameters (i.e., OSA endotypes). However, wakefulness physiology measures can also provide prediction insight for certain OSA therapies, yet their relationship with sleep parameters has not been extensively investigated. This study aimed to investigate potential relationships between awake ventilatory control parameters and sleep OSA endotypes and their potential to predict changes in OSA severity with morphine. Data were acquired from a randomized, crossover t

Drug-Induced Sleep Endoscopy in Children With Positional Obstructive Sleep Apnea.

OBJECTIVE: Positional obstructive sleep apnea (POSA)-defined as obstructive sleep apnea twice as severe supine than nonsupine-may offer clues to the underlying pattern of upper airway collapse in children. We compared drug-induced sleep endoscopy (DISE) findings in children with and without POSA. We hypothesized that children with POSA would have significantly higher obstruction at the gravity-dependent palate and tongue base but not at the adenoid, lateral wall, or supraglottis. STUDY DESIGN: Retrospective case series. SETTING: Tertiary pediatric hospital. SUBJECTS AND METHODS: We included

Marked reduction in obstructive sleep apnea severity in slow wave sleep.

INTRODUCTION: Obstructive sleep apnea (OSA) is widely accepted to improve during slow wave sleep (SWS) compared to lighter stages of NREM sleep. However, supporting data to establish the magnitude and prevalence of this effect is lacking. Consequently, we examined this phenomenon, controlling for posture, in a large group of patients investigated for OSA at an academic clinical sleep service. METHODS: A detailed retrospective analysis was conducted on data obtained from each 30-sec epoch of sleep in 253 consecutive full-night diagnostic polysomnography studies performed over a 3-month period.

Prevalence of concomitant sleep disorders in patients with obstructive sleep apnea.

We determined the prevalence of concomitant sleep disorders in patients with a primary diagnosis of obstructive sleep apnea (OSA). We retrospectively analyzed 643 patients, aged > or =18, with a primary diagnosis of OSA, evaluated by sleep specialists, in whom clinical and polysomnographic data were derived using standardized techniques by reviewing data from a standardized database and clinical charts. Concomitant sleep disorders were listed according to the International Classification of Sleep Disorders (American Academy of Sleep Medicine, 2000). The mean age was 48.5+/-13.5 years and 55% w
